For nurses who work in the emergency department, would a 1:4 nurse-to-patient ratio, verse the current 1:6 nurse-to-patient ratio reduce error and poor patient outcomes while increase the safety of patients and nurse satisfaction during a three month trial period? In a growing emergency department, a new trial assignment protocol will be devised to ensure adequate patient care, increase patient outcomes with increased patient safety as well as increasing nurse satisfaction. In just one of the many examples, research shows an increase of one RN per patient day was associated with a 9% reduction in patient mortality in critical care areas, as there is a consistent relationship between higher ratios and hospital-related death (Shekelle, 2013). The trial will assess the nurses' needs to aid in sufficient patient care, and attempt to secure the safety of patients. The proposal includes an implementation plan involving an additional staff position during the times of higher census and acuity while placing another nurse on-call for further coverage during mass codes, traumas, and to cover nurse absences. Staff will be educated on the process and supportive research. Evaluation will be done with data collection of patient complaints, nurse and patient surveys, and compilations of incident reports, medication errors, and investigation of poor patient outcomes. Results of the study will be presented to staff, administration and stakeholders, to determine whether the trial is effective in decreasing patient risk for adverse outcomes, and increasing nurse satisfaction for permanent protocol initiations.

Book excerpt: Positive patient outcomes are directly related to quality of care received and there is a vast amount of research that determines the relationship between nurse staffing ratios and patient outcomes. The problem observed in this study is a facility in which there is a high acuity and high nurse-to-patient ratio assignments. The quality of care decreases due to the nurse having so many patients, typically six, and so much to do and assess for each one. Due to the negative patient outcomes and stress of nurses not being able to complete every task, especially not on time. With at least four extensive medication passes per patient and ordered tasks for each patient throughout the day, one can observe how the nurses ability to turn six patients every two hours on the hour, providing them with prompt PRN medications for pain, and performing tasks as proper as they should be performed is hindered. The nurse-to-patient ratio in the acute care setting should be 1:4 (one nurse per four patients), no exceptions. The problem associated with this plan is that the facility does not want to spend the money on hiring and having more nurses per shift. The facility provides staffing according to Hours Per Patient Day or HPPD. The problem with this system is that the HPPD has a set number and only the number of patients determines the staffing, not the acuity level of the patients. The problem needs to be presented to administration and management along the company chain with an exceptional and effective strategy, goal, implementation process, and evaluation process. After one year of implementation, data collected and obtained will be compared with data from one-year prior to determine the difference in results. The results will compare occurrence of wounds, medication errors, hospital-associated infections, falls and injuries, all associated costs with each as well as patient satisfaction surveys and nurse job satisfaction surveys. The results will be disseminated in an effective and appropriate manner.

In order for patients to receive the highest quality of care, nursing staff must be able to provide these patients with their undivided attention. There is an overwhelming amount of research/data that shows the direct correlation between nurse staffing (id est nurse-to-patient) ratios and positive patient outcomes. This study takes place in Albuquerque, New Mexico at the Presbyterian Hospital, on the unit Adult Progressive Care 1 (APC1). Based on data collected and mere observation, quality of care and patient safety suffer when nursing staff is required to care for many high acuity patients at one time. Nursing staff on APC1 are generally assigned five patients, and more often than not these patients are of high acuity. In the acute care setting, nurse-to-patient ratio's should be 1:4 (one nurse per four patients) due to the following observations: the need for thoroughly and accurately performed and documented patient assessments and rounding, many extensive medication passes (id est scheduled and as needed (PRN) medications), preparation for tests and procedures, patient education (id est plan of care, medication, procedural, and safety), and patient advocacy. With the high nurse-to-patient ratios seen on APC1, one can see that accuracy, proper follow-up and adequate time spent with patients is hindered. Not only do the patients overall outcome suffer, but the job satisfaction of the nurse suffer. APC1, on average, sees nurse turn around every 1 to 3 years. With this type of turn around, Presbyterian cannot accomplish their goal of hiring the nurse for the lifetime of his/her career. This problem needs to be presented with an effective strategy, goal, implementation process, and evaluation process, to the hospitals management team. After successful implementation of the proposed changes, it is the writers hope to disseminate these changes not only hospital wide, but statewide.

