Book excerpt: The overall objective of this proposal is to fully automate and optimize the performance of a 3-D dedicated emission mammotomography system for enhanced semi-automated clinical testing. A retrospective study of 103 clinical MRI uncompressed breast scans was conducted to create surface renderings of the uncompressed breasts and analyze how to adapt existing acquisition orbits for varying breast shapes. Laser ribbon ranging sensors and associated hardware to fully automate the radius of rotation were acquired and tested. An observer based 3D contrast-detail study was performed in an effort to evaluate the limits of object detectability for the dedicated CZT-based SPECT mammotomography imaging system under various imaging conditions. Other aspects of the training program were also initiated including clinical shadowing in the mammography, nuclear medicine, and breast oncology divisions at Duke University.

This novel system allows fully 3-D imaging of a pendant, uncompressed breast using novel 3-D complex orbit capabilities. A prototype system has been designed, developed, and extensively characterized with a number of figures of merit including SNR, dose efficiency, and contrast sensitivity. Results indicate the potential for sub dual-view dose uncompressed breast imaging. Observer studies have been performed to determine the lower limits of detectibility. Patient bed optimization has also been performed as well as system/bed positioning and tilt angles for optimal chest wall imaging and patient comfort. The outcome of this project is a fully functioning 3D dedicated breast imaging system ready for initial patient studies. In addition, the training provided by this grant has allowed the PI to continue on with further research and development of this system through SBIR and North Carolina state funding with the goal of commercialization.

Book excerpt: Nuclear medicine based molecular imaging techniques can be used as an adjunct tool in the diagnosis of breast cancer, particularly for women with dense breast tissue. The dedicated emission mammotomograph is now fully functional; the tomographic gantry was enhanced by including an automated radius-of-rotation capability, to allow for fully 3D imaging of a pendant, uncompressed breast. As a result, novel 3D acquisition orbits have been developed to contour various sized breasts. Initial phantom measurements with a l6x20 sq cm field-of-view cadmium zinc telluride detector show the capability of the system to acquire data about concentric hemispheres of the breast, minimizing distance-dependent spatial resolution effects, maximizing the viewable breast volume while more-nearly- completely sampling the breast, and minimizing background contamination from the heart and liver. Results indicate 3D mamnotomography, incorporating a compact CZT detector and unique 3D camera trajectories, is a promising dedicated breast imaging technique for visualization of tumors less than 1cm in diameter. Future work includes evaluation of: patient bed shielding, sampling schemes for various shaped breasts, and axillary imaging.

Book excerpt: The overall objective of this research is to optimize the development of a combined dual-modality single photon emission computed tomography (SPECT) and x-ray computed mammotomography (CmT) system for the earlier detection and staging of breast cancer, improving surgical biopsy guidance, and the monitoring of patient therapy response. It is believed that co-registered acquisition of emission (nuclear) and transmission (x-ray) data using both 3D imaging modalities in a common field of view may aid to accurately localize the radioactive uptake of a tumor in the emission image by using anatomical structure from the transmission image. In this second year, the radiation dose absorbed by the uncompressed breast during a regular tomographic scan using is extensively quantified using the developed tomographic system. New concept designs for the next generation 3D breast CmT system are also developed, in addition to new tomographic trajectories designed to overcome limitations in the ability of the developed CmT system to view the chest wall of the prone patient. In addition to the technical work, other aspects of the training program were also completed, including site visits to a major medical imaging company, the presentation of the research at scientific conferences, and the ongoing submission of a manuscript to a major peer reviewed journal. Finally, the doctoral thesis arising from this research was written and successfully defended by the author.

Previous meetings were held in York, England; Chicago, IL USA; Nijmegen, Netherlands; Toronto, Canada; Bremen, Germany; Durham, NC USA and Manchester, UK. th The 9 IWDM meeting was attended by a very international group of participants, and during the two and one-half days of scientific sessions there were 70 oral presentations, 34 posters and 3 keynote addresses. Book excerpt: Medical imaging has been transformed over the past 30 years by the advent of computerized tomography (CT), magnetic resonance imaging (MRI), and various advances in x-ray and ultrasonic techniques. An enabling force behind this progress has been the (so far) exponentially increasing power of computers, which has made it practical to explore fundamentally new approaches. In particular, what our group terms "model-based" modalities-which produce tissue property images from data using nonlinear, iterative numerical modeling techniques-have become increasingly feasible. Alternative Breast Imaging: Four Model-Based Approaches explores our research on four such modalities, particularly with regard to imaging of the breast: (1) MR elastography (MRE), (2) electrical impedance spectroscopy (EIS), (3) microwave imaging spectroscopy (MIS), and (4) near infrared spectroscopic imaging (NIS). Chapter 1 introduces the present state of breast imaging and discusses how our alternative modalities can contribute to the field. Chapter 2 looks at the computational common ground shared by all four modalities. Chapters 2 through 10 are devoted to the four modalities, with each modality being discussed first in a theory chapter and then in an implementation-and-results chapter. The eleventh and final chapter discusses statistical methods for image analysis in the context of these four alternative imaging modalities. Imaging for the detection of breast cancer is a particularly interesting and relevant application of the four imaging modalities discussed in this book. Breast cancer is an extremely common health problem for women; the National Cancer Institute estimates that one in eight US women will develop breast cancer at least once in her lifetime. Yet the efficacy of the standard (and notoriously uncomfortable) early-detection test, the x-ray mammogram, has been disputed of late, especially for younger women. Conditions are thus ripe for the development of affordable techniques that replace or complement mammography. The breast is both anatomically accessible and small enough that the computing power required to model it, is affordable.

This book grew out of the lecture notes I made for these students. The physics and engineering students have rather different expectations of what such a course should be like. I hope that I have nevertheless managed to write a book that can satisfy the needs of these different target audiences. The lectures themselves, of course, need to be adapted to the needs of each group of students. An engineering student will not qu- tion a statement like “the velocity of the electrons in atoms is ?1% of the velocity of light”, a physics student will. Regarding units, I have written factors h and c explicitly in all equations throughout the book. For physics students it would be preferable to use the convention that is common in physics and omit these constants in the equations, but that would probably be confusing for the engineering students. Physics students tend to be more interested in theoretical physics courses. However, physics is an experimental science and physics students should und- stand how experiments work, and be able to make experiments work. This is an open access book.
