Book excerpt: L'objectif principal de cette étude est d'évaluer l'atteinte des finalités du service sanitaire des étudiants en santé en 2018/2019 dans les Hauts-de-France. L'objectif secondaire est d'apporter des éléments de réflexion à l'amélioration du SSES. Il s'agit d'une étude mixte menée auprès des responsables de terrain de stage ayant accueilli des étudiants en santé dans le cadre du service sanitaire en 2018/2019. 538 responsables de structures d'accueil ont répondu à l'enquête et 33 entretiens ont été réalisés. Nous observons donc une mise en place importante du SSES dans l'ensemble des formations en santé avec une bonne perception des enjeux de santé publique et de la prévention primaire, la réalisation d'actions concrètes de prévention primaire auprès de public cible en inter professionnalité et pluridisciplinarité. Les résultats de cette étude mettent en évidence un dispositif dont les objectifs ont été atteints à des degrés variables dans la région des Hauts-de-France. Les difficultés rencontrées ne sont pas spécifiques à la région et s'expliquent en partie par la première année de mise en place. La mise en place du service sanitaire dans la formation initiale des étudiants en santé a permis de les sensibiliser aux enjeux de la prévention primaire et de réaliser des actions concrètes de prévention primaire en inter professionnalité et pluridisciplinarité. Il convient d'encourager et de poursuivre cette dynamique sur l'ensemble du territoire et pour toutes les formations en santé dans un objectif de changement des pratiques professionnelles.

Il permet aujourd'hui, chaque année, à plus de 50000 étudiants en santé d'effectuer des actions de prévention et promotion de la santé sur l'ensemble du territoire national et sur de nombreuses thématiques. Néanmoins la rapidité de mise en place exigée par les textes, et l'envergure de la mise en place sans réelles études de faisabilité préalable impose des évaluations a posteriori pour connaître la pertinence du dispositif en vie réelle. Une enquête a été réalisée afin de recenser et évaluer la pertinence des actions menées par les associations d'étudiants en santé au regard des bonnes pratiques de prévention et de promotion de la santé. Parmi les nombreuses actions de santé publique recensées, trois actions de santé publique permettant de valider le SSES ont pu être analysées au cours de cette enquête. Malgré des responsables associatifs toujours soucieux de faire de leur mieux, des écarts aux bonnes pratiques sont constatés au niveau du diagnostic de terrain et de l'évaluation des actions notamment. En particulier, l'évaluation des actions existe dans chacun des projets mais parfois plus proche d'un questionnaire de satisfaction que d'une réelle évaluation de la montée en connaissances et/ou compétences du public cible. Les résultats de cette étude, ainsi que ceux d'autres études et référentiels de données probantes suggèrent des améliorations possibles et au niveau de la formation des étudiants et de l'accompagnement des associations proposant des actions de santé publique validantes pour le service sanitaire des étudiants en santé.

It is now widely recognized that human resources for health (HRH) are a key enabler for the attainment of universal health coverage, and for the achievement of Sustainable Development Goal (SDG) 3 - Ensure healthy lives and promote well-being for all at all ages. As is stressed in the Global Strategy on Human Resources for Health: Workforce 2030, there can be no viable national, or global, health system without an effective health workforce. The Global Strategy, adopted at the Sixty-ninth World Health Assembly in May 2016, challenges the erroneous narrative of health workers as a unit of cost in the production of health. The evidence instead presents an intersectoral agenda on the pre-condition of equitable access to health workers in the attainment of universal health coverage, along with a dynamic labour market understanding of the substantive impact on education, employment, jobs and innovation in the health and social care economy. The Global Strategy, therefore, enables governments and other relevant stakeholders to adopt a holistic, rather than fragmented, approach to ensuring that the health workforce contributes both to improved health and to broader socioeconomic development.

This discerning insight, which dates back more than 2000years to Aristotle, seems to have been ignored by most psycholo gists. For more than 40years theories of human action have assumed that cogni tion and action are merely two sides of the same coin. Approaches as different as S-O-R behaviorism,social learning theory, consistency theories,and expectancy value theories of motivation and decision making have one thing in common: they all assume that "thought (or any other type of cognition) can move any thing," that there is a direct path from cognition to behavior. In recent years, we have become more and more aware of the complexities in volved in the relationship between cognition and behavior. People do not always do what they intend to do. Aside from several nonpsychological factors capable of reducing cognition-behavior consistency, there seems to be a set of complex psychological mechanisms which intervene between action-related cognitions, such as beliefs, expectancies, values, and intentions,and the enactment of the be havior suggested by those cognitions. In our recent research we have focused on volitional mechanismus which presumably enhance cognition-behavior consistency by supporting the main tenance of activated intentions and prevent them from being pushed aside by competing action tendencies.
