Book excerpt: Le «protéome» est l’ensemble des protéines exprimées par une cellule dans des conditions données à un moment donné. Son étude, « la proteomique », permet d’identifier les protéines, d’étudier leur fonction, ainsi que les interactions qui peuvent exister entre elles à l’intérieur ou à l’extérieur d’une cellule. L’approche protéomique trouve un intérêt dans diverses disciplines médicales, notamment en parodontologie. Les critères cliniques traditionnels trouvent leurs limites pour identifier les sites parodontaux actifs, la réponse tissulaire du patient au traitement parodontal actif ou encore pour dépister les patients susceptibles à l’apparition et à la progression des maladies parodontales. La salive, fluide biologique complexe, reflète l’état de santé orale et systémique des individus. Les différentes protéines salivaires, présentes au cours des maladies parodontales, et les changements protéiques qualitatifs et quantitatifs entre les sujets sains et les sujets atteints de maladie parodontale sont des sources d’informations biologique et clinique. Ces peptides et protéines d’intérêt sont des biomarqueurs et sont à la base du diagnostic moléculaire en parodontologie. Bien que de nombreux biomarqueurs de la maladie parodontale aient déjà été identifiés dans la salive par différentes méthodes biologiques (biochimiques, immunologiques), les résultats sont restés inutilisés en clinique. L’approche protéomique, plus discriminante à grande échelle, redonne un nouvel intérêt au diagnostic salivaire en parodontologie. L’objectif de cette thèse est donc de réaliser une revue de littérature sur l’apport de l’analyse protéomique salivaire dans l’étude des maladies parodontales et de leurs liens avec les autres maladies (obésité, diabète). Après des rappels sur le diagnostic salivaire en parodontologie, nous développerons les différents outils techniques et méthodologiques utilisés en protéomique salivaire puis nous présenterons les études cliniques utilisant une approche protéomique salivaire et les principaux biomarqueurs identifiés.

Un déséquilibre entre les bactéries et la réponse inflammatoire aboutit à la destruction tissulaire et aux signes cliniques des parodontites. La destruction parodontale serait sous la dépendance de certains gènes; leur identification a été menée depuis plus d’une décennie. L’objectif de ce travail est de comprendre les bases génétiques des maladies et d’évaluer le possible caractère héréditaire des maladies parodontales, qu’elles soient chroniques ou agressives. L’identification des marqueurs de risque génétique des parodontites, est examinée en fonction de l’origine ethnique et/ou raciale des populations qui ont été plus particulièrement étudiées. Si aucun gène à effet pathogénique majeur n’a été découvert jusqu’à aujourd’hui, certains polymorphismes sont de possibles marqueurs de susceptibilité ou de sévérité des maladies parodontales. Au plan clinique, les résultats sont conflictuels entre les différentes populations étudiées. Il n’y a pas de preuve scientifique suffisante à l’utilisation de tests génétiques au plan thérapeutique.

La méthode de référence est la technique de culture bactérienne permettant l'identification des pathogènes de manière non spécifique et la réalisation d'un antibiogramme. Mais la méthode de prélèvement et de transport peut avoir une influence sur les résultats car les pathogènes sont vivants. La PCR en temps réel permet une identification et une quantification précise de l'ADN grâce à une analyse entièrement automatisée. Cependant, cette méthode détecte qu'un faible nombre de bactéries, elle est spécifique, ciblée par des sondes prédéterminées. Des études ont été réalisées pour comparer les diagnostiques bactériens, l'efficacité in vitro des antibiotiques en fonction des méthodes et des molécules utilisées. Ces résultats sont confrontés avec les recommandations de l'AFSSAP qui sont à ce jour les références pour les thérapeutiques parodontales anti-infectieuses en France. En fonction de ces années de recherche en collaboration avec la faculté et chirurgie dentaire de Nantes et le laboratoire de maladies infectieuse de Toulouse, l'utilisation des prélèvements bactériens, cliniques et techniques varient selon les objectifs de diagnostic bactérien, de traitement et de la prise en en charge infectieuse des maladies parodontales.
