Chadwick, of Boston, recommends the following method of intra-uterine injection which is safe, and its general adoption might prevent many of the accidents now reported Dr. Chadwick says. Injections into the vagina should be made with the patient lying upon her Side until the fluid begins to ooze from the vulva. The patient is then gradually turned upon her face while the injection into the vagina is continued; by this plan the vagina is distended to its utmost, as in the knee and elbow position, while the uterus gravitates into the abdominal cavity and allows the fluid to flow through the patulous cervical canal into the cavity of the organ with the force of pneumatic pressure, any air thus forced into the vagina by the syringe will remain in the vagina, and thus the possible danger of its passage into the uterine sinuses be avoided. Book excerpt: "A text-book for nurses on the use of antiseptics in gynaecology and midwifery, by the obstetrician and member of the Royal College of Physicians, John Shaw, which provides an insight into not only the developing level of antiseptic knowledge and practice at the time, but also into the state of formalised training of nurses. Dedicated to both Semmelweis and Lister, the work is based upon a series of lectures given to nurses between 1887-1889 at the Hampstead Home Hospital, in an effort to combat what Shaw felt was a subject 'still so imperfectly understood by the majority of nurses' (Preface). It is his hope that 'these lectures may be of some use, especially to that large class of nurses who have not had the advantage of the thorough instruction which now characterises the curriculum of the monthly nurse; for this reason, a short glossary of terms, as might possibly present difficulty, is appended' (ibid). Shaw goes on to present in the first chapter a brief historical synopsis of the introduction of antiseptics into general obstetrical practice, noting the importance of Semmelweis in particular, before outlining in the remaining chapters the main causes of infection, and the various antiseptic precautions and practices to be used. The work is illustrated by four plates including a microsopic representation if various germs, together with an evocative photograph showing two obstetric nurses preparing for theatre. A folding statistical table clearly charts the progress of two patients recovering from a blood cyst on the right ovary, and a caesarean section. It is, however, the chromolithographed frontispiece that most catches the attention, depcting a number of series of 'tubes of beef-tea or beef-jelly, which had been successfully sterilised in the manner already described, but were subsequently exposed to certain conditions of germ influence, each of which is distinctly illustrative of some error in obstetric nursing.
