Book excerpt: Educating College Students About Human Papillomavirus Background: Human Papillomavirus (HPV) infection is the most common sexually transmitted infection in the US affecting almost 60% of college women ages 20 to 24 years. At one west coast community college, two thirds of males and half of females were unaware of HPV vaccine recommendation through age 26 years. Objectives: This aim of this project is to determine if an evidence-based online educational video about HPV and HPV vaccine would increase knowledge of HPV and intent to receive the vaccine among community college students enrolled in urban and suburban community colleges in Southern California. Measured outcomes of interest included HPV knowledge and intent to receive HPV vaccine. Methods: A quasi-experimental, one-group pre-posttest design was used to determine the effect of the HPV educational video on a sample of 24 gender and ethnically diverse community college students ages 18 through 26 years. Knowledge changes were measured using an online Student HPV-Knowledge Assessment immediately before and after video education. Statistical analysis included descriptive statistics, paired t-test, and the McNemar chi-square test for paired dichotomous variables. Results: Study results revealed all participants were aware of HPV and a majority was aware of the vaccine. Findings confirmed increased knowledge of HPV, perception of risk, and increased intent to get HPV vaccine in participants who were previously aware of disease following the intervention. These results support the utility of an online educational intervention to increase knowledge of HPV and intent of HPV vaccine uptake. Conclusion: HPV infection is common. Awareness of vaccines as protection from disease is increased during the pandemic, providing opportunity for influence of increased HPV vaccine uptake. The impact of HPV vaccine education through an educational video may be an effective method to reach young college students to heighten awareness and increase knowledge of HPV leading to increased HPV vaccine uptake. Future studies may examine if widespread education on vaccination as disease prevention during pandemic conditions could have impact on other vaccine uptake, specifically HPV vaccine, or if perception of risk from HPV infection might be overshadowed by perception of risk for COVID-19 and thus affect vaccine interest.

In the United States (US), young adults, especially college students, are mostly affected. HPV vaccine was approved in the US since 2006. They are safe and effective, but coverage remains lower than other vaccines for young adults, implying missed vaccination chances, and a need to improve HPV vaccination promotion strategies. Comics are potentially effective for health education of diverse groups, because they are easily accessible, low-cost, engaging, and unobtrusive. They may help to improve knowledge and beliefs regarding HPV vaccine, increase its uptake, and decrease HPV sequelae.MethodsWe conducted a mixed methods study to examine predictors of intention to complete HPV vaccination among college students in northeast Ohio. We developed an educational HPV/HPV vaccine comic book for college students informed by evidence from current target population-based studies on HPV vaccination, pilot testing, and the Integrated Behavior Model (IBM). We hypothesized that the intervention would improve their HPV vaccine knowledge, beliefs, attitude, perceived norm, personal agency, and intention to complete HPV vaccination within 12 months. Using a quasi-experimental pretest/posttest survey design, we evaluated the effect of the intervention on a diverse population of 18 to 26-year-old male and female students recruited from a college in northeast Ohio in 2017 to review the comic book. ResultsA majority of participants, n=314, males (28%), females (72%) were satisfied with the comic and reported it is an acceptable way to get HPV vaccine information, easy to read, culturally relevant, with good quality information and graphics. Only 29% reported they have completed the recommended three doses of HPV vaccine. Multiple linear regression (n=157) showed age (ß ^=-0.11*); race (ß ^=0.81*); instrumental attitudes (ß ^=0.43*); injunctive norms (ß ^=0.20*); and descriptive norm (ß ^=0.55**) were significant predictors of intention to complete HPV vaccination in the next 12 months [R2=0.47, F (8) =16.12, p

