Book excerpt: The introduction of antiretroviral therapy (ART) in middle and low income countries is arguably one of the most meaningful outcomes recorded in the fight against HIV and AIDS. A record number of some 6.2 million people living with HIV and AIDS are reported to be benefiting from the treatment, which is reported to have risen by 19 per cent between 2010 and 2011 and as a result of this, the region has also enjoyed a significant decline in AIDS mortality. This volume is the outcome of the call for abstracts put out by OSSREA in 2011 for senior researchers, social scientists and practitioners to write scientific articles on issues surrounding ARVs. The volume contains eight chapters organized into four sections: ART and quality of life; Adherence to ART; Traditional medicine and ART; and Sexual behaviour of ART attendants. The chapters are contributed by Academics and researchers from three different African countries: four from Ethiopia, two from Uganda and two from Zimbabwe.

Book excerpt: Models for Life: Advancing antiretroviral therapy in sub-Saharan Africa is based on two conferences that were held in Copenhagen and Uppsala, in September 2004. The events brought together more than 70 key actors from Denmark, Sweden and Uganda in the field of HIV/AIDS and its treatment: antiretroviral therapy. The conferences were unique in that each panel and the subsequent discussion brought together researchers, donors and representatives from non-governmental organisations, in order to link theory, operations research and practice. Each section begins by introducing the core issues to be addressed. The different presentations are then put into perspective by summarising the main questions from the participants and the other panellists. In addition, the editors have further developed some of the key points made by drawing on current literature in the field. The final section concludes with a list of take-home messages for non-governmental organisations.

Book excerpt: With the development of effective antiretroviral therapies (ART) in the mid-1990s, HIV became a treatable although serious condition, and people who are adherent to HIV medications can attain normal or near-normal life expectancies. Because of the success of ART, people 50 and older now make up a majority of people with HIV in high-income countries and other places where ART is accessible. The aging of the HIV epidemic is a global trend that is also being observed in low- and middle-income countries, including countries in sub-Saharan Africa, where the greatest number of older people with HIV reside (3.7 million). While globally over half of older adults with HIV are in sub-Saharan Africa, we have little information about the circumstances, needs, and resiliencies of this population, which limits our ability to craft effective policy and programmatic responses to aging with HIV in this region. At present, our understanding of HIV and aging is dominated by information from the U.S. and Western Europe, where the epidemiology of HIV and the infrastructure to provide social care are markedly different than in sub-Saharan Africa. Aging with HIV in Sub-Saharan Africa addresses this gap in our knowledge by providing current research and perspectives on a range of health and psychosocial topics concerning these older adults from across this region. This volume provides a unique and timely overview of growing older with HIV in a sub-Saharan African context, covering such topics as epidemiology, health and functioning, and social support, as well as policy and program implications to support those growing older with HIV. There are very few published volumes that address HIV and aging, and this is the first book to consider HIV and aging in sub-Saharan Africa. Most publications in this area focus on HIV and aging in Uganda and South Africa. This volume broadens the scope with contributions from authors working in West Africa, Botswana, and Kenya. The range of topics covered here will be useful to professionals in a range of disciplines including psychology, epidemiology, gerontology, sociology, health care, public health, and social work.

They have traditionally been viewed as repositories of information and wisdom, and are critical pillars of the community but as the HIV/AIDS pandemic destroys family systems, the elderly increasingly have to deal with the loss of their own support while absorbing the additional responsibilities of caring for their orphaned grandchildren. Aging in Sub-Saharan Africa explores ways to promote U.S. research interests and to augment the sub-Saharan governments' capacity to address the many challenges posed by population aging. Five major themes are explored in the book such as the need for a basic definition of "older person," the need for national governments to invest more in basic research and the coordination of data collection across countries, and the need for improved dialogue between local researchers and policy makers. This book makes three major recommendations: 1) the development of a research agenda 2) enhancing research opportunity and implementation and 3) the translation of research findings.

