Book excerpt: Childhood obesity and food insecurity are public health issues that often coexist, and both conditions are more prevalent among rural compared to non-rural populations. Social ecological prevention efforts are recommended, yet limited research has examined how rural family-home environments may influence obesity-preventing and -promoting behaviors. The purpose of this dissertation was to examine whether and how family-home environmental and behavioral factors are associated with body mass index (BMI) and food security in rural children. To accomplish the aims of this project, a mixed methods study was conducted. For the quantitative phase of this study, relationships between family nutrition and physical activity (FNPA), food security, and children's BMI and dietary behaviors were examined. Results indicated no significant associations between FNPA and BMI or between FNPA and food security. More favorable FNPA factors were associated with higher consumption of fruits, vegetables, and dairy, and with lower intake of added sugar. The qualitative phase of this research involved nine semi-structured focus group interviews, conducted across six rural communities, to explore parent/caregiver perceptions of factors that influence behaviors related to nutrition and physical activity in the home environment. Four major themes that influence family nutrition- and activity-related behaviors were identified, including family eating habits, food procurement, family physical activity, and screen time. Eight themes emerged as environmental supports and/or barriers to family nutrition and physical activity, including seasonal variation, features of home, distance from resources, foods and beverages encouraged and discouraged, screen time limits, financial constraints, outdoor safety, and schedule constraints. This study provides evidence that certain factors in the family-home are associated with children's dietary behaviors, and that opportunities for rural children to eat healthfully and be physically active at home are influenced by factors internal and external to the family-home. Understanding how family-home and other environmental factors influence children's eating and activity behaviors and future health outcomes, as well as how public health efforts may support families in navigating challenges specific to rural areas, is an important area of research that warrants further exploration.

Book excerpt: Background The relationship between changes in family socioeconomic status (SES) and the development of obesity in childhood is unknown. The purpose of this research was to investigate the relationship between growth and two measures of SES change in childhood: family income trajectory and early-life food insecurity, and whether SES modified the relationship between the school nutrition and physical activity (N & PA) environment and growth during middle school, 6th to 8th grade. Methods This longitudinal research employed a birth cohort (n=595) located in rural New York State, followed from birth to 15 years. Data were collected through an audit of medical records, mailed questionnaires, and an assessment of the middle school N & PA environments. Family income and body mass index (BMI) z-score trajectories were created using latent-class modeling techniques to group children based on similar trends across time. Linear mixed models were used to estimate rate change in BMI zscore. Results 1. Children with poorer income trajectories were more likely to be in overweight and obese trajectories. Children who were persistently low-income were more likely to be in the overweight-stable BMI trajectory, and downwardly mobile children were more likely to be in the obese BMI trajectory. 2. The association between food insecurity and growth status varied across time. Foodinsecure children had a lower estimated BMI z-score in early childhood compared to food-secure children, but their elevated rate of growth during childhood resulted in a higher estimated BMI z-score by the age of 15 compared to food-secure children. 3. The association between the school environment and change in BMI z-score depended on income trajectory. Specifically, reductions in BMI z-score were associated with better physical education and general physical activity promotion environments among adolescents with unstable and persistent low-income trajectories. Conclusion Low SES trajectories and food insecurity were positively associated with the development of overweight and obesity in complex ways across childhood. Depending on an adolescent's income trajectory, better middle school environments for physical activity were associated with decreased obesity risk.

A lack of activity increases the risk of heart disease, colon and breast cancer, diabetes mellitus, hypertension, osteoporosis, anxiety and depression and others diseases. Emerging literature has suggested that in terms of mortality, the global population health burden of physical inactivity approaches that of cigarette smoking. The prevalence and substantial disease risk associated with physical inactivity has been described as a pandemic. The prevalence, health impact, and evidence of changeability all have resulted in calls for action to increase physical activity across the lifespan. In response to the need to find ways to make physical activity a health priority for youth, the Institute of Medicine's Committee on Physical Activity and Physical Education in the School Environment was formed. Its purpose was to review the current status of physical activity and physical education in the school environment, including before, during, and after school, and examine the influences of physical activity and physical education on the short and long term physical, cognitive and brain, and psychosocial health and development of children and adolescents. Educating the Student Body makes recommendations about approaches for strengthening and improving programs and policies for physical activity and physical education in the school environment. This report lays out a set of guiding principles to guide its work on these tasks. These included: recognizing the benefits of instilling life-long physical activity habits in children; the value of using systems thinking in improving physical activity and physical education in the school environment; the recognition of current disparities in opportunities and the need to achieve equity in physical activity and physical education; the importance of considering all types of school environments; the need to take into consideration the diversity of students as recommendations are developed. This report will be of interest to local and national policymakers, school officials, teachers, and the education community, researchers, professional organizations, and parents interested in physical activity, physical education, and health for school-aged children and adolescents.

