Book excerpt: Excerpt from An Investigation Into the Mortality Rates of the City of New York Each decennial census of Great Britain, from that of 1841 to that of 1891, has, with few exceptions, been followed by a thorough investigation into the mortality rates of the people at the time of the enumeration. Dr. Farr made the first three of these investigations, which are summed up in what are known to us as Farr's English Tables Nos. I, 2, and 3, and which set forth the mortality rates of the English people for the years 1841-1851. The English Table No. 4 was the result of the labors of Dr. Ogle, Farr's successor, and was based upon the census returns of 1871 and 1881, and the deaths for the ten years, 1871-1880. Dr. Tatham constructed the English Table No. 5 from the enumera tions of 1881 and 1891, and the deaths for the years 1881-1890. These tables show a constant decrease in the death rates and a corresponding increase in the Expectation of Life at birth, which rises from years and years, males and females respectively, as given in Farr's Table No. 3, to years and respectively, as found by Dr. Tatham. This is a gain of 3% years for males and 573 years for females. The last English Table shows an improvement over its predecessors in the Expectation of Life at every age from birth to 26; but at the older ages it develops a decrease in the span of life, due, it seems to me, not to any deterioration of the people, but to the continual out ow of young, strong lives from the mother country to the various colonies and to the States. It is only by such studies as these that the Executives in charge of the health of a community are enabled to keep their fingers upon the public pulse, and note the effects of every advance in medicine and surgery, and the effects of every improvement in sanitation. Including that of 1900, there have been made twelve enumerations of the people of the United States. Much data and valuable information have been gathered regarding the population and deaths, but we have thus far contented ourselves with mere guesses as to the possible trend of the death rates. Book excerpt: Excerpt from Report of the Medical Commission for the Investigation of Acute Respiratory Diseases of the Department of Health of the City of New York, Vol. 7: Part I, Studies on the Pneumococcus For some years the problem presented by the very large and constantly increasing death-rate from the acute respiratory diseases has been the cause of serious concern to the Board of Health of New York City, and in August, 1904, with the consent of the Mayor and the Board of Estimate, it appointed a Medical Commission to conduct an investigation as to the causes for the great prevalence of the acute respiratory diseases in the city, with the hope that some means could be devised for reducing the excessive morbidity and mortality from this cause. The problem has long been recognized as one of the first in sanitary importance, and efforts have frequently been made to find some method for its solution. The inherent difficulties of the problem, however, are so great and the other questions pressing for consideration have been so numerous and important that no determined efforts have hitherto been made to discover means or devise methods to effectually meet the situation. Succinctly stated, the situation is this: During the last twenty years the general death-rate in New York City has fallen 25% and the death-rates from all the principal causes of deaths have fallen from 10% to 35%, excepting in four groups of diseases, in which there has been an increase. These are, first, the acute respiratory diseases; second, cancer; third, the diseases of the heart and blood-vessels; fourth, the diseases of the kidney. The increases in these groups have been from 10% to 30% or 35%, - in the acute respiratory being from 10% to 15%. In the first half of 1904 there was an extraordinary increase in this group, the deaths from the acute respiratory diseases alone forming no less than 23% of the total death-rate. The importance of the situation, so far as this group is concerned, is more forcibly brought out when we compare the percentage which the total deaths from the acute respiratory diseases bear to the total deaths from all causes. The Mortality from Cancer in Different Occupations is discussed in Chapter IV, with an appendix of eight tables of the mortality from cancer in selected industries and employ ments, derived from the decennial reports of the registrar-general Of England and Wales, but rearranged and recalculated for the present purpose. In addition, the appendix includes cancer mortality data by occupations, derived from the Industrial mortality experience Of The Prudential and the cancer census of Hungary. Chapter V presents an extended discussion of Cancer as a Problem in Life Insurance Medicine, historically and practically considered, with an appendix of 121 tables, including a concise and uniform presentation Of the general cancer experience data of a large number of American and foreign life insurance companies and the collective results of the Medico Actuarial Mortality Investigation. Chapter VI, on The Geographical Incidence of Cancer Throughout the World, brings out forcibly the wide range in the cancer frequency rates of different countries and cities with widely varying circumstances of race, climate, habits, etc., all of which are shown to have an important bearing upon the cancer problem as a whole. Included in this chapter are the results of a special analysis Of the data collected by the New York State Institute for the Study of Malignant Disease concerning the primary seat of growth, probable cause, the personal and family history, etc. The information is made available for the first time through the courtesy of the Director Of the Institute, who placed the original material at our disposition for tabula tion and analysis. The principal tables of the appendix to this chapter show the facts Of cancer mortality according to latitude; size of cities and the local rates of incidence by organs and parts Of the body for thirteen representative countries throughout the world. In Chapter VII, on The Statistical Data Of Cancer Frequency in American States and Cities, the rate Of cancer occurrence throughout the United States is discussed at some length, and amplified by an appendix of 259 tables Of cancer mortality for the registration area and for the several states and cities in a uniform manner and with a due regard, as far as practicable. Book excerpt: Excerpt from Summer Mortality in the Cities of the United States: Paper Read Before the New York Academy of Medicine, June 17th, 1869 The idea advocated by one of the gentlemen who participated in the discussion had upon this paper, that the phenomena of sunstroke are to be accounted for by the sudden suspension of the process of elimination of effete and poisonous material by the skin, is a totally untenable one, for the following reasons - First, Cases of sunstroke are not always preceded by a drying up of the skin; and hence in such cases there is no even probable reason for supposing that elimination of poison from the blood has been sus pended, so far as the skin is concerned Second, The time between the dry ing of the skin and the occurrence of the grave symptoms of the disease is, in a great proportion of cases, too short to render it at all probable that ar rested elimination is the cause of those symptoms. Such rapidity has no analogue in the known suspension of the eliminating function of any organ of the body, if we except the lungs. Third, Physiological experiments upon the lower animals, and thermometric observations upon the human Vlctims of sunstroke, suffice to Show that the overheating of the blood and tissues of the body is the real cause of the phenomena of this affection. The argument, therefore, that to prevent sunstroke perspiration must be promoted by heavily clothing the person, even in tropical temperatures, has no pathological support, and is directly contradicted by the most extended observation, as well as by the teachings of natural history. If it were true, as this argument assumes, that the animal body is protected from solar and atmospheric heat by covering it, is it probable that the Creator would have made it a characteristic of tropical animals that they have little or no hairy covering? And in the temperate zone, would He have ordered that the thick and heavy fur and hair covering possessed by its animals in winter should fall off at the approach of summer, leaving the skin comparatively nearly naked during the heated months? We think no reflecting person can long entertain such views of the pathology of sunstroke, nor of the influ ence of heavy clothing in protecting from it. The example given, on the occasion of the discussion, of the heavy dress of the inhabitants of the steppes of Russia, has certainly very little weight in the argument for while these people have but comparatively few hot days in the year, they occur during a period in which the atmosphere is extremely dry, so that evapora tion of the perspiration and consequent cooling is rapid, even though the body he thickly covered.
