Book excerpt: Human papillomavirus (HPV) is the most common sexually transmitted infection, with 14 million new cases in the U.S. annually. Adolescents bear 49% of this burden, placing them at increased risk for HPV-related cancers that could be prevented with the HPV vaccine. Unfortunately, national and New Mexico (NM) immunization rates for HPV are well below the Healthy People 2020 target of 80% (U.S. Department of Health & Human Services [USDHHS], 2011). School-based vaccine programs are strong predictors for higher vaccination rates, however less than 50% of the NM school-based health centers (SBHCs) have the ability to provide vaccines. Operational barriers interfere with some SBHC's ability to store vaccines in accordance of the Centers for Disease Control and Prevention (CDC) guidelines. This situation prevents SBHCs from being able to access vaccines through the universal vaccine purchase programs (UVP) and the federally-funded Vaccines for Children (VFC) program at no cost to the SBHC clinic or patients. The Precede-Proceed model was utilized to develop a health promotion implementation and evaluation program at the organizational policy level to improve vaccine access for SBHCs. A collaborative partnership between the SBHC and the local Public Health Office (PHO) through a Memorandom of Agreement (MOA) is reflective of the health policy necessary for a formalized shared vaccine program. This partnership reduces barriers related to vaccine storage and provides access for adolescents to receive HPV vaccines at the SBHC at the time of their health care visit. An earlier report from the Institute of Medicine (IOM), Calling the Shots: Immunization Finance Policies and Practices, pointed to the instability of the fundamental infrastructure that supports immunization programs throughout the United States, including growing financial burdens and operational complexities in immunization services, shortcomings in public- and private-sector investments in vaccine purchases and immunization programs, and fluctuations in insurance plans in the public and private health care sectors that create uncertainties regarding coverage of vaccine purchase and service delivery arrangements. In October 2001, a group of about 50 health officials, public health experts, health care providers, health plan representatives and purchasers, state legislative officials, and community leaders met at the Texas Medical Association in Austin to explore the implications of the IOM findings and recommendations for Texas. The 1-day workshop was the second in a series of four meetings organized by IOM with support from the Centers for Disease Control and Prevention to foster informed discussions about challenges for immunization finance and future strategies for strengthening immunization activities and the public health infrastructure that supports those activities. This report of the Austin workshop summarizes the findings of the previous IOM report and reviews continuing challenges in immunization finance for the nation and for individual states, with a particular focus on Texas. The report also highlights strategies proposed by individual workshop participants that can be used to address those challenges. Several presenters and discussants emphasized that adequate funding is necessary for immunization programs but that financial resources alone are not sufficient to guarantee success. Similarly, they indicated that no single agency or group in the public or the private sector should expect, or should be expected, to solve immunization problems. Speakers from both public and private health agencies observed that collaboration, consultation, and partnership efforts across levels of government and between the public and private sectors are essential.

However, because this is a new responsibility for the nurses working in the clinic, no processes or procedures exist. For example, the clinic has no policies or procedures for storing, handling, administering vaccines, or preparing staff for this new responsibility. It is essential to provide the team access to these resources to provide safe patient care and avoid adverse events and high medical costs. This project aimed to create policies, procedures, and education to support this change. Three themes came to light during a literature review related to avoiding medication errors, the nurse's role in patient education and perceptions of vaccines, and following CDC recommendations to help decrease adverse events. The evidence concerning these themes found the need to create policies, procedures, and educational content regarding vaccines. Patients and staff will benefit from this project because providing education will help them fulfill their responsibilities, and having a process in place will ensure patient safety.
