O'ROURKE One of the principal problems of hypertension is the precise definition of blood pressure as a cardiovascular risk factor. Clinicians indicate peak systolic pressure and end diastolic pressure in the brachial artery as the principal criteria for blood pressure measurement. Consequently, these values are as indicators for clinical management and therapeutic adjustment. This used methodology, based on indirect blood pressure measurements at the site of the brachial artery relates only to the highest and lowest pressure in that vessel, and does not give any information of the blood pressure curve itself; this carries more information than peak systolic pressure and end diastolic pressure. As a first step in better analysis of the blood pressure curve, research workers in experimental hypertension defined in addition to peak systolic pressure and end diastolic, another blood pressure value, mean arterial pressure, i. e. the average pressure throughout the cardiac cycle, and about which pressure fluctuates. This is the pressure recorded by Hales [1] and by Poiseuille [2] in their pioneering studies. By application of Poiseuille's Law, this definition of mean arterial pressure led to the concept that increased mean arterial pressure (and therefore hypertension) was related, at any given value of cardiac output, to an increase in vascular resistance, i. e. to a reduc tion in the caliber of the small arteries.

With the improvement of diagnosis through the development of image techniques, didn't the contrast media already have their future behind them? Many answers were provided during the course of the workshop, and in order to best answer the journalists I proposed the following synopsis. 1. Since the 1979 Colorado Springs workshop organized by E. Lasser, progress has been so rapid and the newly available works so numerous that another meeting on an international level for the purpose of pre senting and discussing these advances appeared indispensable. Why not then in Europe and why not in Lyon? To expand on this progress, by 1981 the new contrast media with less-hyperosmolar molecules, still in the trial stage in 1979, were al most all available commercially for angiography, albeit at prohibitive prices. Pulmonary heart disease is by definition a condition when the lungs cause the heart to fail. The left ventricle in combination with the other structures in the “left heart” pumps blood throughout the body. The right ventricle (and structures of the “right heart”) pumps blood to the lungs where it is oxygenated and returned to the left heart for distribution. In normal circumstances, the right heart pumps blood into the lungs without any resistance. The lungs usually have minimal pressure and the right heart easily pumps blood through. However when there is lung disease present, like emphysema, chronic obstructive lung disease (COPD) or pulmonary hypertension- the small blood vessels become very stiff and rigid. The right ventricle is no longer able to push blood into the lungs and eventually fails. This is known as pulmonary heart disease. Pulmonary heart disease is also known as right heart failure or cor pulmonale. The chief cause of right heart failure is the increase in blood pressure in the lungs (pulmonary artery).

The first edition was favourably received, and I have frequently been asked to write a new edition. My own knowledge in the years that have passed since then, and my clinical and research experience in the field of the subject have both increased. For all these reasons, I decided that the second edition was overdue. The long time that has elapsed between editions necessitated rewriting, rather than simply revising, most of the text. For the same reason, many of the figures are new. Most of the references I have cited appeared in the literature after 1981, but I have retained a number of earlier ones, either because they are classics or, in my opinion, have not yet been bettered. It is my hope that the new edition will be a useful resource for clinicians called upon to manage patients with pericardial disease and for physiologists when the pericardium is relevant to their investigations. I make no apology for the in-depth treatment of the pericardial physiology and pathophysiology throughout the book, for they are the foundation on which diagnosis, hemodynamic and imaging studies, and management must rest.