Book excerpt: This dissertation analyses the impact of the prevailing international intellectual property (IP) system on the access to antiretroviral treatment in Sub-Saharan Africa. For the past decades, HIV/AIDS have been causing an unprecedented public health crisis in the region, where approximately 1.2 million people die every year of AIDS-related diseases. Despite the fact that a life-saving treatment exists, the high prices on antiretroviral drugs make them inaccessible to a vast number of patients. Arguably, this is the direct consequence of the World Trade Organization's Agreement on Trade-Related Aspects of Intellectual Property Rights (TRIPS), by virtue of which signatory states agreed to implement minimum standards of IP protection within their domestic laws. Now, multinational pharmaceutical corporations are entitled to obtain and enforce patents in the jurisdiction of all WTO members states. Since generic competition is prevented during the patent period, these are entitled to arbitrarily set the prices for their new drugs, which tend to be excessively high. The TRIPS agreement establishes a set of opt-out clauses for those cases of national emergencies caused by a public health crisis (such as the HIV/AIDS epidemic). Governments are entitled to grant compulsory licenses and to allow for parallel imports of patented drugs, in order to make treatment available at affordable prices. However, the U.S. and the E.U. have continuously opposed the use of TRIPS-get out clauses under the menace of trade retaliations. Further, multinational pharmaceutical corporations have threaten to file law suits against governments issuing compulsory licenses and allowing for parallel imports of their drugs. The WTO Ministerial Conference Declaration on the TRIPS Agreement and Public Health, agreed during the Doha Round of Negotiations in 2001, reinforced the right of member states to issue compulsory licenses and to freely determine the grounds under which they might be granted. Nonetheless, the U.S and the E.U. have been celebrating free trade agreements with developing countries containing provisions that increase the protection and enforcement of IP, which hinder the use of TRIPS-get out clauses. Despite the attempts by the international community and civil society to enhance the access to antiretroviral treatment in the developing world, at the present date many patients cannot afford the high prices of antiretroviral drugs.

Strategies for agricultural intervention are of particular importance in sub-Saharan Africa, given the fact that most of the countries hardest hit by the epidemic are heavily reliant on agriculture. Different agricultural sector based mitigation strategies are discussed. It is further argued, however, that such interventions need to be complemented by interventions from the health and other sectors, in particular by treatment regimes including access to anti-retroviral drugs. Book excerpt: HIV has become one of the most feared diseases in the world. It has claimed millions of lives, especially in Sub-Saharan Africa. Although it is impossible to cure, studies have shown that using antiretroviral therapy (ART) can greatly improve quality of life and decrease viral load. There are four main classes of ART—NRTIs, NNRTIs, protease inhibitors, and fusion inhibitors. Although access to ART is improving, it is vital that its use is increased in Sub-Saharan Africa, where the prevalence can be as high as 33%. This thesis explores the current need of ART in Sub-Saharan Africa, as well as the existing coverage. In addition, it addresses the obstacles preventing ART distribution and use—both provider and patient-related. Provider-related obstacles include limited availability of drugs, total cost, lack of healthcare staff and facilities, lack of organization, and government regulations. Patient-related obstacles include cost, transportation, geographic proximity to healthcare facilities, side effects and hunger, pill burden, hospital-related factors, cultural beliefs, social stigma, lack of adequate counseling, and drug resistance. It is important to be proactive, assess ART delivery, consider suggestions for improvement, and tackle the HIV epidemic.

Book excerpt: This thesis describes the virological and clinical outcomes of children with HIV in sub-Saharan Africa who start antiretroviral treatment (ART). We show that pretreatment HIV drug resistance among children is a serious problem in sub-Saharan Africa and that its prevalence is increasing. Children who start first-line ART have much lower virological suppression rates compared to adults and data on long-term treatment outcomes of these children are largely missing. Outcomes of children on second-line ART, containing a protease inhibitor, are relatively favorable for young children, but the rate of virological failure among adolescents is alarmingly high. Although major improvements have been made in HIV treatment in sub-Saharan Africa over the past 15 years, these advances have not been realized on the same scale in children. We conclude that, in order to improve pediatric HIV care in sub-Saharan Africa, a combination of better drugs, improved drug access, and child-friendly HIV treatment is urgently needed.
