Book excerpt: "It is 2014 and approximately 40% of the world population still has no access to adequate sanitary toilets. For these 2.6 billion people the problem is not only finding a safe and dignified place to defecate, but also trying to combat deadly diseases associated with the exposure to pathogens in feces left on the ground or near waterways. Improving sanitation is not only favorable to health, but also promotes dignity, economic benefits and environmental conservation. Although there have been numerous efforts to improve sanitation systems in the developing world, adoption rates and long term use are relatively low due to poor understanding of the multiple requirements for sustaining such systems such as environmental conditions and cultural habits. Quantifying and comparing the costs and benefits of these systems to the environment is one step in better informing decision makers in large-scale development projects, and thus facilitating the selection of sustainable sanitation systems. The research conducted puts forth a method to assess and hierarchically classify large-scale systems based on their environmental performance and context. The proposed method provides structured steps of environmental assessment and multiple-criteria decision analysis to compare and contextually evaluate the environmental implications of large-scale systems. A case study on specific sanitation systems in Cap-Haïtien, Haiti was reviewed to demonstrate and evaluate the framework. The study compared the use of urine-diversion toilets coupled with a collection system that diverts waste to a compost facility versus flush toilets connected to sewer systems with either endpoint to waste stabilization ponds or discharged into the environment without treatment. Overall, the results from this study show that the alternative involving diverting waste to a compost facility was preferred to the other alternatives for large-scale sanitation systems for Cap-Haïtien, Haiti; although there are specific conditions where it might not be. Various scenarios and analysis were developed to help provide some perspective into the results and conclusions of the methodology and the modeled case study."--Abstract.

Sub-Saharan Africa has low, but improving, rates of access to these services, but the infrastructure in use are often rudimentary and provide only marginal control of contaminants. The challenge of achieving universal water and sanitation service delivery in sub-Saharan Africa was investigated from a public service system perspective. A framework for defining and measuring the support systems needed to maintain infrastructure was developed and applied in six countries with the aim to foster greater alignment of implementing actors and improve practice.In the first phase of the research, interviews and field visits to promising and failed sanitation interventions in East Africa revealed the importance of both local participation and government buy-in to achieve sustainability and to be able to scale up innovations. Fragmentation of knowledge and an incomplete understanding of the problem by many actors suggested the need for a more shared understanding of how services can be provided and improved. In the second phase of the research, a conceptual and analytical framework was developed based on a review of the literature, case studies in east and west Africa, and expert consultation. The framework defined the water and sanitation system according to nine sub-systems: institutions, policy and legislation, finance, planning, regulation, monitoring, water resource management, learning and adaption, and infrastructure. These were determined to be a logical way to divide the overall system into manageable parts, based on iterative problem analyses with local and national actors. It is posited that these functionalities must be present in the national framework and understood and implemented at decentralised levels.In the third phase, the framework was applied using mixed methods participatory action-research to assess drinking water service delivery in Uganda using the case of Kabarole District. The research was carried out with a nationally registered NGO (IRC) and a learning alliance of stakeholders working in support of the Government of Uganda's objective to provide universal access to basic water by 2030, and to extend piped water to all households by 2040. Qualitative policy assessments, a review of statements from national level officials and the results from a quantitative analysis of the drinking water services in Kabarole in 2017 and 2019 were used to identify emerging trends in service delivery. Service delivery models present in Kabarole were self-supply, community management, and two different variants of a public utility model. Uganda has undergone policy reforms in recent years and is in the early stages of a transition to professionalized utility water supply systems. The research investigated the social learning of stakeholders in the learning alliance. The multi-level perspective for socio-technical systems transitions was used to analyse bottom-up and top-down innovation in Uganda. A participatory scenario development process made use of GIS maps, interviews, workshops and focus group meetings to identify the most important and uncertain factors that may influence progress toward the 2030 targets. The most likely scenario was identified, and several strategies were developed to adapt to anticipated changes while pursuing the goal of universal services. The final phase of research was a critical reflection on the WASH systems framework and assessment methodology. A public systems approach to analysis and planning was found to be useful for structuring collaborative work in a complex environment. The use of nine subsystems to define the larger system helped to reduce the complexity and aid in analysis. Participatory assessment of these subsystems was effective in helping local, national, and international actors to establish a more common understanding of the problem and to develop future visions for public service delivery"--

As part of the framework, this thesis is intended to help decision makers choosing the appropriate technology for developing community. A case study in Cimahi, Indonesia is presented as the framework s application.

