I, and the people from my laboratory, have been much involved with developing Nan'I for the diagnosis and treatment of hyperthyroidism and well-differentiated thyroid cancer. Second, I was a participant throughout the entire symposium upon which this book is based. Our latest '311-labeled compound, [l3lll metaiodobenzylguanidine, took us 13 years to develop. It has succeeded in diagnosing and treating adrenergic tumors '31 under circumstances where Na 1 would have failed for the diagnosis and treatment of thyroid cancer. I therefore hope that more scientists will be attracted to this field. This book is written by intelligent, experienced investigators who are knowl edgeable. It is a must for anyone new in the field. For the experienced, I would like to remind you that I attended the entire symposium; the breadth and timeliness of the update has helped us to redirect some of our current efforts into more scientifically productive channels. Book excerpt: Radiophannaceuticals labeled with short-lived radionuclides are utilized to unravel biochemical processes, and to diagnosis and treat diseases of the living body are-developed through extensive evaluation in ~iological models. 'fhC first attempt to compile information was a volume entitled ANIMAL MODELS IN RADIOTRACER DESIGN that was edited by William C. Eckelman and myself in 1983. The volume had a focus on the animal models that investigators were using in order to design radiotracers that displayed in vivo selectivity as measured by biodistribution and pharmacokinetic studies. A concern in the early days of nuclear medicine was species differences. Often a series of labeled compounds were evaluated in a several different animal models in order to gain confidence that the selected radiotracer would behave appropriately in humans. During the past 12 years there have been remarkable advances in molecular genetics, molecular biology, synthetic radiopharmaceutical chemistry, molecular modeling and visualization, and emission tomography. Biological models can now be selected that are better defined in terms of molecular aspects of the disease process. The development of high resolution PET and SPET for clinical applications facilitates the development of new radiopharmaceuticals by the use of models to quantitatively evaluate drug effects, and progression of disease, and hence to arrive at better diagnosis and treatments for animals and humans. With these advances there is an effective use of biological models, and the refinement of alternatives for the development of new radiophannaceuticals.

Safety and efficacy of the non-radioactive pharmaceuticals have attracted great attention during the last 10 years and so have similar aspects of radiopharmaceuticals during the later years. Regulatory agencies are extending their work also to the preparation of radiopharmaceuticals at hospitals and to requirements for registration of radiopharmaceuticals. In a fast developing field there might be tendencies to confrontation between interests and there have certainly been some tendencies to put undue restrictions on the use of radio pharmaceuticals due to the lack of understanding between the industry and the regulatory authorities and between regulatory authorities and hospitals. Much of this may have been due to lack of information and certainly is due to the lack of fundamental scientific knowledge in many radiopharmaceutical aspects. A fast and safe introduction of new radio pharmaceuticals and the proper handling of these requires a lot of development work, but also an understanding of how general principles from the non-radioactive drug field may be sensibly transformed into the radiopharmaceutical area. It may even require compromises between requirements for safety in different areas such as radiation protection and pharmaceutical aspects.

Book excerpt: Breast cancer, being a disease with very high prevalence in the female population, has permanently attracted the attention of biologists, biochemists, pathologists, surgeons, and oncologists. In the U.S. alone approximately 120,000 new cases are diagnosed and 37,000 deaths a year are recorded as an outcome. Early diagnosis, clear cut prognosis and the hope of new therapeutic approaches has spurred the enthusiasm of researchers, who with the advent of newer methodologies, tried to employ it in different approaches to the breast cancer patient. In spite of the efforts devoted towards the creation of new approaches, reagents and procedures taking advantage of special characteristics of the breast tissue were slow to come. In general, most diagnostic and prognostic techniques were those shared and applied to other carcinomas. In the early seventies we discovered the first sets of antigens associated with breast epithelial tissue both in the mouse and the human. The recognition of such group of molecules, present in the milk fat globule membrane, gave impetus to several studies in these areas. These mammary cell surface antigens proved useful in several diagnostic approaches such as radioimaging, histochemical and serum assay procedures. However, the apparently inherent limitations of polyclonal antibodies reduced the impact of these novel approaches.