With that said, the controversial topic of patient ratios per nurse has been talked about for years. Inadequate staffing of nurses or a high patient-to-nurse ratio will negatively influence patient care, outcomes and their satisfaction. High patient ratios will also lead up to nurse burnout and increased turnover rates. In the hospital acute care settings globally, nurses are caring for an average of eight patients to a minimum of two (Hinno, Partanen, and Vehvilainen-Julkunen, 2012). These high ratios are not conducive to give patients the care they deserve or the nurses their satisfaction of knowing they provided optimal care. So, the question of this study ask 0−́In adult hospitalized patients, does a lower patient-to-nurse ratio of 5:1 or less produce a better quality of care to patients compared to a higher patient-to-nurse ratio of 6:1 or greater?0+́ A literature review of 15 research articles was performed regarding the issue of high patient- to nurse ratios and the outcomes resulted from the studies. Because staffing and ratio issues are a concern in organizations, a proposal was set forth in an attempt to correct the problem at hand. California was one of the first states to implement mandated low patient-to- nurse ratios, which studies have shown increased patient satisfaction, improved patient outcomes, decreased nurse burnout and improved nurse retention and satisfaction (Aiken, Sloane, Cimiotti, Clarke, Flynn, Seago, Spetz and Smith, 2010). Mandated staffing ratios, as California has set in place and the implementation of an evidenced based staffing methodology are proposed solutions set forth by this writer. Implementation of this proposed solution will involve careful assessment, planning and communication efforts. To gain approval, the proposal will be submitted to administration with the current policy and the proposed policy for changes. Evaluation of the projects effectiveness, will be by the use of online surveys and graphs that show positive gain in patient and nurse outcomes as a result of a lower patient to nurse ratio. Educational tools will also be incorporated in the form of a power point presentation and surveys to assess for staff readiness for the new staffing changes. To inform others regarding the new staffing ratio changes, this writer will list the information of the organizations information page 0−́The Bridge0+́. The dissemination process will be ongoing until completion of the proposal. Staffing issues have the attention of many researchers; these researchers are looking for avenues to resolve these issues and assist in making the patients and the nurses satisfied and happy. The end of this article will have a complete literature review of fifteen studies and their results pertaining to nurse patient ratios.

Increasing the nurse to patient ratios has been recommended as a means to improve patient safety. However, the cost effectiveness of increasing registered nurse (RN) staffing is controversial. This systematic review analyzes associations between hospital nurse staffing and patient outcomes with consideration of variables that could influence the primary association. The basic research questions were: 1. How is a specific nurse to patient ratio associated with patient outcomes (i.e., mortality; adverse drug events, nurse quality outcomes, length of stay; patient satisfaction with nurse care)? How does this association vary by patient characteristics, nurse characteristics, organizational characteristics, and nursing outcomes? 2. How is a measure of nurse work hours (hours per patient or patient day) associated with the same patient outcomes? 3. What factors influence nurse staffing policies? 4. What nurse staffing strategies are effective for improving the patient outcomes listed in question 1? 5. What gaps in research on nurse staffing and patient outcomes can be identified to address in future studies? Questions 1, 2, and 4 are addressed in the systematic review using meta-analytic approaches. The literature associated with question 3 does not lend itself to meta-analysis. Questions 1 and 2 address the same basic association but employ two different measures of nurse staffing. The nurse to patient ratio relies on a general ratio, which may include all nurses assigned to a unit, including non-clinical time, whereas nurse work hours look specifically at nurses involved in patient care. Even beyond this distinction, the varied ways staffing rates are calculated complicates pooling data.

Research evidences suggested that regulated nurse staffing influences quality of care, patient outcomes and job satisfactions (AACN, 2003). The purpose of this study is to examine the relationship between nurse staffing and patient outcomes, as well as nurses' job satisfaction at St. Francis hospital in Illinois. Solutions for this problem are implement minimum nurse-to-patient ratio, provide in-services for the nurses to handle patients with high acuity, provide continuity of care and plan the staffing according to the patients' acuity. The ratio should be one to five on medical-surgical unit to secure patient safety and quality of service. In ICU, if the nurse oversees one or two patients, there is significant improvement in patient's outcome and reduction in mortality rat.
