La correction classique de cette rétromaxillie est réalisée par des procédés de chirurgie orthognatique utilisant une ostéotomie de Lefort,plusieurs études ont rapporté un taux de récidive total ou partiel non négligeable. Depuis plusieurs années,la distraction osseuse maxilaire apparaît comme une approche alternative intéressante de la correction de ces hypoplasies maxillaires. Après des rappels généraux d'embryologie et d'anatomie,nous aborderons la distraction pour développer l'histoire de cette technique,les principes,mais également une approche histologique. Ensuite nous nous intéresserons aux fentes labio-maxillopalatines,leur étiopathogénie,les différentes formes liniques et le rôle de la chirurgie et des traitements primaires dans les troubles de la croissance faciale. Enfin nous présenterons les bénéfices qu'a pu apporter la distraction dans le traitement des fentes,et nous l'illustrerons par des cas cliniques.

Les chirurgies primaires, bien que nécessaires pour l’établissement d’une phonation et ventilation physiologiques, entraînent inévitablement des cicatrices rétractiles contraignant la croissance des maxillaires et aboutissant à des dysmorphoses de classe III squelettique. L’orthopédie visant à stimuler la croissance des maxillaires par protraction apparaît comme une thérapeutique utile dans la normalisation des relations maxillo-mandibulaires chez les enfants atteints de FLAPs. L’avènement des ancrages osseux, en particulier des mini-plaques permet de limiter voire annuler les effets parasites dento-alvéolaires rencontrés avec le masque facial conventionnel. Néanmoins, la protraction maxillaire sans chirurgie orthognathique a ses limites dans les cas les plus sévères. Une solution thérapeutique avec chirurgie orthognathique est alors indiquée : Le Fort I conventionnel, distraction ostéogénique maxillaire totale ou segmentaire antérieure maxillaire. Cette thèse a pour objectif de mettre en lumière la prise en charge des enfants atteints de FLAPs, en particulier les différentes options thérapeutiques de protraction maxillaire chez les enfants atteints de FLAPs

Les mécanismes qui aboutissent à la formation des fentes, isolées ou syndromiques, ont une origine multifactorielle avec une interaction entre des facteurs génétiques et environnementaux. L'objet de ce travail a pour but de décrire la prise en charge multidisciplinaire dans le traitement des fentes en insistant particulièrement sur l'importance de l'orthophonie avec la nécessité de réaliser un bilan précoce afin d'évaluer les atteintes des structures fonctionnelles et de programmer une rééducation adaptée. L'orthophonie aura un rôle de conseil et de prévention. Elle offrira aux équipes des éléments décisionnels par des examens cliniques, phonétiques mais aussi paracliniques. Après réparation chirurgicale, l'orthophoniste évaluera et s'assurera de la mise en place du système phonétique de l'enfant et surveillera son évolution langagière. Un suivi régulier ainsi que des contrôles à la puberté seront recommandés. La considération de l'enfant dans sa globalité est primordiale pour permettre, au fil des années, son développement personnel et sociétal.

Book excerpt: Introduction : le calendrier de prise en charge des fentes labio-palatines reste un sujet de débat, autant pour les techniques chirurgicales que par leur âge de réalisation, chaque geste pouvant avoir des conséquences néfastes à long terme. La chéïlorhinoplastie primaire est l'un des points les plus discutés, entre partisans d'une chirurgie précoce (avant 1 mois), ou plus tardive (de 3 à 6 mois). Matériel et méthodes : ont été étudiés les patients porteurs de fente labio-palatine unilatérale complète non syndromique pris en charge dans deux centres de Chirurgie Maxillo-Faciale, dont le calendrier ne varie que sur le moment de réalisation de la chéïlorhinoplastie primaire. Le centre de Caen la réalise à 1 mois, quand celui d'Amiens la pratique à 6 mois, précédée du port d'une plaque palatine passive. Un groupe de patients de chaque centre a d'abord été évalué à 6 mois sur moulages pour comparer l'effet de la chéïloplastie néonatale et du port de la plaque palatine réadaptée. Puis des groupes de patients de 5-6 ans et de 10-11 ans issus de chaque centre étaient comparés par tranche d'âge sur des analyses céphalométriques de profil avec un groupe contrôle, à la recherche de signes de rétromaxillie. Résultats : à 6 mois, l'effet des orthèses passives était comparable à celui d'une chéïlorhinoplastie précoce sur la diminution de la largeur antérieure de la fente et de l'angle basal antérieur. A 5 ans, les patients étudiés dans les deux centres ne présentaient pas de rétromaxillie comparativement au groupe contrôle (angles ANB et F1MF1m comparables). A 10 ans par contre, on retrouvait une tendance à la rétromaxillie statistiquement significative pour le groupe de Caen uniquement (angle ANB et F1MF1m diminués par rapport au groupe contrôle). Discussion : ce travail a permis d'étudier l'effet spécifique de l'âge de réalisation de la chéïlorhinoplastie primaire sur la croissance maxillaire, après s'être assurés en premier lieu que l'utilisation de plaques palatines ne constituait pas un biais. Tout en restant conscients des limites statistiques de petites séries comme celle-ci, les résultats semblent orienter vers une tendance à la rétromaxillie à distance dans le groupe opéré en période néonatale. Conclusion : le suivi à plus long terme de cette cohorte sera nécessaire afin de compléter ces résultats, prenant en compte le recours à un masque de Delaire ou à une chirurgie orthognathique après l'adolescence.
