Book excerpt: Excerpt from Archives of Neurology and Psychiatry, Vol. 8 Neurologic Examination (feb. 9, - General Survey: The patient was poorly nourished and weighed 125 pounds (56 kg). He was nervous and trembling, and sweated profusely at the time of examination. A fine muscular tremor was present on both sides of the face and neck, most marked on the left side. The thyroid cartilage was drawn somewhat to the right. The Romberg sign was negative, and the gait was normal. Coordination: A slight ataxia was present in the finger to nose test, with marked intention tremor. The heel to knee test was normal. The spread fingers showed a coarse tremor. An intention tremor was present when the patient attempted coordinate movements (reaching for a glass) under observation. Neuromuscular: Twitchings (tremor) of facial and platysma muscles were present; there was a slight atrophy of the right platysma. The muscles of mastication seemed weak as a whole; there was marked atrophy of the left masseter and temporal muscles. The left pterygoids were involved (inability to move the jaw laterally to the right as far as to the left). The grip of the teeth was weak; the grip was weak in both hands, more marked in the right. Reflexes: The tendon reflexes were hyperactive throughout. There were no pathologic pyramidal reflexes: the Babinski, Oppenheim and Gordon signs were negative. A means of relief in selected cases is afforded by chordotomy. The operation Will not be entirely successful in every case in which it is employed some pain fibers occasionally will escape the knife - but the Operation has now been performed sufficiently often to justify a belief in its efficacy. It is a delicate procedure, requires a skilful technic, and is not to be undertaken lightly by one untrained in the surgery of the spinal Cord. It requires on the part of the Operator a visualization of the microscopic anatomy of the cord, and a realization that a Slight misplacement of the incision may cause motor paralysis of one or both lower limbs, depending on whether the division is unilateral or bilateral. We 1'have written. On this subject previously and therefore do not refer to the literature at the present time. We report in this paper eight cases in Which chordotomy has been performed. All but one have not been reported previously, but in this one case chordotomy for certain reasons mentioned in the notes has been performed three separate times on the same side of the cord. In one case (case the operation was performed to remove the intense hyperesthesia above the anesthetic area produced by transverse myelitis, and was entirely successful. It is apparent that the dangers Of peace may be even greater than the dangers Of war, and the pano rama of the world - as Spread before us today - may make the timid exclaim with King Pyrrhus, Another such victory, and we are lost. These world phenomena appeal to us not as politicians but as physicians, not as partisans but as scientists; it is for us to discover the pathologic in them, for even in the mad Whirl of pandemic hysterias in which SO many others are losing their heads, the neurologists may be trusted to keep theirs on their shoulders. Let us gather the fruits Of a disastrous Victory, for sometimes the gleaning Of Ephraim is better than the Vintage Of Abiezer. The condition in which the world finds itself imposes an Obligation on such a society as ours. It should aim to take a lead in sane think ing; to Oppose with a persistent and trenchant criticism the vagaries Of thought in high and low places: to stem the turbid tide which bears on its bosom the otsam and jetsam of a neurasthenic age. A recent writer in Harper's Magazine exults in the thought that this age and country is that Of the commonplace.