Book excerpt: Abstract: Rates of childhood overweight and obesity continue to be high, particularly among rural Appalachian children and adolescents. Specific information about current eating behaviors and their relationship to possible economic barriers is necessary to develop interventions and educational programs that will effectively reduce rates of overweight and improve the health of Appalachian children and adolescents. This study aims to elucidate the interrelationships between food security, dietary patterns and overweight among Appalachian adolescents. Students in the ninth grade at participating southeastern Ohio high schools were recruited for this study. Students and their parents or primary caregivers completed surveys and participated in focus groups. All surveys asked for demographic information as well as weight and height, which were used to calculate weight status. Student surveys included the Early Childhood Longitudinal Study Food Frequency Questionnaire and the physical activity questions from the 2003-2004 National Health and Nutrition Examination Study. Parent surveys included the U.S. Household Food Security Survey Module Short Form, used to assess food security status, in addition to questions on income and educational attainment. Separate focus groups for students and parents were conducted to probe participants for their perceptions of healthy weight, healthy diet and barriers to health. Data was collected from eight student-caregiver pairs at three Appalachian Ohio high schools. Overweight and obesity were found to be widespread among both adolescents and adults, although food insecurity was not prevalent in this sample. Overweight in adolescents does not appear to be linked to household food insecurity. Fast food, snack and sugar-sweetened beverage consumption were high among adolescents, while fruit and vegetable consumption was low. In focus groups, lack of time, desire and ability to prepare healthy meals along with the greater availability of unhealthy foods were cited as barriers to consuming a more nutritious diet. Further research is needed to elaborate on these relationships and determine the most promising areas for intervention.

To achieve the Healthy People 2020 goal of reducing childhood obesity, interventions must be developed based on an understanding of the origins of childhood obesity. The goal of this study was to determine relationships between maternal food insecurity, child feeding practices and perceived child weight in a Mexican-origin population. Data were obtained through food security and medical history surveys along with anthropometric measurements of 175 Mexican-origin mother-child dyads living in rural, agricultural towns. No relationships were seen between food insecurity and other variables; however, only 31% of mothers of overweight children accurately identified them as such and only 47% of these mothers reported an attempt to change her child's nutrition or physical activity. This study exposed the need to determine factors involved in the identification of childhood obesity and in how mothers decide to change nutrition and physical activity for their children.

