However, two serious drawbacks are associated with their use: hypotension during surgery and the development of tolerance in long-term epidural analgesia. When opiates are injected epidurally or intrathecally, numerous side effects such as itching, nausea and vomiting, or respiratory depression have been reported. The pathophysiology of complications arising during a major conduction block and measures to prevent them were discussed by 19 experts at the 4th International Symposium at DUsseldorf. This volume contains 21 papers presented on three topics: 1. tachyphylaxis in long-term epidural anesthesia, 2. hypotension due to epidural and spinal anesthesia, and 3. effects and dangers of epidural and intrathecal opiates. With regard to the safety of our patients during anesthesia, current practices of preventing complications were evaluated, and preliminary guidelines for a more rational approach toward prevention and treat ment based on a knowledge of pathophysiologic mechanisms were developed. It is our hope that the anesthesiologist will profit from the discussion of regional anesthesia contained in this volume, many aspects of which have never before been presented in such detail.

Book excerpt: Fifty years ago surgeons often performed various operations in conduction anesthesia or local anesthesia in the hope of circumventing pulmonary complications. The prepa ration for the operation was the task of an assistant who had to know and carry out the diverse local anesthetic procedures and who was responsible for their effective ness. In this way a large number of physicians learned to carry out nerve blocks, which they also applied more and more outside their operative duties. The introduction of modern general anesthesia led almost to the disappearance of the special techniques of nerve blocking at the former "classic" places of their teaching. Therefore it is a great merit of my former associate F. L. Jenkner to recall the art of nerve blocking, be it for diagnostic or therapeutic purposes. He presents the many possibilities as an addition to the therapeutic armamentarium and explains their indi cations, bases, and techniques. I do not think that one needs to be a specialist to carry out these useful techniques; but one has to have an understanding of certain topographic-anatomic situations, and one needs to know the rules of the procedure to obviate the risks that are always inherent in any disruption of the integrity of the integument of the human body. Thus by presenting all the mentioned aspects a renaissance of good though old practices may be initiated in the light of new achievements.

Their development over the past century ranges from a documented influence on Freud's Interpretation of Dreams 1 to the synthesis of the ubiquitously popular lidocaine, as described in Chapter 1. For surgical procedures the use of regional, epidural and intrathecal local anesthesia has increased continuously during the past decade. Local anesthetics are also applied by physicians to ameliorate unpleasant sensations and reactions to other procedures, such as tracheal intubation. The presence or the threat of cardiac arrhythmias is often countered by chronic administration oflocal anesthetic-like agents, such as lidocaine or procainamide. Relief of acute pain, accompanying dental manipulations, for example, and of chronic pain are also accomplished with traditional local anesthetics. And over-the-counter formula tions of topical local anesthetics provide practitioners of solar indiscretion welcome relief from their otherwise unaccommodating sunburn. In all these applications the final effect of the local anesthetic is an inhibition of electrical activity, accomplished as a reduction or total blockade of action potentials. The primary site of action is the sodium channel, a transmembrane protein which is essential for the influx of sodium ions that subserves impulse generation and propagation in nerves, skeletal muscle, and heart. The detailed mechanisms oflocal anesthetic action are still being investigated and Chapter 2 of this volume provides a current overview of that subject.
