Increasing incidence has pushed malignancy of the upper aerodigestive tract into the first rank of cancer. In some countries it follows bronchial carcinoma in frequency and is more common than gastro-intestinal and gynaecological cancer. This increasing incidence makes it difficult to train enough highly specialised staffwho are also responsible for the care of many other patients, requiring sophisticated microsurgery of the ear and nose. The question of quality is even more difficult. Oncological success in the treat ment of head and neck cancer is bought at the price of crippling of vital functions such as eating, breathing, voice and sight and furthermore of striking aesthetic de formity. Mutilation of this highly functional collection of organs is more keenly felt than that of any other region of the body. It is vital, therefore, that the surgeon keeps up with the recent achievements of functional surgery in order to offer the best ser vice to his patients. Cancer of the larynx is no exception. Despite newer techniques of radiotherapy and chemotherapy, surgery still gives the best oncological results. This requires a wide spectrum of operations varying from minimal ablation to total laryngectomy.

Book excerpt: The cries of infants and children are familiar to essentially all adults, and we all have our own common sense notions of the meanings of various cries at each age level. As is often the case, in the study of various aspects ofhuman behavior we often investigate what seems self evident to the general public. For example,if an infant cries, he or she needs atttention;if the cry is different than usual, he or she is sick; and when we areupsetby othermatters, children's crying can be very annoy ing. As a pediatric clinician often faced with discussing with parents their concerns or lack of them with respect to their children's crying, these usual commonsense interpretations were frequently inadequate. As this book illustrates, when we investigate such everyday behaviors as children's crying and adults' responses to crying, the nature of the problem becomes surprisingly complex. As a pediatrician working in the newborn nursery early in my career, I knew from pediatric textbooks and from nursery nurses, that newborn infants with high, piercing cries were often abnormal. In order to teach this interestingphenomenon to others and tounderstand under what circumstances it occurred, I found I needed to know what consti tuted a high-pitched cry or even a normal cry, for that matter, and how often this occurred with sick infants. Certainly I saw sick infants who did not have high-pitched cries, but I still wonderedif their cries were deviant in some other way.

In this classic of French cultural studies, Georges Didi-Huberman traces the intimate and reciprocal relationship between the disciplines of psychiatry and photography in the late nineteenth century. Focusing on the immense photographic output of the Salpetriere hospital, the notorious Parisian asylum for insane and incurable women, Didi-Huberman shows the crucial role played by photography in the invention of the category of hysteria. Under the direction of the medical teacher and clinician Jean-Martin Charcot, the inmates of Salpetriere identified as hysterics were methodically photographed, providing skeptical colleagues with visual proof of hysteria's specific form. These images, many of which appear in this book, provided the materials for the multivolume album Iconographie photographique de la Salpetriere. As Didi-Huberman shows, these photographs were far from simply objective documentation. The subjects were required to portray their hysterical "type"—they performed their own hysteria. Bribed by the special status they enjoyed in the purgatory of experimentation and threatened with transfer back to the inferno of the incurables, the women patiently posed for the photographs and submitted to presentations of hysterical attacks before the crowds that gathered for Charcot's "Tuesday Lectures." Charcot did not stop at voyeuristic observation. Through techniques such as hypnosis, electroshock therapy, and genital manipulation, he instigated the hysterical symptoms in his patients, eventually giving rise to hatred and resistance on their part. Didi-Huberman follows this path from complicity to antipathy in one of Charcot's favorite "cases," that of Augustine, whose image crops up again and again in the Iconographie. Augustine's virtuosic performance of hysteria ultimately became one of self-sacrifice, seen in pictures of ecstasy, crucifixion, and silent cries.

However, it was not until the th middle of the 19 century that a more comprehensive analysis of the role of the thymic gland and its role in pathogenesis began to emerge. Currently, while the knowledge gained on the diverse aspects of the thymic gland has furthered our understanding of its role in a gamut of processes, more knowledge is still being sought, and by no means is a full understanding of the gland’s physi- ogy and pathology complete. Different aspects, including its purported endocrine function, its association with other autoimmune diseases like multiple sclerosis, rheumatoid arthritis, and lupus erythematosus, among others, are under evaluation and research.

They are time locked with an external stimulus and can be influenced by subjective intentions. Evoked potentials have become increasingly popular for clinical diagnosis over the last few years. Evoked potentials from the visual system are used by ophthalmologists in order to localize the abnormalities in the visual pathway. The otologists are mainly involved in brainstem auditory evoked potentials, while the pediatricians, neonatologists, neurologists and clinical neurophysiologists make use of multimodal stimulation. The psychiatrists and psychologists, generally, examine the slow potentials such as P300 and CNV. Anesthesiologists use short latency somatosensory and visual evoked potentials in order to monitor the effectiveness of the anesthesia. Pharmaco evoked potentials are very promising measures for the quan tification of the effectiveness of drug action on the cerebral cortex. Urologists are more and more involved in pudendal somatosensory evoked potentials and in the intensive care unit evoked potentials are used in order to monitor the functional state of the central nervous system of the patient. This overwhelming number of examinations and exam ina tors clearly demonstrates the need for guidelines and standardization of the methods used. The evoked potential metholody is restricted by the relative poor signal to noise ratio. In many diseases this signal to noise ratio decrease rapidly during the progression of the illness. Optimal technical equipment and methodology are therefore essential.
