Book excerpt: Aortic Valve Transcatheter Intervention Calcific aortic stenosis (AS) is the most common heart valve anomaly, with a largely age-dependent prevalence, a calculated annual incidence rate in the range of 4-5% in general populations and up to 6% in patients aged 75 years and over. Surgical aortic valve replacement (SAVR) was previously the only option available to patients with symptomatic, severe aortic stenosis. After the first-in-human transcatheter aortic valve implantation (TAVI) was performed by Alain Cribier in 2002, the treatment strategy for patients with symptomatic AS has been revolutionized. Since then, TAVI has grown exponentially, as a result of accruing evidence demonstrating safety and efficacy, and reduced invasiveness compared with SAVR. TAVI devices are continuously expanding to include several valve design options. As this strategy is continuously evolving to treat younger patients and lower-risk populations, aside from the long-term durability of the valve systems, procedural safety will become the focus of newer-generation devices. This book is a practical handbook devoted to the optimization of TAVI procedures, through a focused containment of complications. Book excerpt: Several techniques have evolved during the past few decades for addressing lesions affecting the aortic valve, aortic root, and proximal ascending aorta. As diseases affecting the aortic valve and aortic root are varied, it is not surprising that several operative techniques have evolved and some of these techniques are based on achieving the long term durability, hemodynamic advantage, and perhaps the increased survival of the patients. The aortic root is attached to the left ventricular outflow tract and continues as the proximal ascending aorta. The proximal ascending aorta is composed of sinus aorta or aortic sinuses from which the aortic valve is suspended. The diseases can involve the aortic valve, sinus aorta, and aortic root in common such as in aortic aneurysms, ascending aortic dissection, and bacterial endocarditis. Surgical corrective procedures to address these lesions are so described in common with procedures on the aortic valve. During the past decade with the emergence of new technologies the aortic valve and aortic root surgery has changed with introduction of procedures such as catheter based interventions for aortic valve replacement, mini sternotomy approach, aortic stent grafting and valve replacement, and introduction of endoscopic surgery and robotics. Despite these advances, the basic surgical techniques on the aortic valve have not changed and these basic surgical procedures are described in this text in detail in addition to xenograft and homograft replacement of the aortic root and aortic valve. In addition to the detailed description of the surgical techniques involving the aortic root and aortic valve, a brief discussion of the normal anatomy of the aortic root, aortic valve, and closely related anatomical structures is provided to enable the readers to perform surgical procedures which are safe without untoward technical complications. Several practical issues encountered at the operating table while performing certain surgical techniques are addressed for accomplishing the operative techniques with ease.

Book excerpt: Aortic valve stenosis has become the most prevalent valvular heart disease in Europe and North America, and is generally caused by age-related calcification of the aortic valve. For most patients, severe symptomatic aortic stenosis needs effective mechanical relief in the form of valve replacement before it becomes a lethal obstruction to outflow. This dissertation focuses on the evaluation and performance of the stentless Freedom SOLO bioprosthesis. We found several clinical and hemodynamic advantages for the Freedom SOLO. Due to the implantation technique, there are few patients that require the implantation of a permanent pacemaker. In addition, the postoperative valvular gradients are excellent and remain stable during midterm follow-up, with large effective orifice areas. Furthermore, structural valve deterioration and aortic valve reoperation are infrequent. However, because of its specific design and implantation technique, not every patient profits from this valve and therefore patient selection is crucial. Based on our extensive implant experience we presented our contraindications for the use of the Freedom SOLO. We discussed several options in the event of a degenerated Freedom SOLO and reported for each technique the pitfalls. Additionally, 4D flow MRI, a new imaging modality, was used to compare stentless and stented valves in a pilot study. At this moment, the Freedom SOLO should be considered for use in patients with a small aortic root, or in patients that in any other way are at risk for prosthesis-patient mismatch, or in patients that could profit from the superior hemodynamic performance and the large effective orifice area.
