Book excerpt: Le présent guide vise à fournir aux inspecteurs du travail les compétences nécessaires pour mener des enquêtes efficaces sur les accidents du travail, les maladies professionnelles et autres événements inattendus (tels les événements dangereux et les quasi-accidents/incidents), qui auraient pu causer des lésions corporelles aux travailleurs ou à la population. Il ne faut pas perdre de vue que les accidents du travail, les maladies professionnelles et autres événements inattendus sont évitables. Une enquête efficace permettra d'identifier non seulement les facteurs de causalité, mais aussi les mesures qui auraient pu empêcher l'événement de se produire. Si l'OIT sait qu'il existe d'autres méthodologies pour conduire des enquêtes, elle estime que la méthodologie présentée ici aidera les inspecteurs à identifier toutes les causes immédiates et profondes de l'événement faisant l'objet de l'enquête. En retour, cela permettra aux inspecteurs d'aider les employeurs, les entreprises et les représentants des travailleurs à identifier les mesures de prévention/de maîtrise des risques appropriées, de façon à éviter que les actes ayant conduit à l'événement faisant l'objet d'une enquête se reproduisent et, partant, à améliorer la gestion de la sécurité et de la santé au travail. Ce guide a été élaboré en collaboration avec le Centre international de formation de l'OIT à Turin, suite à un atelier sur la réalisation d'enquêtes sur les accidents du travail et les maladies professionnelles, auquel ont participé des mandants du Brésil, des Etats-Unis, de l'Italie, de la Norvège, du Portugal, de la Roumanie et du Royaume-Uni, ainsi que des spécialistes du Service de l'administration du travail, de l'inspection du travail et de la sécurité et la santé au travail du BIT.

Although the quality of organization and management within the work environment plays an important role in these accidents, the significance of individual human action (as a direct cause and as a mitigating factor) is undeniable. This has created a need for new, integrated approaches to accident analysis and risk assessment. This book detailing the use of CREAM is, therefore, both timely and useful. It presents an error taxonomy which integrates individual, technological and organizational factors based on cognitive engineering principles. In addition to the necessary theoretical foundation, it provides a step-by-step description of how the taxonomy can be applied to analyse as well as predict performance using a context-dependent cognitive model. CREAM can be used as a second-generation human reliability analysis (HRA) approach in probabilistic safety assessment (PSA), as a stand-alone method for accident analysis and as part of a larger design method for interactive systems. In particular, the use of CREAM will enable system designers and risk analysts to: • identify tasks that require human cognition and therefore depend on cognitive reliability • determine the conditions where cognitive reliability and ensuing risk may be reduced • provide an appraisal of the consequences of human performance on system safety which can be used in PSA.