It was a unique event as it enabled a truly multi-disciplinary approach in discussing Sanitation Challenges in North and South with social and political scientists, natural scientists, environmental engineers and practitioners in one s- entific conference. Book excerpt: As the Millennium Development Goals (MDGs) come to a close, 783 million people still lack access to improved water (Onda et al., 2012). An estimated 30 - 40% of development efforts to improve access to water still fail (are broken and/or unused) (Lockwood et al., 2010), which is similar to rates found 40 years ago (Imboden, 1977). While there have been increasing efforts to address this issue - especially the financial and technical aspects, a review of recent sustainability frameworks in the Water, Sanitation, and Hygiene (WASH) sector reveal that there is a need for better understanding how to plan for and address the social aspects of sustainability in delivering rural water services. The purpose is to explore different strategies used by leaders in the WASH sector to deliver socially sustainable safe drinking water in low income countries. The goal is to provide insights based on specific examples and participant experiences on ways to increase the social sustainability of services: a) in the bigger picture of international aid effectiveness, and b) with a specific focus on creating effective motivation infrastructure. Twenty-two key informants were purposively selected and interviewed for an average of one hour and fifteen minutes, during which they were asked to describe in detail their organization's approach to sustainability along with a specific example of an effort that failed and one that succeeded. Interviews were recorded, transcribed, and analyzed using qualitative analysis and ©NVivo10 software. Results: Eight main themes emerged which were consistent across what participants believed worked and did not work and found worked and did not work. Socially sustainable efforts create effective incentives, build local capacity, respect local stakeholders priorities and preferences, learn before, after and throughout the process, prevent failure and protect investment, ensure on-going monitoring and maintenance, identify and avoid assumptions, and focus on what people want rather than need. Given the variability of local context and culture, it is not helpful and may not be possible to create a fixed list of actions that accomplish the themes listed above. However, learning from themes that emerged provides valuable insights into how we might more consistently include mechanisms that improve sustainability by recognizing the importance of social versus technical challenges. Examining how these themes align with the bigger picture of development, we find that 85% of the themes fit into one of the five categories of commitments made in the Paris Declaration on Aid Effectiveness (2005); but a lack of consistent, wide-scale monitoring in the WASH sector makes it difficult to tell to what extent each of these international commitments have been met. Looking at how these themes align with a specific focus - motivation as described in Maslow's Theory of Motivation (1943), we find that strategies participants believe to be effective align more with higher level needs such as self-actualization (n=105), self-esteem (n=88), and safety and security (n=110) compared to physiological (n=8). More rigorous, collaborative research is needed to better document the definitions, combinations, and contexts in which strategies work and do not work. There is no magic bullet that will guarantee sustainability 100% of the time, but a more accurate understanding of social challenges and barriers like human motivation provide us with better tools from which to make evidenced based decisions in addressing a missing area of sustainability.

This report examines how Ethiopia s Health Extension Program (HEP) has contributed to the country s move toward Univeral Health Coverage (UHC), and to shed light on how other countries may learn from Ethiopia s experiences of HEP when designing their own path to UHC. HEP is one of the government s UHC strategies introduced in a context of limited resources and low coverage of essential health services. The key aspects of the program include the capacity building and mobilization of more than 30, 000 Health Extension Workers (HEWs) targeting more than 12 million model families, and the mobilization of health development army ? to support the community-based health system. Using the HEP-UHC conceptual model and data from Demographic and Health Surveys, the study examines how the HEP has contributed to the country s move toward UHC. During the period that the HEP has been implemented, the country has experienced significant improvements in many dimensions: in terms of socioeconomic, psychological, behavioral, and biological dimensions of the beneficiaries; and in terms of the coverage of health care services. The study finds an accelerated rate of improvements among the rural, less-educated, and the poor population, which is leading to an overall reduction in equity gaps and improvements in the equity indicators including the concentration indices - that suggest a more equitable distribution of resources and health outcomes. The HEP in Ethiopia has demonstrated that an institutionalized community approach is effective in helping a country make progress toward UHC. The elements of success in the HEP include the emphasis on community mobilization which identifies community priorities, engages and empowers community members, and supports their ability to solve local problems. The other aspect of HEP is the emphasis on institutionalization of the activities, which addresses the sustainability of community programs through high level of political commitment, and effective coordination of national policies and leveraging of support from partners. These findings may offer useful lessons for other low income countries facing similar challenges in developing and implementing a sustainable UHC strategy.

While in smaller basins the focus of water resources management may be on project implementation, irrigation and drainage management, water use efficiency and flood operations; in larger basins, because of the greater complexity and competing interests, there is often a greater need for long-term strategic river basin planning across sectors and jurisdictions, and considering social, environmental, and economic outcomes. This puts a focus on sustainable development, including consumptive water use and non-consumptive water uses, such as inland navigation and hydropower. It also requires the consideration of hard or technical issues—data, modeling, infrastructure—as well as soft issues of governance, including legal frameworks, policies, institutions, and political economy.
