Building upon two prior studies from the Institute of Medicine (the 1991 Institute of Medicine's report Disability in America and the 1997 report Enabling America), The Future of Disability in America examines both progress and concerns about continuing barriers that limit the independence, productivity, and participation in community life of people with disabilities. This book offers a comprehensive look at a wide range of issues, including the prevalence of disability across the lifespan; disability trends the role of assistive technology; barriers posed by health care and other facilities with inaccessible buildings, equipment, and information formats; the needs of young people moving from pediatric to adult health care and of adults experiencing premature aging and secondary health problems; selected issues in health care financing (e.g., risk adjusting payments to health plans, coverage of assistive technology); and the organizing and financing of disability-related research. The Future of Disability in America is an assessment of both principles and scientific evidence for disability policies and services. This book's recommendations propose steps to eliminate barriers and strengthen the evidence base for future public and private actions to reduce the impact of disability on individuals, families, and society.

One of the main issues in transit services is accessibility, particularly for people with disabilities. This research is intended to compare estimated job accessibility measures by transit in Kalamazoo county for workers with reduced physical mobility and without. Using OpenTripPlanner (OTP) software, General Transit Feed Specification (GTFS), Open Street Map (OSM) and Longitudinal Employer-Housing Dynamics (LODES) data were analyzed to estimate job accessibility for people with mobility constraints. By using the shortest tree path method, OpenTripPlanner (OTP) calculates a series of origin-destination matrices with multi-modal travel times and distances. The developed Python scripts estimate AM peak accessibility measures at five-minute intervals for four time thresholds (30, 60, 90 and 120 minutes). This research addresses the two main research questions. First, have we provided sufficient access to jobs through public transit for people with disabilities for their mobility independence? Second, how do we select bus stops to add wheelchair accessibility in the context of better job accessibility for people with disabilities?

Book excerpt: This two-part study employs fine-scale performance measures and analytical techniques designed to evaluate and improve transit services for people experiencing disability. Part one puts forth a series of time-sensitive, general transit feed system (GTFS)-enhanced employment accessibility models that account for multiple transportation modes, categories of functional limitation and design characteristics of existing public transit infrastructure. Model results shed light on the degree to which a medium-size city’s public transit system addresses the gap between a theoretical continuum of rider capacities and the physical demands required to achieve mobility and access to employment. Our research finds that an individual’s combined physical mobility constraints (e.g., walking speed and maximum walking distance) and public transit infrastructure requirements (e.g., presence/absence of wheelchair boarding platforms and connections to pedestrian access routes) may reduce employment accessibility outcomes by as much as 86 percent. Part two of the study utilizes performance measures developed in part one to model--via spatially explicit structural equations--the degree to which employment accessibility explains variations in public transit ridership and work commute transportation mode share. Here we find that commute share and ridership...(results). Developing a better understanding of relationships between accessibility and transit usage, we reason, will help shed light on how American Disabilities Act (ADA) compliant transit infrastructure affects mode choice decisions among people with considerable functional limitations and across the broader population.

Book excerpt: This case study of transportation policy for disabled people illustrates the flaws in policymaking that lead many Americans to believe government is not working as it should. Robert A. Katzmann examines the workings of the legislative, administrative, and judicial processes, both separately and in interaction, as he relates the erratic path of transportation policy for the disabled over two decades. An estimated 13.4 million people in this country have difficulty using public transportation, but the federal response to their problems of mobility is of fairly recent vintage, beginning with legislation in the early 1970s. Since then, there have been many twists and turns in policy, involving a wide array of governmental institutions. These constant shifts have confused state and local governments, the transit industry, and the disabled community. Assessing why policy was so erratic, Katzmann concludes that in part the confusion resulted from the inability to choose between conflicting approaches to the problem--one oriented toward the rights of equal access for the disabled, and the other favoring effective mobility by any practical means. In addition, the conflict between these two policy approaches was compounded by increasing fragmentation within and among national institutions.

