Since the mid-1980s accidents have come to be seen as the consequence of complex interactions rather than simple threads of causes and effects. Yet progress in accident models has not been matched by advances in methods. The author's work in several fields (aviation, power production, traffic safety, healthcare) made it clear that there is a practical need for constructive methods and this book presents the experiences and the state-of-the-art. The focus of the book is on accident prevention rather than accident analysis and unlike other books, has a proactive rather than reactive approach. The emphasis on design rather than analysis is a trend also found in other fields. This bias towards performance failures leads to a neglect of normal performance. It assumes that failures and successes have different origins so there is little to be gained from studying them together. Erik Hollnagel believes this assumption is false and that safety cannot be attained only by eliminating risks and failures. The alternative is to understand why things go right and to amplify that. The ETTO Principle looks at the common trait of people at work to adjust what they do to match the conditions. It proposes that this efficiency-thoroughness trade-off (ETTO) is normal. While in some cases the adjustments may lead to adverse outcomes, these are due to the same processes that produce successes.

Covering the Coptic minority in Egypt; Yezidis in Sinjar in north-west Iraq; and, in Tunisia, the Black community in Djerba, Gabes and Sfax, the Jewish community in Djerba and the Amazigh community in Tatouine, the report identifies barriers to access to health care in a context of public health systems that have been weakened by Covid-19, as well as poor systems of governance and under-resourcing.

While the human factors of personal safety have been widely studied and widely adopted in many high-risk industries, process safety â€" the application of engineering, design, and operative practices to address major hazard concerns â€" is less well understood from a human factors perspective, particularly in the offshore oil industry. The National Academies of Sciences, Engineering, and Medicine organized a workshop in January 2018 to explore best practices and lessons learned from other high-risk, high-reliability industries for the benefit of the research community and of citizens, industry practitioners, decision makers, and officials addressing safety in the offshore oil industry. This publication summarizes the presentations and discussions from the workshop.

To scale up action to leave no one behind, WHO’s Fourteenth General Programme of Work, 2025-2028 (GPW 14) has an explicit and strong commitment to equity. In support of this, WHO has released a handbook with methods for the identification of barriers to effective coverage with health services. The 8-module handbook applies mixed method research approaches -- grounded in the Tanahashi framework for effective coverage -- to focus on barriers experienced by potential users and non-users of health services. The handbook facilitates the capturing of evidence on the interface between the population and the services. The handbook has four objectives. 1.To orient national authorities and partners on key concepts, definitions, frameworks and principles relevant to barriers assessments. 2.To build the capacity of national authorities and partners to design a research plan and apply diverse methods (e.g. informant interviews, literature reviews, quantitative analysis and focus groups) in barriers assessments. 3.To provide guidance for reporting on barrier assessment findings in a clear and actionable manner and convening key stakeholders to deliberate next steps. 4.To adapt the methods for humanitarian contexts, accounting for the need for differentiated approaches.

Book excerpt: The opioid crisis in the United States has come about because of excessive use of these drugs for both legal and illicit purposes and unprecedented levels of consequent opioid use disorder (OUD). More than 2 million people in the United States are estimated to have OUD, which is caused by prolonged use of prescription opioids, heroin, or other illicit opioids. OUD is a life-threatening condition associated with a 20-fold greater risk of early death due to overdose, infectious diseases, trauma, and suicide. Mortality related to OUD continues to escalate as this public health crisis gathers momentum across the country, with opioid overdoses killing more than 47,000 people in 2017 in the United States. Efforts to date have made no real headway in stemming this crisis, in large part because tools that already existâ€"like evidence-based medicationsâ€"are not being deployed to maximum impact. To support the dissemination of accurate patient-focused information about treatments for addiction, and to help provide scientific solutions to the current opioid crisis, this report studies the evidence base on medication assisted treatment (MAT) for OUD. It examines available evidence on the range of parameters and circumstances in which MAT can be effectively delivered and identifies additional research needed.
