Book excerpt: Neurological disorders, such as Alzheimer's disease and Parkinson's disease, are an important cause of disability and death worldwide and pose a serious burden on the global health system over the past 30 years. Acupuncture, as the most widely used traditional Chinese medicine therapy in the world, has been widely used in 183 countries and regions. Acupuncture can treat more than 300 kinds of diseases, especially for neurological diseases. Due to the unique advantages of acupuncture such as good therapeutic effect and low side effects, many researchers have conducted extensive clinical studies on the effectiveness and safety of acupuncture in the treatment of neurological diseases, laying a foundation for the practicability and scientific vigor of this complementary and alternative therapeutic approach. There are still many problems unanswered in the current clinical research on acupuncture for the treatment of neurological diseases: First of all, clinical research is lagging behind due to the lack of emphasis on modern advanced methods and analytical techniques (e.g., artificial intelligence, omics analysis, bioinformatics analysis, data mining) and multidisciplinary integration process. Secondly, the quality of reports on most clinical studies of acupuncture treatment for neurological diseases is extremely low. Most clinical studies have not adopted standardized methodological evaluations, authoritative disease diagnosis standards, and efficacy evaluation standards. The lack of compliance regulations and strict study design reduces the accuracy and authenticity of the research. Thirdly, there is a lack of integrated assessment of the distribution of evidence for acupuncture treatment of neurological diseases. Therefore, strictly designed clinical research and in depth and systematic analysis method are currently needed to provide high-quality evidence for acupuncture treatment of neurological diseases. In this Research Topic, we hope to further understand the role of acupuncture in the treatment of neurological diseases through clinical research and secondary research. The range of acupuncture includes manual acupuncture, electro-acupuncture, body needling, auricular acupuncture, scalp acupuncture, laser acupuncture and acupressure, etc. Neurological diseases include stroke, meningitis, encephalitis, tetanus, Alzheimer's disease and other dementias, Parkinson's disease, epilepsy, multiple sclerosis, motor neurone disease, migraine, tense-type headaches, headaches from drug overuse, and others.

Book excerpt: Cardiovascular diseases include ischemic and hemorrhagic diseases involving the heart, brain, whole body tissue, and includes coronary heart disease, heart failure, arrhythmia, atherosclerosis and stroke. This particular group of diseases continue to be a leading cause of death throughout the world with mortality rate remaining high. Currently, drugs administered orally and intravenously and surgical treatments are used to treat such diseases. Traditional Chinese medicine (TCM) refers to natural herbal medicines and their processed products used for preventing and treating disease under the guidance of traditional Chinese medicine’s theory. The implementation of prevention and treatment programs of ischemic cardiovascular diseases with the use of TCM have been associated with positive outcomes; in terms of a reduction in the disability and mortality rate of some patients. Other studies have also shown that specific multi-component TCM preparations have therapeutic benefits based on multi-target and multi-pathway mechanisms, which may have advantages over the current single-component and single-target therapy. Based these characteristics, approved pharmaceutical drugs based in TCM, such as Compound Danshen Dripping Pills, Naoxintong Capsules, Tonxinluo Capsules, and Danhong Injection, are currently used to treat cardiovascular diseases. However, there are a number of areas that still need further investigation. For example, the identification of effective components in herbal medicine is essential in furthering our understanding of what occurs at a pharmacological level; the metabolizing pharmacological pathways of such components; the cellular target of the components; and the lack of standardized guidelines to enhance clinical research.

Understanding the molecular mechanisms underlying metabolic diseases and the potential benefits of herbal medicines (incl. herbal medical products) can provide valuable insights into new therapeutic targets and treatment options.

Natural products, including phytochemicals and functional foods are known for their ability to affect metabolic processes such as ameliorating reactive oxygen species-induced mitochondria dysfunction, mitigating inflammatory response, and other cellular functions that could ameliorate the disease developments. Despite the relatively low toxicities of natural products as compared with synthetic products, further studies are still required to optimize the bioavailability, therapeutics, pharmaceutics, and clinical trials of natural products in patients suffering oxidative and metabolic diseases.

The core principle of a network target is to construct a biological network that can be used to decipher complex diseases. The network is then used as the therapeutic target, to which multicomponent remedies are applied. Book excerpt: SARS is a newly identified human infection caused by a corona virus unlike any other known human or animal virus in its family. The analysis of epidemiological information obtained from the sites of the outbreaks of SARS is still underway but the overall case fatality ratio is known to approach 11% although the rate among the elderly is much higher. Currently the major challenges for the treatment of SARS are: the source of the SARS virus and mode of transmission are still not well understood; there are problems with diagnostic tools; there is no effective treatment; and there is no vaccine for SARS. The above-mentioned difficulties and challenges have motivated national authorities health workers and scientists to explore the potential of complementary treatment. The results of research on integrated treatment with TCM and Western medicine showed that it is safe and that it also has some potential clinical benefits. Therefore the experts suggested that records of such experience could serve as reference material for treatment of SARS in the future. This publication is intended to share experience in the complementary treatment of SARS patients; share the experience of clinical studies in the field of traditional medicine for treatment of SARS between the physicians and researchers; and to further encourage and promote the quality of research in the filed of traditional medicine.

The ongoing opioid crisis lies at the intersection of two public health challenges: reducing the burden of suffering from pain and containing the rising toll of the harms that can arise from the use of opioid medications. Chronic pain and opioid use disorder both represent complex human conditions affecting millions of Americans and causing untold disability and loss of function. In the context of the growing opioid problem, the U.S. Food and Drug Administration (FDA) launched an Opioids Action Plan in early 2016. As part of this plan, the FDA asked the National Academies of Sciences, Engineering, and Medicine to convene a committee to update the state of the science on pain research, care, and education and to identify actions the FDA and others can take to respond to the opioid epidemic, with a particular focus on informing FDA's development of a formal method for incorporating individual and societal considerations into its risk-benefit framework for opioid approval and monitoring.