Book excerpt: This qualitative study explores how a society recognizes an individual with a disability as a valued part of a community's economy, culture, and spirit; and the role that access to transportation plays within the community setting. The Americans with Disabilities Act (ADA) serves as a starting point and provides background for the study. This study consists of interviews with sixteen individuals, including caregivers as well as those with disabilities, on the experiences and choices that they must make when navigating the transportation system in the fast-growing communities of Fort Collins, Colorado, and Madison, Wisconsin, as well as in northern Illinois and Croatia. The results indicate that the difficulty in using, and the unpredictability of the transportation system have a pronounced effect on those with an ambulatory or visual disability. After all, transit often thought to be "accessible" is full of accommodation issues, lack of information, and unpredictability for someone with or who may develop a physical or visual limitation. By using open-ended questions, this study used a context-specific approach whereby participants would describe a situation and how that experience unfolded when using a specific mode of transportation. Participants were also asked about their opinions and experience with the built environment, universal design, and thoughts on emerging technologies such as autonomous vehicles. The results from this research question and hypotheses were coded and then broken into themes. From these themes separate chapters were created regarding transportation and discrimination, the changing needs of the aging Baby Boomer generation, and travel by air. The specific chapters are: Chapter 1: History of Transportation and the Disability Rights Movement. Chapter 2: How Unintentional Discrimination in the Transportation System May Cause Individuals with Disabilities to Become "Discouraged Workers" at Great Cost to Individuals, Families, and Communities. Chapter 3: Maintaining Transportation Accessibility for an Aging Baby Boomer Population. Chapter 4: Discriminatory Implications for People with an Ambulatory Disability Caused by the Systemic Design of the Airline Industry.

There can be barriers in the physical environment and delivery of services that render transport inaccessible. The principle of the UN Convention on the Rights of Persons with Disabilities (CPRD) brings new momentum to ensuring accessibility in the delivery of transport infrastructure and services. The CRPD recognizes that obstacles and barriers to indoor and outdoor public facilities and buildings and the physical environment should be removed to ensure equal access by people with disabilities and all members of society. This note summarizes the analysis done of the accessibility features of recent transport projects in the East Asia and Pacific (EAP) region. It seeks to highlight good practice in national laws, policies and project implementation to improve the welfare of transport users across projects. The overarching objective is to suggest how to improve the implementation of accessibility features in transport projects for people with disabilities and people with limited mobility. Mobility and access requirements of people with disabilities should be considered by planning and designing barrier- free transport systems. This implies an understanding and identification of the circumstances that create barriers for people with disabilities. Many countries have made progress in reducing barriers in the transport environment, particularly in high income countries. Countries have implemented regulation and design guidelines which explicitly consider accessibility for people with disabilities.

Book excerpt: With the passage of the American with Disabilities Act (ADA) it has become a civil rights violation to deny access to persons with disabilities to public transportation. The ADA requires transit agencies to provide accessible buses or equivalent services to persons with mobility, sensory or cognitive impairments. This study examines issues concerning persons with sensory and cognitive impairments, and their access to fixed route transit services. The goal of this report is: to specify operating guidelines, suggest technological changes, offer operating policy and training programs modifications that solve problems of transit access for persons with sensory and cognitive impairments. To determine where the access problems exist in fixed route transit systems, an examination was undertaken of existing technologies and programs that are in use by transit agencies in North America. Upon review of the available literature it was noted that there were available technologies and policies for persons with visual and hearing impairments, however, little appeared to be available for persons with cognitive impairments. To fill in this knowledge gap, surveys and interviews were conducted with persons with cognitive and other impairments, persons who trained persons with sensory and cognitive impairments, and persons who provide transportation services. A major conclusion of this study is that for persons with cognitive impairments, technological solutions are not the answer to increase bus accessibility. Personal interaction is needed to solve each individual circumstance with the transit user. Training for transit personnel is needed so personnel become aware and more sensitive to the needs of all transit users. Training for the transit user is necessary so use of the transit system is accomplished with grace, speed, efficiency and dignity. Training for the trainers of persons with disabilities is necessary so transit travelers will be informed of all the available services the transit agencies are offering. Another conclusion is visual signage must be standardized to be effective. This includes location, lighting, contrast, and content.
