Book excerpt: The purpose of this thesis was to develop an artificial intelligence algorithm to classify patient specific bone density from DICOM images acquired from orthodontic radiographic scans and to develop an osteoporosis screening tool. Osteoporosis is a degenerative disease that results in weakened, brittle bones; unfortunately, this disease presents no outward symptoms until a bone is fractured. The goal of this research was to develop a method to assist in the early diagnosis of this disease using routine dental and orthodontic scans. Bone density in this thesis was approximated by pixel intensities from patient DICOM images. Individual cephalometric patient scans from an orthodontist were read into a custom MATLAB algorithm, which extracted points from the patient0́9s alveolar bone and determined the pixel intensity as an indicator for bone density. The alveolar (anterior mandible) bone was analyzed due to a correlation between tooth loss and osteoporosis documented in the literature. The intensity values of these pixels were averaged together as one global value for each patient. By comparing these global values versus age for 32 patients, a correlation between patient age and bone density was evident: as patients increase in age past 35 years, their bone density decreases, with different trend characteristics for men versus women. The results of this thesis provide a basis for the creation of a commercialized graphical user interface to aid medical professionals in the early detection of decreasing bone density in their patients.

Available in PDF, EPUB and Kindle. Book excerpt: In this dissertation, we developed and applied image processing tools that can perform automated quantitative assessment of whole bone images. We developed Quantitative Total Skeleton Imaging (QTSI) for assessing whole bone. QTSI can be performed on anatomical CT images (QTSI(CT)) or on functional PET/CT images (QTSI(PET/CT). QTSI quantifies both image intensity and heterogeneity of bone on a regional, as well as whole skeleton, level and can perform image-based response assessment of patients on a voxel-level. We developed a method to create whole skeleton statistical appearance models, which can provide population average and ranges of expected bone intensity for each voxel within the bone, which for CT is bone density and for PET is bone activity. Using statistical appearance models of average and ranges of healthy bone intensity, QTSI(CT) and QTSI(18F-NaF PET/CT) were used to quantify healthy bone density with CT and healthy bone turnover with 18F-NaF PET/CT, respectively. We developed Quantitative Total Marrow Imaging (QTMI(PET/CT)) for assessing the bone marrow. We created a quantitative reference of healthy bone marrow proliferation, providing expected values of healthy bone marrow activity for each bone region in the skeleton. QTMI(18F-FLT PET/CT) analysis was demonstrated in two patients with acute myeloid leukemia (AML). Both uptake and response metrics were found to differentiate patient response. We developed Quantitative Total Bone Imaging (QTBI(PET/CT)) for assessing patients with multiple bone lesions. To improve detection of lesions in QTBI(18F-NaF PET/CT), we developed a Statistically Optimized Regional Thresholding (SORT) method to optimize lesion detection. We created disease distribution models that represent distributions of benign and malignant bone lesions. We developed a machine learning tool for automated classification of definite benign vs definite malignant lesions (AUC=0.95). This model was trained on different physicians and could accurately predict (AUC=0.93-0.94) how each physician classified lesions. With these tools we can comprehensively assess bone disease by identifying if patients have abnormalities within the bone, automatically identifying the location of abnormal regions of bone, assessing if the abnormalities are in areas of higher risk of certain bone diseases, classifying lesions as benign or malignant, and longitudinally monitoring lesions over the course of treatment.
