Book excerpt: A vast literature has been concerned with arteriosclerosis and yet, many aspects of pathogenesis and of the mechanism of development of the arteriosclerotic vascular lesion remain only poorly understood. In recent years, our knowledge of the earliest stages of arteriosclerosis have greatly improved. By now, we have learned to relate morphologic changes to disturbances in function. It has been of particular impor tance that components of the arterial wall could be analyzed in regard to dysfunction, for example, in the endothelium or the vascular smooth muscle. The interaction of the different morphological components of the vascular wall could thus be much bet ter understood. Likewise, the interaction between the arterial wall and the flowing blood could be much better described, including the intimate relationship between platelets and the endothelium, the coagulation system and the endothelium, the granulocytes and the endothelial cell layer, as well as processes of migration of blood cells into the subendothelial space. The recognition of functional and morphological disturbance has attained clinical significance not only because the arteriosclerotic diseases have quantitatively reached the dimensions of an epidemic, that is, of a magnitude never been witnessed. It is also because of the development of new drugs that interfere with the atherogenic process and thereby prevent the development of the disease or halt its progression. It is also becoming increasingly possible to inhibit the occurrence of complications in existing arteriosclerotic lesions in manifest disease, i. e.

Book excerpt: The inferior segments of the abdominal aortas of 15 dogs were irradiated with electrons in doses ranging from 1000 to 9500 rads, and were examined histologically at intervals ranging from 2 weeks to 17 months following irradiation. Pronounced aortic arteriosclerosis developed and was sharply localized to the aortic segment irradiated. Except in one dog, radiation-induced arteriosclerosis was more pronounced on the dorsal than on the ventral aortic wall. There was no evidence that severity of aortic arteriosclerosis was related to the dose of irradiation or was increased with time following irradiation. The development of arteriosclerotic lesions of the aorta induced by electron irradiation was similar to that observed after X-irradiation and to that occurring naturally in old dogs. It is suggested that electron irradiation may have caused selective injury of the internal elastic lamina, which was then followed by intimal fibrous proliferation and formation of intimal plaques. Recent data from a number of laboratories suggest that arterial mesenchyme is of considerable importance in the pathogenesis and fate of arteriosclerotic lesions. The significance of some of these observations made it clear that there was need for intensified research on the connective tissue components of the arteriosclerotic lesion and that arteriosclerosis research workers could benefit from a more comprehensive view of the subject. Because of their experience in the field of arteriosclerosis and their interest in stimulating new directions for research on the lesion, the Committee on Coronary Artery Lesions and Myocardial Infarctions of the Council on Arteriosclerosis, American Heart Association, planned an International Workshop on Arterial Mesenchyme and Arteriosclerosis. The Workshop brought together scientists expert in connective tissue research and research on arteriosclerosis who presented the current status of knowledge in their areas of expertise. The Workshop was held April 2-3, 1973 at the Royal Orleans Hotel, New Orleans, Louisiana and was attended by more than 170 people. The twenty papers and discussions presented in this volume summarize the proceedings of the Workshop and represent a comprehensive review of the role of arterial mesenchyme in arteriosclerosis.
