Certaines études soulignent la concentration des soins dentaires sur une faible proportion d'enfants. Or, nous savons qu'une mauvaise santé bucco-dentaire entraine des répercussions tant sur le plan psycho-social que médical. Les services de Protection Maternelle et Infantile sont de véritables maisons de santé de proximité pour la mère et l'enfant et qui permettent de toucher une population plus vulnérable, en situation de précarité. Les PMI sont des lieux propices à la prévention et au dépistage bucco-dentaire des enfants dont les autres systèmes de prévention mis en place comme M'TDents n'ont pas réussi à atteindre. Cette étude qualitative exploratoire préliminaire s'intéresse aux attitudes, connaissances et pratiques des professionnels de PMI dans les Hauts de France qui sont chargés du dépistage et de la prévention de la santé orale de l'enfant. Elle permettra également de faire émerger les freins et les leviers à l'accès aux soins dentaires chez l'enfant dans ces services.

Par ailleurs, la collaboration interprofessionnelle est une pratique encore assez peu développée en France malgré les bénéfices qu’elle procure à la prise en charge des patients. Cette thèse d’exercice a permis d’évaluer les Connaissances, Attitudes et Pratiques en matière de santé bucco-dentaire des professionnels de santé exerçant au sein des centres de Protection Maternelle et Infantile du secteur Nord-Ouest de l’Essonne. Un message de prévention bucco-dentaire a aussi pu être délivré, et a permis des échanges oraux entre les différents professionnels de santé sur le sujet de la prévention bucco-dentaire chez les plus jeunes.

Book excerpt: La carie précoce de l’enfant est de nos jours un véritable enjeu de santé publique, dont les répercussions affectent le développement général des enfants. Les médecins généralistes, pédiatres libéraux et professionnels en centres de Protection Maternel et Infantile sont les interlocuteurs privilégiés des parents lors des premières années de l’enfant. Ils ont donc un rôle primordial dans l’accès à la prévention bucco-dentaire, et le conseil des bonnes conduites à adopter dès le plus jeune âge. Nous avons donc souhaité réaliser une étude sur les connaissances et pratiques de ces professionnels de santé en Ille-et-Vilaine par l’envoi d’un questionnaire. Ce travail a mis en évidence que les médecins de la petite enfance d’Ille-et-Vilaine souhaitent s’investir davantage dans la prévention bucco-dentaire. En effet ils sont bien informés sur les habitudes alimentaires à adopter, et pratiquent régulièrement un examen bucco-dentaire des jeunes patients. Cependant, il semblerait que d’autres aspects de leurs pratiques divergent des recommandations en vigueur, notamment concernant l’hygiène bucco-dentaire, l’utilisation du fluor ou l’âge de la première visite dentaire du tout petit. Ces dissonances pourraient venir d’une formation initiale et complémentaire insuffisantes concernant la prévention bucco-dentaire. De plus, face à un programme universitaire dense, les connaissances concernant la santé bucco-dentaire peuvent avoir été relayées au second plan. Cependant, les professionnels de santé sont ouverts à l’enrichissement de leurs connaissances. La mise en place d’un enseignement intégré au cursus initial, la réalisation de formation complémentaire et la confection d’un guide de recommandations chez le jeune enfant pourraient les aider dans leur pratique quotidienne.
