Ce protocole permet de répondre à toutes les situations cliniques rencontrées de manière systématique.

L'enjeu du praticien sera de minimiser celle-ci afin de maintenir un volume tissulaire pour la mise en place de l'implant puis d'une prothèse d'usage avec une pérennité de résultat. Le clinicien pourra alors réaliser des greffes osseuses ou de tissus mous dans un premier temps ou simultanément à la mise en place de l'implant. La technique d'extraction/implantation et mise en esthétique immédiate montre tout son intérêt puisqu'elle permet un gain de temps ainsi qu'un confort psychologique pour le patient. Nous verrons alors que ce protocole présente le même taux de succès que le protocole traditionnel d'implantation différé. Cette technique ne peut s'appliquer à l'ensemble des cas et devra présenter d'importants critères de sélections : parodonte épais, table osseuse vestibulaire présente et intacte, mise en place en palatin de l'implant et stabilité primaire satisfaisante. Si l'un de ces critères n'est pas présent le praticien s'expose au risque d'échec. Si tous les critères sont réunis, le praticien devra réaliser une extraction atraumatique, le nettoyage de l'alvéole, la mise en place de l'implant en respectant des critères de positionnement et avec une stabilité primaire satisfaisante. Une fois l'implant mis en place le praticien devra réaliser une couronne provisoire qui sera vissée ou scellée sur un pilier provisoire. La réalisation de cette couronne provisoire est une étape importante, un polissage minutieux doit être réalisé et celle-ci ne doit pas être en occlusion statique et dynamique.