Many health, psychosocial, and economic consequences are associated with childhood overweight and obesity, which concerns health researchers and professionals. But how and why might rural children be more at risk for being overweight and obese? This dissertation investigates childhood overweight and obesity in rural settings through three separate studies. First, a systematic literature review was conducted to identify determinants and mechanisms of childhood obesity-related behaviors that are specific to rural locations. The findings from the review show that lack of health resources and poverty within the rural environment may impact children's social environment and individual factors. However, results are inconclusive and there continues to be a lack of studies focusing on linking environmental influence with individual factors. Second, a meta-analysis of current research evidence was conducted to assess the efficacy of rural interventions designed to reduce childhood overweight and obesity. Results showed that interventions have been efficacious yet modest, with a mean effect size of 0.18. Moderating variables were also examined. Mean intervention effect size was moderated by children's age and intervention duration. Last, secondary data were used to examine the association between rural food stores and availability and affordability of fresh fruits and vegetables. A multilevel analytical approach was used to determine if rural location was associated with availability and affordability of fresh fruits and vegetables. After controlling for other variables, results showed that rural location was not associated with fruit and vegetable availability and affordability. The findings from this dissertation suggest that the area of rural childhood overweight and obesity remains understudied. More research is needed in order to understand the mechanisms of social ecological influences on diet, physical activity, and childhood overweight and obesity. This area of research, however, is rife with opportunities for public health education and promotion. Public health educators can help promote and advocate for environmental conditions that support healthy lifestyles. Book excerpt: The purpose of this cross-sectional population study was to provide an assessment of weight status of a county's preschool population utilizing the new growth charts and expressed as Body Mass Index, or BMI, -for- age percentile. This study was conducted in conjunction with an annual health screen for incoming kindergartners and consisted of two phases. The first phase involved assisting in the collection of, and statistically analyzing preschoolers' data collected during the Tillamook Health Screen on May 23-25th, 2001. Height, weight, blood pressure, hemoglobin, and blood lead levels were measured. Also, the preschoolers' parents completed a 24-hour food intake record and answered questions on mealtime habits. Phase Two consisted of a mailed questionnaire that was sent to parents of preschoolers who were screened in May, 2001, to investigate dairy food consumption, where meals are eaten, and physical activity habits of their preschoolers. Four significant findings were documented in this research. Foremost, Tillamook County preschoolers had a lower prevalence of healthy weight and a higher prevalence of at risk of overweight and overweight levels than children their age nationwide. Also, both systolic and diastolic blood pressure increased with increasing BMI-for-age percentiles for males and females. Third, hours spent viewing television -sedentary behavior-was positively related to BMI-for-age percentiles. The combination of more hours of physical activity with less television viewing time was inversely related to BMI-for-age percentiles. Last, Tillamook County preschoolers who were above the healthy weight range ate more Food Guide Pyramid servings of concentrated fats/sweets than children in the healthy weight range. Data that were not strong enough to reach conclusions about weight status related to dairy product consumption, fat content of dairy products, mealtime habits, meals eaten away from home, blood hemoglobin, and blood lead. Also, no significant associations were found between dairy food intake and blood hemoglobin, blood lead, or blood pressure. Even at preschool ages, physical activity and diet are important to assess when increasing rates of overweight levels and associated increases in blood pressure are being investigated.

Book excerpt: The obesity epidemic is a major public health concern, where the prevalence rates amongst the American children population have more than doubled since the 1980s. Among overweight children, the risk of becoming an overweight or obese adult is 70% higher than children of normal weight, and obese children are more likely to remain obese into adulthood and face a number of morbidities associated with it, including lower quality of life and increased financial burden. In this research, we examined the relationship between household food security and obesity among children and adolescents between the ages of 2-18 years old. We used data from the NHANES 2005-2006 (n= 3,432). Amongst the children aged 2-18 years, 31.21% were determined to be obese or at-risk for obesity. Children aged 2-18 years were 1.27 times more likely to be obese or at-risk when living in a food insecure household after adjusting for race/ethnicity. Adolescents aged 12-18 years were 1.47 times more likely to be obese or at-risk when living in a food insecure household. No significant association was found for young children aged 2-11 years. After adjusting for race/ethnicity and poverty level status, however, the association between food insecurity and obesity was not significant for either age group. Further investigation of other potential confounders could explain the association for both young children and adolescents. There are other factors, like social and societal, that influence the trends of obesity. Future programming could work to ameliorate the conditions of food insecurity and other infrastructure factors.

The magnitude of the problem has increased dramatically during the last three decades and, despite some indications of a plateau in this growth, the numbers remain stubbornly high. Efforts to prevent childhood obesity to date have focused largely on school-aged children, with relatively little attention to children under age 5. However, there is a growing awareness that efforts to prevent childhood obesity must begin before children ever enter the school system. Early Childhood Obesity Prevention Policies reviews factors related to overweight and obese children from birth to age 5, with a focus on nutrition, physical activity, and sedentary behavior, and recommends policies that can alter children's environments to promote the maintenance of healthy weight. Because the first years of life are important to health and well-being throughout the life span, preventing obesity in infants and young children can contribute to reversing the epidemic of obesity in children and adults. The book recommends that health care providers make parents aware of their child's excess weight early. It also suggests that parents and child care providers keep children active throughout the day, provide them with healthy diets, limit screen time, and ensure children get adequate sleep. In addition to providing comprehensive solutions to tackle the problem of obesity in infants and young children, Early Childhood Obesity Prevention Policies identifies potential actions that could be taken to implement those recommendations. The recommendations can inform the decisions of state and local child care regulators, child care providers, health care providers, directors of federal and local child care and nutrition programs, and government officials at all levels.
