An additional, crucial question is raised when the author asks if the therapist can allow the patient to terminate treatment. Has the therapist achieved separation from their own parents—or, indeed, from their analyst? Exploring the plight of patients of the unseparated analyst, Usher describes how these generational factors rear their unfortunate heads when it is time to end therapy. Listening to patients from the perspective of separation-individuation is not new; what is new is Usher’s emphasis on how these particular issues are often masked by significant achievement in adult professional life. How revolutionary this current research might be, the quest for a localisable ethical centre or moral organ is far from new. The moral brain was a recurrent theme in the works of neuroscientists during the 19th and 20th century. From the phrenology era to the encephalitis pandemic in the 1920s a wide range of European and American scientists (neurologists, psychiatrists, anthropologists and criminologists) speculated about and discussed the location of a moral sense in the human cortex. Pushing aside the primacy of the tension-discharge-gratification model of mental dynamics, this work introduced the notion of a "daemonic force" within all human beings that slowly but insistently seeks psychic inactivity, inertia, and death. Politely dismissed by some as a pseudo-biological speculation and rapturously espoused by others as a bold conceptual advance, "death instinct" became a stepping stone to the latter conceptualizations of mind's attacks on itself, negative narcissism, addiction to near-death, and the utter destruction of meaning in some clinical situations. The concept also served as a bridge between the quintessentially Western psychoanalysis and the Eastern perspectives on life and death. These diverse and rich connotations of the proposal are elucidated in On Freud's "Beyond the Pleasure Principle". Other consequences of Freud's 1920 paper - namely, the marginalization of ego instincts and the "upgrading" of aggression in the scheme of things - are also addressed.

It accounts for indigenous experiences of the lunatic asylum in the Bombay Presidency (1793-1921). The book argues that the colonial lunatic asylum failed to assimilate into Indian society and therefore remained a failed colonial-medical enterprise. It begins by assessing the implications of lunatic asylums on indigenous knowledge and healing traditions. It then examines the lunatic asylum as a ‘middle-ground’, and the European superintendents’ ‘common-sense’ treatment of Indian insanity. Furthermore, it analyses the soundscapes of Bombay’s asylums, and the extent to which public perceptions influenced their use. Lunatic asylums left a legacy of historical trauma for the indigenous community because of their coercive and custodial character. This book aims to disrupt that legacy of trauma and to enable new narratives in mental health treatment in India.
