The early science of chemistry and microbiology were the source of most drugs until the revolution of genetic engineering in the mid 1970s. Then biotechnology made available novel protein agents such as interferons, blood factors and monoclonal antibodies that have changed the modern pharmacy. Over the past year, a new pharmacy of oligonucleotides has emerged from the science of gene expression such as RNA splicing and RNA interference. The ability to design therapeutic agents from genomic sequences will transform treatment for many diseases. The science that created this advance and its future promise will be discussed. Phillip Allen Sharp is an American geneticist and molecular biologist who co-discovered RNA splicing. He shared the 1993 Nobel Prize in Physiology or Medicine with Richard J. Roberts for “the discovery that genes in eukaryotes are not contiguous strings but contain introns, and that the splicing of messenger RNA to delete those introns can occur in different ways, yielding different proteins from the same DNA sequence. He works in Institute Professor Koch Institute for Integrative Cancer Research, Massachusetts Institute of Technology (MIT), Cambridge, MA, US. Beck published the now classic "Cognitive Therapy of Depression," and Michael J. Mahoney declared the "Cognitive Revolution," much has happened. What was proposed as the "cognitive revolution" has now become the zeitgeist, and Cognitive Therapy (CT) has grown exponentially with each passing year. A treatment model that was once seen as diffe rent, strange, or even alien, is now commonplace. In fact, many people have allied themselves with CT claiming that they have always done CT. Even my psychoanalytic colleagues have claimed that they often use CT. "After all," they say, "Psychoanalysis is a cognitive therapy." Cognitive Therapy (or Cognitive Psychotherapy) has become a kaleidoscope model of treatment, with influences coming from many sources. Some of these contributory streams have been information pro cessing, behavior therapy, Constructivist psychology, and dynamic psychotherapy. Each of these sources have added color, shading, and depth to the CT model. What was originally uni dimensional in terms of the CT focus on depression has become multidimensional as the CT model has been applied to virtually every patient population, treatment setting, and therapy context. CT must now be seen as a general model of psychotherapy that, with modifications, can be applied to the broad range of clinical problems and syndromes. What has tied these various applications of CT together is the emphasis on a strong grounding in cogni tive theory, a commitment to empirical support, and a dedication to broadening the model.

Why do researchers exclude patients with primary progressive multiple sclerosis from enrolling in clinical trials? Please let me know if you hear of studies that I might be allowed to enter or treatments that I could try for my condition. " Thus, in recent years, the sad lament of the patient with primary progressive MS (PPMS). This variant, often in the guise of a chronic progressive myelopathy or, less commonly, progressive cerebellar or bulbar dysfunction, usually responds poorly to corticosteroids and rarely seems to benefit to a significant degree from intensive immunosuppressive treatments. In recent years, most randomized clin ical trials have excluded PPMS patients on two counts. Clinical worsening devel ops slowly in PPMS and may not be recognized during the course of a 2-or 3-year trial even in untreated control patients. This factor alone adds to the potential for a type 2 error or, at the very least, inflates the sample size and duration of the trial. In addition, there is mounting evidence that progressive axonal degeneration and neuronal loss (rather than active, recurrent inflammation) may be important components of the pathology in this form of the disease. Although contemporary trials are evaluating whether PPMS patients may benefit from treatment with the ~-interferons and glatiramer acetate, preliminary, uncontrolled clinical experi ence suggests that the results may not be dramatic.
