Book excerpt: Background: Type 2 diabetes (T2D) is the leading cause of death in the U.S., and it incurs tremendous health expenditures associated with various complications due to poor diabetes control. Medication adherence, which is correlated with patients' health literacy, should be consistently practiced in order to achieve optimal diabetes control. However, no literature has systematically explored the multiple psychosocial and communication barriers and facilitators at the individual level that mediate the linkage between health literacy and medication adherence. A deeper understanding of the specific communication and psychosocial factors related to medication-taking behaviors across different levels of health literacy among people with T2D will guide the development of effective interventions and customized strategies to enhance medication adherence. Objectives: (1) To examine whether the barriers and facilitators associated with medication adherence differ among people with T2D across different levels of health literacy. (2) To explore patients' perceptions of the barriers and facilitators of medication adherence across different levels of health literacy. (3) To understand how the qualitative interview data complement the quantitative survey results about the barriers and facilitators of medication adherence. Methods: This mixed-methods study used an explanatory sequential design, including a quantitative study with a survey questionnaire followed by a qualitative study with semi-structured interviews. The Health Literacy Pathway Model was used to identify the psychosocial and communication factors that may influence medication adherence. The target population was patients who were 20 years of age or above, were diagnosed with T2D, understood English, and were prescribed at least one oral diabetes medicine daily. Analysis of covariance and direct content analysis were used respectively in quantitative and qualitative analysis. A joint display was used to present the integrated findings from quantitative and qualitative data. Results: In the quantitative phase, 205 participants provided complete information in the survey questionnaire. In the qualitative phase, 23 participants completed semi-structured interviews. Confirmed by quantitative and qualitative data, holding stronger self-efficacy, having fewer concern beliefs about medication, and possessing fewer perceived barriers to medication-taking are necessary for better medication adherence. Linking medication-taking to daily routine and focusing on the internal locus of control is an imperative approach to foster self-efficacy of medication use. Addressing patients' life experience and clarifying medication misinformation may help participants cope with their concerns with medications. Solving the high cost of medications and therapy-related problems could lessen participants' perception of barriers to medication-taking. Conclusion: To optimally support patients' diabetes care, practitioners should address concern beliefs among non-adherent patients with low health literacy, as well as emphasize self-efficacy and perceived barriers to medication adherence among all non-adherent patients with T2D.

Within this case study, we will share our experience of using a convergent mixed-methods design to investigate the extent of and barriers associated with medication nonadherence in Qatari primary care settings among patients with type-2 diabetes mellitus. Briefly, we captured quantitative data (prevalence of nonadherence and associated factors) using a validated questionnaire, and qualitative data (barriers to medication adherence) using one-to-one interviews with patients and health care providers. This case study also discusses the challenges encountered and practical lessons learned relevant to this experience.

Book excerpt: Introduction Medication adherence remains a problem among Type-2 diabetes (T2D) patients despite availability of effective treatments. Three analyses of extant data sets were conducted to examine barriers to using medication as prescribed as an alternate method to assess medication adherence: 1) development and psychometric evaluation of the Murage-Marrero-Monahan-Medication barriers (4M) scale to assess patients' perceived barriers; 2) patient demographic factors associated with barriers to using medication as prescribed, and 3) the association between patients' perceived barriers to medication use and cardiovascular disease (CVD) risk factor control. Methods Twelve focus groups and a cross-sectional study of 362 T2D patients contributed to develop and evaluate psychometric properties of the 4M scale. A cross-sectional survey of 964 T2D patients was used for the other two studies. Analysis of covariance identified demographic factors associated with reported barriers. Multivariable logistic regression was used to identify barriers associated with CVD risk factors (glucose, blood pressure and lipids) categorized as either poor or good control. Results Exploratory factor analysis with Varimax rotation resulted in a 19-item 4M scale with acceptable psychometric properties. As a five-domain (or single-domain) structure, coefficient alpha ranged from 0.70 to 0.83 (0.92). Both structures demonstrated discriminant validity and known-group validity. Age was inversely associated with all identified barriers while income was inversely associated with poor communication with providers and side effects. A unit increase in the overall barrier mean score on the 4M scale was associated with 92% increase in the odds of having poor control of two or more CVD risk factors compared to good control of all three risk factors (adjusted OR=1.92, 95% CI: 1.16 - 3.17; p

Despite the availability of medications and other treatment options, diabetes is still a challenging disease to control in part due to a lack of knowledge about treatment regimens thereby leading to inadequate adherence to medications. Evidence-based (EBP) project question: In African American adult ages 18 years and older with uncontrolled type 2 diabetes, does education increase knowledge of diabetes medication and adherence to diabetes medication?. Participants: African American adults with uncontrolled type 2 diabetes. Project Implementation: Private outpatient clinic in the suburbs of Largo Maryland for a period of 10 weeks duration. Each participant was followed for four weeks using a phone script to determine medication knowledge and compliance to prescribed treatment regimes. The 25 participants completed pretest and posttest questionnaires of the Morisky Green Levine Scale (MGLS) and the Diabetes Medication Knowledge. Results: Statistical data analysis revealed the percentage in means change for both outcomes to be 47% for MGLS and 49.5% for knowledge of diabetes questionnaire. This change in means scores surpassed the set benchmark of 25% for this project. Conclusion: The findings suggest that targeted education combined with sequential phone call follow-up, can be effective to impact increases in both individual and population health by controlling diabetes and delaying the development of diabetes complications. Although, a patient cannot be forced to adhere to their medication regimen, healthcare professionals can make sure that the patients make an educated decision. If healthcare providers assist patients with making an accurate decision when making decisions related to their healthcare and comply with their medication, management of DM will be better. The manner in which this assists is that it provides more information on this topic and improves education techniques that can be more comprehendible.
