Book excerpt: Excerpt from Annual Report of the State Board of Health of Maryland: For the Year Ending December 31, 1904 To His Excellency Edwin Warfield, Governor of Maryland. Sir: The health of the State during the past two years has been threatened by no unusual epidemic. Smallpox, which has given us a good deal of concern during the past eight years almost disappeared. For a period of eighteen consecutive months our chief city was wholly exempt. But four cases occurred in Maryland in 1905 down to December 10th, when a few cases appeared in Baltimore and its environs, and the year ends with a renewed menace, arising in the State of Virginia; not serious enough, however, to justify any alarm. During the year 1905, Typhoid Fever has been unusually prevalent. The mortality for this year will exceed that for 1904, when 508 deaths were charged to typhoid fever. The increased prevalence in 1905, included several institutional outbreaks. The typhoid history of 1905 has strongly stimulated public interest in the supervision of water supplies, and has emphasized need of better control of the production and distribution of milk. Tuberculosis killed 2,635 persons in Maryland in 1904, and a like number in 1905. The State has already provided satisfactory legislation for restricting the spread of this disease. With co-operation among local boards of health, the State Board of Health, and the charitable and philanthropic agencies, we can look for measurable profits in the control of this disease in the next ten years, and after that the gains should be larger. The two Acts of 1904 (Chapters 899 and 412) went into actual operation on May 1, 1905. More than 2,000 cases of tuberculosis are now registered, and the great majority of these have received the prophylactic supplies and the home instruction designed to prevent the communication of the disease to friends and attendants. The death of Mr. Frederick H.Smith and the appointment of Dr. S. Chase de Krafft to a position in the federal army necessitated a change of duties on the part of the members of the Board. Any inconvenience that might have resulted from these unforseen circumstances was remedied by the prompt and unremitting services of the Secretary, whose very ample report we have now the honor to present to your Excellency. The work done that is most important may be summarized as follows: The vital statistics law of the last General Assembly, the good effects of which have been already felt throughout the State. The bacteriological service of the State has been of great and generally recognized utility by the medical profession. Analyses have been made of about three hundred specimens of water from different parts of the State during the past year. This work has been done at the request of medical men, and frequently by the heads of families, which requests are made through local health officers. The inspector has, by direction of the Board, visited and inspected slaughter-houses throughout the State. He also interviewed all the undertakers in all the important towns, and instructed them concerning the operations and technical details of the vital statistics law. In addition to this he has travelled extensively in examining water supplies. In his manifold labors he has greatly assisted the Secretary. The Board now meets monthly and your Excellency will agree that the amount of necessary important business could not be properly discussed and treated with due consideration if we met as formerly, but four times a year. The number of local boards have increased in two years from twenty-five to thirty-nine. This of itself proves the extent and progress of the work of the Board. These thirty-nine boards of health give employment to forty-two health officers, not including the fifteen vaccine physicians in Washington County, all of whom have definite sanitary duties aside from vaccination. The Board of Health of Baltimore is not included in this list. At the last meeting of the Board, the second week in January, 1900, the following officers and employees were elected: Dr. William H. Book excerpt: Excerpt from Annual Report of the State Board of Health of Maryland: For the Year Ending December 31, 1912 In accordance with the provisions of Sections 5 and 6 of Article 43 of the Code of Public General Laws of Maryland. I transmit herewith the report of the State Department of Health for the year 1912. The report begins with the work of the Bureau of Vital Statistics, and shows that good results may be expected from the new registration law which became effective on July 1st, 1912. An appreciable improvement is noted, especially in the registration of births, during the second half year, and at this writing (October 31st, 1913) we have reason to believe that the State of Maryland, within a year, will be admitted by the Census Bureau to the United States Registration Area for Births. Admission to the Registration Area means that the Census Bureau believes the registration within the State to lack no more than 10 per cent. of completeness. To be counted as a registration State will not, therefore, satisfy the purpose of the State, as expressed in the registration law. Maryland has been in the registration area for deaths for the past eight or nine years, but the mortality is not fully accounted for in some counties, notably in St. Mary's and Garrett. In each of these counties the local registration offices have been taken over by the State Board of Health and operated for a time at the expense of the county. These two counties have thus incurred expense much in excess of the cost of the proper operation by their own agents, but both counties relapsed very rapidly within a short time after the withdrawal of the State. Our intervention showed that Garrett and St. Mary's Counties did not record, previous to that intervention, more than one-sixth or one-seventh of the deaths occurring. The importance of the conservation of human life is fully recognized by this Board, and we believe that if properly equipped and supplied with sufficient financial support from the State, that very far reaching results in the betterment of the people of Maryland can be accomplished. I am fully in accord with the recommendations of the Secretary, that the local health officers throughout the State should be selected, as far as possible, on the basis of efficiency and special training and experience in sanitary science, and also that they should receive adequate remuneration for the important and responsible duties which they must perform. The greatest immediate need of the Department is for an additional appropriation sufficient to enable us to equip and maintain a Bureau of Sanitary Engineering, which we were given the authority to establish by the General Assembly of 1910. We also need, as pointed out by the Secretary, extensive amendments to the laws for the registration of births and deaths. I would also point out the importance of the work which we are doing in the control of tuberculosis and the necessity for an increased appropriation to enable us to continue the provisions of the State laws in operation.
