Book excerpt: The forward surgical team (FST) is a 20-man team which provides far forward surgical intervention to render nontransportable patients sufficiently stable to allow for medical evacuation to a Level III hospital (combat support hospital [CSH]). There are 57 patient condition codes (Appendix A) that identify patients with the type of injuries that would benefit most from FST intervention. Surgery performed by the FST is resuscitative surgery; additional surgery may be required at a supporting Level III hospital in the area of operations (AO). Patients remain at the FST until they recover from anesthesia, once stabilized they are evacuated as soon as possible. The postoperative intensive care capacity of the FST is extremely limited, there is no holding capability. The FST is not a self-sustaining unit and must be deployed with or attached to a medical company or hospital for support. Further, the FST is neither staffed nor equipped to provide routine sick call functions. This field manual (FM) outlines doctrine for the employment of the FST. It is the primary reference document for the Active Component (AC) and the Reserve Component (RC) of the Army. It presents tactics, techniques, and procedures for employing FSTs. It is primarily intended for the use of the FST chief, his team, and the medical company/troop commanders and their staff. Other intended users include senior medical commanders, senior medical staff advisors, and joint and Army health service support (HSS) planners. This publication is fully compatible with Army operations doctrine in war and stability operations and support operations as outlined in FM 3-0. It is also compatible with combat service support (CSS) and HSS doctrine outlined in FM 4-02, FM 100-10, and FM 100-15. This publication assumes that the user has a fundamental understanding of FM 4-02, FM 100-10, and FM 100-15; it does not repeat the concepts contained therein except to explain operations unique to the FST.

The principal audience for this publication is all commanders and their staffs, command surgeons, Army health systems planners, Army Medical Department personnel and units involved in medical evacuation operations.The Army Health System is a complex system of interrelated and interdependent systems which provides a continuum of medical treatment from the point of injury or wounding through successive roles of medical care to definitive, rehabilitative, and convalescent care in the continental United States, as required. Medical evacuation is the system which provides the vital linkage between the roles of care necessary to sustain the patient during transport. This is accomplished by providing en route medical care and emergency medical intervention, if required, to enhance the individual's prognosis and reduce long-term disability. Army Techniques Publication 4-02.2 contains six chapters and four appendices as follows: Chapter 1 provides an overview of Army Health System and how medical evacuation relates to the principles of the Army Health System. It discusses the purpose, primary tasks, and attributes of the Army medical evacuation system. It also defines the differences between medical evacuation and casualty evacuation. Chapter 2 discusses the employment of medical evacuation resources and the coordination and synchronization required to effectively execute medical evacuation operations. This includes the medical evacuation request process, consideration for evacuation missions, support planning considerations, and evacuation in specific environments. Chapter 3 describes the mission, function and capabilities of medical evacuation units and elements as specified in the unit's table of organization and equipment. It also discusses the mission command headquarters to which they are assigned. Chapter 4 discusses the factors that establish the evacuation policy and the impact of the evacuation policy on Army Health System support. Chapter 5 provides insight and considerations into developing the operational and tactical medical evacuation plan that supports the combatant commander's mission. Chapter 6 describes the medical regulating system designed to ensure the efficient and safe movement of regulated patients to the appropriate military treatment facility by the most effective means. It also discusses the multi-Service responsibility and assets used to conduct this mission. Appendix A provides a summary of the Geneva Conventions and The Law of War. Appendix B provides an example of a medical evacuation plan as part of an operations order. Appendix C provides an example of the 9-line medical evacuation request format. Appendix D provides examples of medical evacuation during operations to shape, prevent, large-scale combat operations, and to consolidate gains.

Their omission from this version of ATP 4-02.5 is not an error nor a defect. This publication addresses the casualty care aspects of the health service support mission under the sustainment warfighting function. It describes the various organizational designs for the units providing this support and doctrinal guidance on the employment of these organizations and their functional capabilities. The staffing and organizational structures and positions presented in this manual are established in table's organization and equipment (TOEs). This publication implements or is in consonance with North Atlantic Treaty Organization (NATO) International Standardization Agreements (STANAGs) and American, British, Canadian, Australian and New Zealand (ABCA) standards and publications.

It is to be used as a guide in both obtaining and providing Role 1 Army Health System support in a maneuver battalion's area of operations. Information in this publication is applicable to decisive actions in support of unified land operations. It is compatible with the Army's command and control, protection, and sustainment doctrine and is aligned with Joint Publication (JP) 4-02.

This publication provides doctrinal guidance for all formations assigned to the ABCT, the IBCT, and SBCT. The following is a summary of each chapter in the manual: Chapter 1 addresses the role of Cavalry in unified land operations and Cavalry organizations. Chapter 2 discusses understanding the threat, potential threat groups and threat characteristics. Chapter 3 addresses the operational environment, shaping, engaging, and influencing outcomes, and consolidating gains. Chapter 4 highlights the updated concepts of mission command in relation to commander's reconnaissance and security guidance, the operations process and information collection. Chapter 5 begins with an overview, followed by a detailed discussion of the fundamentals of reconnaissance, forms of reconnaissance, and reconnaissance handover. Chapter 6 begins with an overview, followed with the fundamentals of security operations, counterreconnaissance, and the forms of security.
