One major cause of stroke is atherosclerosis, especially when developed in carotid arteries. Certain plaque components, such as intraplaque hemorrhage (IPH), lipid-rich necrotic core (LRNC), calcification, and high vessel wall (VW) volume are critical determinants for the stability of a plaque and can now be identified noninvasively with multicontrast magnetic resonance imaging (MRI). This thesis examined the feasibility of using a standardized multicontrast MRI protocol in a multicenter setting with only clinically available hardware and software and showed over 90% of patients had acceptable quality images to allow study of plaque components quantitatively. This thesis also examined the quantitative relationship of IPH with other plaque components. Specifically, the volume of IPH was significantly associated with volume of VW and LRNC but not calcification. Our results provided quantitative insights of IPH and may facilitate the planning of future multicenter endeavors studying carotid atherosclerosis.

It is directly related to atherosclerotic development in the carotid arteries where risk is mainly assessed by measurement of stenosis. Diabetic patients are predisposed to cerebrovascular event occurrence and atherosclerotic development. An advanced feature of atherosclerotic disease is intraplaque hemorrhage (IPH), which increases the risk of such events. In this study, magnetic resonance imaging techniques were used to define an optimal method for measuring IPH. This method was applied to a cohort of diabetic patients without carotid artery stenosis, to find the prevalence of IPH, which was compared with carotid artery wall measurements to determine its related effects. This thesis provides evidence that advanced features of atherosclerosis, in the form of IPH, can be found even when no carotid stenosis is present and may provide insight into the added risk borne by diabetic patients.

Book excerpt: As a critical feature of vulnerable atherosclerotic plaque, intraplaque hemorrhage (IPH) is associated with fast plaque progression and subsequent cerebral ischemic events. Although presence/absence of intraplaque hemorrhage is a recognized biomarker for plaque vulnerability assessment, quantitative measurement of the intraplaque hemorrhage signals on MRI may contribute greater utilities to access the evolution of intraplaque hemorrhage and its vulnerability. Recent studies have demonstrated that intraplaque hemorrhage volume and signal intensity could either progress or regress along time and that the change differs between symptomatic and asymptomatic plaques. Therefore, intraplaque hemorrhage quantification may contribute to a precise assessment of atherosclerosis's clinical risk. However, intraplaque hemorrhage detection and segmentation have mainly relied on manual review, which is not only time-consuming but also prone to measurement errors due to flexible window display level settings. Furthermore, carotid MRI often suffers from complex motion problems, causing degradation of image quality and inaccurate vessel wall delineation. In this dissertation, a semi-automatic method was developed based on histological validation for intraplaque hemorrhage detection and quantification from magnetic resonance imaging (MRI). First, intraplaque hemorrhage detection criteria was established on the widely used Magnetization-Prepared Rapid Acquisition Gradient-Echo (MP-RAGE) sequence and recently developed Simultaneous Noncontrast Angiography and intraPlaque hemorrhage (SNAP) sequence. Both adjacent soft tissue and local median values on MP-RAGE were found to be good intensity normalization references for intraplaque hemorrhage detection, while the sternocleidomastoid muscle on the SNAP reference image was chosen for SNAP image intensity normalization. Second, a volumetric image processing method was developed on 3D SNAP and reproducibility for intraplaque hemorrhage quantitative measures were demonstrated. Lastly, a non-marker-attached motion detection and correction technique based on structured light was proposed for both abrupt and bulk motion correction in carotid MRI for better vessel wall delineation, which could also potentially improve delineation for intraplaque hemorrhage and other plaque components.

Study aims to compare the IPH occurrence in patients with asymptomatic stable (ASS), asymptomatic progressive (APS) and symptomatic (SS) carotid artery stenosis u02c350%.Materials and methods: Serial duplex ultrasound (DUS) in a 6-month period and magnetic resonance imaging (MRI) using axial 3DT1w sequence were used for IPH detection in patients with carotid stenosis. Stenoses in patients with ipsilateral stroke/transient ischemic attack within previous 4 weeks or acute ischemic lesion on diffusion-weighted MRI were evaluated as symptomatic. Stenoses with progression of u02c310% since last DUS examination were evaluated as progressive. Echolucent part of atherosclerotic plaque u02c38 mm2 on DUS and hyperintensity on 3DT1w-MRI were evaluated as IPH. Differences in IPH occurrence between ASS, APS and SS patients were statistically evaluated.Results: Totally 32 patients (18 males, mean age 71.3u00b17.7 years) were enrolled during 18 months; 5 patients with ASS, 18 with APS and 9 with SS. MRI examination was not performed in 3 ASS and 1 SS patient. IPH was detected using DUS/MRI in 1 (20%)/2 (40%) of ASS patients, 9 (50%)/8 (53%) of APS patients, and 5 (56%)/4 (50%) of SS patients (pu02c30.05 in all cases). IPH on both DUS and MRI were detected in none of ASS patients, 5 (28%) APS patients and 3 (33%) SS patients (pu02c30.05 in all cases).Conclusion: No significant difference in IPH occurrence was found between ASS, APS and SS patients. Totally 200 patients will be enrolled to the ongoing study.Supported by grant MHCR 17-31016A, 16-30965A.
