Book excerpt: L'implantation cochléaire précoce, en s'appuyant sur une courte durée de déprivation auditive et une plasticité cérébrale optimale, s'est révélée fondamentale dans les performances langagières atteintes par les enfants sourds. L'ambition de notre étude a donc été d'observer si le bénéfice langagier était réel pour ces enfants, s'il leur permettait d'obtenir un langage comparable à celui des enfants normo-entendants et/ou s'il subsistait des atypies dans leurs productions langagières. Pour cela, nous comparons les performances lexicales et syntaxiques d'enfants implantés précocement (avant 2 ans), d'enfants implantés tardivement (entre 2 et 5 ans) et d'enfants entendants du même niveau scolaire. Nous nous sommes également intéressées aux facteurs susceptibles d'influer sur les capacités langagières, tels que l'âge d'implantation, la durée d'implantation, les capacités perceptives et le niveau de stimulation familiale. Nous avons ainsi pu mettre en évidence que les enfants implantés avant l'âge de 2 ans développent un langage comparable à celui des enfants normo-entendants tandis que les enfants implantés plus tardivement présentent un retard de langage, malgré une compensation multifactorielle effective. De plus, nous avons démontré que les deux groupes d'enfants sourds présentent des scores chutés en production d'énoncés, révélant un traitement du langage spécifique.

Ainsi, l'apprentissage de la lecture et de l'écriture se fait en relation avec un ensemble de développements complexes construits progressivement depuis la naissance. Chez l'enfant sourd, ces développements, notamment le développement syntaxique et la structuration/compréhension du récit, sont souvent peu avancés au moment de l'entrée dans l'écrit. Il apparaît donc pertinent d'examiner ces développements et de les mettre en lien avec le cheminement en lecture et en écriture. L'étude présentée dans ce mémoire, réalisée auprès de quatre élèves sourds de 1re année du primaire évoluant dans un contexte scolaire oraliste, visait ainsi à mettre en relation leur niveau de développement syntaxique, de structuration/compréhension du récit à l'oral et à l'écrit et de conceptualisation du système d'écriture et avec leur cheminement en lecture et en écriture au cours de l'année scolaire. Les résultats obtenus font ressortir le défi important des enfants sourds au regard de l'apprentissage de l'écriture et de la lecture et laisse entrevoir l'importance des développements qui soutiennent l'entrée dans l'écrit pour le cheminement en première année du primaire. Les quatre élèves de l'étude ont tous cheminé en écriture, et ce, dans les différentes composantes d'une production écrite. Cependant, le passage vers la lecture autonome s'est avéré plus ardu d'où l'importance de poursuivre le travail intensif sur les difficultés langagières entravant cet apprentissage.

Cependant, peu d'informations précises existent sur les bénéfices qu'ils peuvent espérer à la suite de celle-ci. Or, afin de prendre une décision la plus sereine possible au moment de l'implantation cochléaire, les parents comme les équipes médicales doivent connaître les bénéfices envisagés, les difficultés possibles et les risques encourus. Pour ce faire, cette étude avait pour objectif de créer un questionnaire permettant d'évaluer les bénéfices de l'implantation cochléaire chez les enfants sourds polyhandicapés. Il a ensuite été transmis à sept familles ayant un enfant sourd polyhandicapé implanté cochléaire. Par la suite, l'analyse des réponses a mis en évidence les avantages perçus par ces familles à savoir le développement de nombreux précurseurs à la communication, de meilleures performances en reconnaissance auditive et en compréhension contextuelle, davantage d'interactions avec leur environnement et une amélioration de la qualité de vie avec une plus grande autonomie. Cependant, les avis divergent entre les familles interrogées et une grande variabilité existe quant aux bénéfices perçus de l'implantation cochléaire chez ces enfants. De plus, les avantages obtenus sont bien différents de ceux envisagés pour les enfants sourds sans déficiences associées qui ciblent principalement le développement du langage oral. Finalement, ce questionnaire créé pour cette étude pourra être utilisé dans la pratique clinique en vue d'améliorer l'évaluation et la prise en soin des enfants sourds polyhandicapés bénéficiant d'un implant cochléaire.
