Additionally, college students with ADHD diagnoses struggle to obtain effective treatment options for their ADHD symptoms. Specifically, pharmacological interventions are effective in mitigating ADHD symptoms; however, adverse effects of stimulant medications (i.e., increased/decreased appetite, headache) impact medication adherence in college students with ADHD. Neurofeedback is a non-invasive, drug-free intervention that uses the theories of biofeedback and cybernetics to increase self-regulation of brain functions.

There is increasing evidence for specific effects of neurofeedback when applying 'standard' protocols (slow cortical potentials, theta/beta, sensorimotor rhythm). Knowledge about underlying mechanisms and moderating variables is increasing. Nevertheless, further well-controlled and conducted trials are needed to answer open questions concerning optimisation and individualisation of neurofeedback. Further improvements may develop with new methods and technical developments (e.g., tomographic neurofeedback) and new concepts (integrated ADHD treatment). This Frontiers Research Topic comprising 14 articles intends to answer the following questions concerning neurofeedback in ADHD: • How efficacious is neurofeedback? • What is the rationale of applying a certain neurofeedback protocol in ADHD? • What are central mechanisms and which moderating variables may affect training and treatment outcome? • How to optimise treatment? Book excerpt: Though there is evidence to suggest that expectancies can impact outcomes of various medical and psychological treatments, little is known about the role of expectancies in neurofeedback for Attention-Deficit/Hyperactivity Disorder (ADHD). Clinical trials of neurofeedback have demonstrated mixed support for its efficacy in treating ADHD symptoms. In particular, placebo-controlled studies of neurofeedback have consistently failed to demonstrate the superiority of neurofeedback over a placebo when outcomes were measured by symptom ratings and neuropsychological tests. These findings suggest that expectancy effects may partly explain the positive outcomes found in clinical trials of neurofeedback. To examine the effect of expectancies on symptom self-report and neuropsychological test performance in neurofeedback, 73 young adults seeking treatment for ADHD were given sham neurofeedback and randomly assigned to receive positive, negative, or neutral false feedback to influence treatment expectancies.
