Book excerpt: Introduction : La saturation des services d'urgences entraine des difficultés de fonctionnement. Le patient n'ayant pas reçu de formation médicale est considéré comme n'étant pas à même de juger de la gravité de son état de santé. De nombreuses études ont été réalisées pour analyser les motifs de consultations dans les services d'urgences, mais la majorité ont été réalisées auprès ou par des professionnels de santé. L'objectif de cette étude est de proposer un questionnaire ouvert aux patients se présentant dans un service d'urgences afin d'analyser leur décision de venir consulter ce service, ainsi que voir leurs représentations des notions d'urgence et de gravité. Matériel et Méthode : Il s'agit d'une étude qualitative monocentrique, basée sur quarante entretiens individuels de patients se présentant aux urgences de l'hôpital de Menton pendant les heures ouvrables des cabinets de médecine générale libérale. Résultats : On recense dans cette étude un certain nombre de motifs de consultation non listés par les études précédentes tels que le manque de confiance envers les autres médecins libéraux, le besoin d'un second avis médical, le constat d'accident du travail, la commodité pour le personnel hospitalier, la praticité pour une personne étrangère ne parlant pas français, le besoin d'un avis médical sans vouloir déranger son médecin traitant. On note dans les représentations de l'urgence et de la gravité des notions supplémentaires telles que la peur du manque de soins et de se laisser dépasser par une situation mal contrôlée, la mauvaise automédication (insuffisante ou délétère), la détresse psychologique, l'urgence sociale, et la reconnaissance par le patient de symptômes connus pour être graves. Discussion : Il ressort de cette étude que le patient se sent souvent inquiet et démuni face à une situation inhabituelle, ressentie comme potentiellement grave. Il ne se sent pas les moyens de la gérer lui-même et a souvent recours aux urgences pour se rassurer, s'informer sur sa situation. Le manque de formation et de sources fiables d'information font qu'il va se tourner vers le médecin le plus proche. Il ressort que souvent ce médecin sera le médecin des urgences, par commodité géographique ou par soucis de rapidité. Dans les pistes de réflexion pour améliorer cette situation, certaines ont déjà été mise en place, telles qu'un médecin d'accueil et d'orientation ou une maison médicale, d'autres pourraient être envisagées telles qu'une éducation à la santé, une formation obligatoire aux gestes de premier secours et une meilleure information du grand public sur le rôle du service des urgences afin de limiter les consultations ressenties comme abusives.
