During the past year the work of the Hospital has progressed smoothly and a constant effort has been made to maintain the standard of accomplishment in relation to each of its numerous functions. The primary role of the Hospital is the care and treatment of the mentally sick. It has also to make a diagnosis in the case of a large number of individuals, whose problems are to a large extent social problems, such as individuals referred from the courts under various charges, individuals whose difficulties are under consider ation by welfare organizations, and whose problems require some technical analysis by a psychiatrist, individuals, both juvenile and adults whose adapta tion at school or in the factory and Store has been so unsatisfactory that special technical examination is required. In addition to the diagnosis and treatment of patients, the Hospital is responsible for prosecuting research into the nature and causes of mental disorders and into the problems of treatment. Another important function of the Hospital is as a training centre for many types of workers in the field of mental disorders; these workers make the valuable contribution of their services to the Hospital, while they get from the Hospital opportunities for work which are highly appreciated. The past year has seen the steady and fruitful development of the program of the previous years. This program consists of two closely correlated activities, first the care and treatment of the large number of patients referred to the Hospital suffering from a great variety of disorders, and secondly investigation into the causes, the mechanisms, the social relations of mental disorder with special attention to the possibilities of prevention and cure. Along both the above lines work has proceeded very smoothly during the past year and from the appended reports from the various departments and from the statistical tables some idea may be gained as to the extent of the clinical work and the lines of special investigation. On the Co-ordination of the Various Departments. In regard to special investigations the individual worker may carry on a somewhat isolated activity or may temporarily have the assistance of some colleague for a conjoint investigation. It is very different with regard to the care and study of the patients, where such isolated activity of the physician is not possible, but where the cooperation of a great number of workers is involved in every single case. The reception, study and treatment of every individual case involves so many different activities that the closest cooperation of the various departments is one of the most essential conditions for efficient work in the hospital. Continuous effort has been paid to the development of this cooperation and with very gratifying success. No rules nor special machinery will ever bring about such cooperation, unless there is thorough good will and mutual understanding between the workers. The Hospital is to be congratulated upon the spirit of loyal and hearty cooperation which is common to all the workers, and which enables the complicated problems presented by the large number of admissions to be adequately dealt with. The staff is so organized that the following requirements are met: the necessary attention to the recognized formalities of admission; immediate clinical examination and emergency treatment where required; the methodical and systematic general investigation of the patient's disorder, involving a study of the whole past evolution of the case and of the environmental situation; the detailed investigation of more special aspects of the case by the clinical pathologist, the psychologist, the social worker; opportunities for group discussion of the diagnosis and treatment of the individual case; the timely arrangement of steps which are necessary if the patient is to be transferred to some other hospital. It is in view of these requirements, which have to be met even during the rather frequent periods of an embarrassing influx of patients, that a smoothly running machine is so necessary. In this connection it seems appropriate to refer to the gratifying way in which the Chief Executive Officer and the Chief Medical Officer have coordinated their activities. The Role of the Executive Staff. The members of the executive staff have the responsibility for the general running of the Hospital with supervision of everything pertaining to the maintenance of the necessary personnel and supplies and to the general administration of the various services. This history begins in a social context in which treatment and rehabilitation were emerging as predominant after America's prisons and asylums had been broadly acknowledged to be little more than embarrassing failures. The resulting progressive agenda was evident: to develop new, more humane and effective strategies for the criminal, delinquent, and mentally ill. The results, as Rothman documents, did not turn out as reformers had planned. For adult criminal offenders, such individual treatment could be accomplished only through the provision of broad discretionary authority, whereby choices could be made between probation, parole, indeterminate sentencing, and, as a measure of last resort, incarceration in totally redesigned prisons. For delinquents, the juvenile court served as a surrogate parent and accelerated and intensified individual treatment by providing for a series of community-based individual and family services, with the newly designed, school-like reformatories being used for only the most intractable cases. For the mentally ill, psychiatrists chose between outpatient treatments, short-term intensive care, or as last resort, long-term care in mental hospitals with new cottage and family-like arrangements. Rothman shows the consequences of these reforms as unmitigated disasters. Despite benevolent intentions, the actual outcome of reform efforts was to take the earlier failures of prisons and asylums to new, more ominous heights. In this updated edition, Rothman chronicles and examines incarceration of the criminal, the deviant, and the dependent in U.S. society, with a focus on how and why these methods have persisted and expanded for over a century and a half despite longstanding evidence of their failures and abuses.