Studies suggest that there is the need for educational intervention to increase HPV vaccine acceptability. At a private pediatric practice in northeastern Maryland, many parents were observed to opt out of the vaccine for their children possibly due to lack of knowledge. The aims of this project were to assess parents' knowledge and to evaluate the effect of a short educational intervention on improving parents' knowledge of HPV and HPV vaccine. Changes in total knowledge score before and after the intervention were measured using a content validated HPV knowledge questionnaire, while changes in the vaccine acceptance were measured by comparing aggregate data from the same month one year prior to the start of the project. Fifty parents were recruited for this evidence based practice change project, all participants completed both pre- and post-intervention questionnaires. Parents' knowledge about HPV vaccine increased after the intervention compared to the baseline results. The pre-intervention mean score was 15.44 (30.88%) and post-intervention mean score was 19.74 (38.88%) indicating a 4.30 (8.60%) increase, and an overall increase of 48% of HPV vaccine accepted. The results of data analysis suggested that increasing parental knowledge on HPV and HPV vaccine led to an increase in the vaccine acceptance. Book excerpt: "The human papillomavirus (HPV) vaccine is a research breakthrough to prevent HPV-related cancers and morbidity. Despite assured vaccine efficacy and safety, adolescent vaccine receipt remains suboptimal. At a Convenient Care clinic in Central New Jersey, several missed clinical opportunities to offer HPV vaccination to adolescent patients were observed due to poor provider vaccine recommendations. An evidence-based practice change project was implemented to improve vaccine recommendations practices among providers. The intervention involved an individualized educational program focusing on HPV vaccine as a method of cancer prevention. A validated tool, the HPV knowledge and attitudes survey (HKAS), was utilized to assess provider knowledge of HPV disease, HPV vaccination and vaccine recommendation intent. Nurse Practitioner providers (n=29) were recruited, and all participants completed a pretest, posttest and knowledge retention survey. Knowledge retention was measured four weeks after intervention with the same tool. Using comparative means analysis, intended project outcomes were determined to be met based on selected benchmarks. Findings revealed an 18% increase in provider HPV disease knowledge, a 19% increase in HPV vaccine knowledge and a 33% higher likelihood to recommend HPV vaccination post intervention. Retention scores at 4 weeks post-intervention demonstrated a 1.12% knowledge increase. Therefore, providers retained knowledge gained. The educational programs directed at increasing providers HPV knowledge and promoting vaccine recommendation practices was an effective approach to closing the missed clinical opportunities to recommend HPV vaccination. Findings from this project support that widespread provider HPV education is warranted as a step towards improving the low adolescent vaccination rates. Book excerpt: Human Papillomavirus (HPV) causes nearly all cervical cancers, 90% of anal cancers, and 60-70% of penile, vaginal, valvular, and oropharyngeal cancers (CDC, 2020). Hispanics have the highest cervical cancer incidence rate of any minority group and a mortality rate 26% higher than Whites (CDC, 2021; American Cancer Society, 2018). HPV vaccination completion rates in the Hispanic population (62.7% of adolescents aged 13 to 17) are much below Healthy People 2030's goal of 80% of 13- to 15-year-olds up to date with HPV vaccinations (Healthy People 2030, n.d.). There is a lack of culturally appropriate interventions to increase HPV vaccine uptake. Prior studies suggest that promotora led education is an effective strategy to increase health behavior activities. The purpose of this study is to determine the effect of a culturally appropriate educational intervention, promotora delivered education, on HPV and HPV vaccine knowledge and intention to vaccinate with the HPV vaccine in Hispanics. The dependent variables are HPV and HPV vaccine knowledge level and intention to vaccinate. This study is significant to advanced practice nursing because providing culturally appropriate education and interventions will help combat ethnic and racial health disparities to ultimately decrease the incidence and mortality rates of HPV related cancers in Hispanics.

Book excerpt: The human papillomavirus (HPV) infects approximately 14 million people annually. a crucial role of all health care providers is to reduce the risk of exposure through primary prevention. In order to lessen the risk of people contracting the virus, education regarding the virus must be passed from health professionals to patients - primary the first interaction via the nursing staff. This will in turn - provides the opportunity to increase vaccination rates, increase HPV knowledge, and confirm information about the virus. Using evidence-based practice to answer questions is a vital component for nursing interactions in a clinical setting. However, they must be educated foremost to achieve appropriate outcomes. Delivering fact-based information helps thwart parents and patients from believing false information. Having nursing staff participate in educating parents - as opposed to only the healthcare provider - helps improve the information that parents receive and helps parents make informed decisions for their adolescent. The Larrabee Model for Evidence-Based Practice Change outlines the approach to enhance the education process for the nursing staff at the selected pediatric clinic. EBS resources and the incorporation of nursing staff provide a multi-person approach and reduce previously failed approaches to provide accurate education. The evidence-based framework model selected helped identify the foundation of the project in order to produce best possible outcomes. The method used included a pre-test questionnaire, PowerPoint educational in-service, and a post-test questionnaire. Through a pre-test questionnaire, the nursing staff answered questions related to HPV, the vaccine, and statements related to the virus. A post-test questionnaire following the intervention was used to evaluate and determine if knowledge increased because of the educational intervention. The results of the questionnaire displayed growth, but through interaction via the inservice, this signified that the base knowledge of nursing staff is limited in relation to HPV and the HPV vaccine. Scores from pret-test questionnaires to post-test questionnaires increased by 57%. This project identified the lack of education the nursing staff has received about HPV and the vaccine, and how improving knowledge can affect long-term manangement of nursing staff to further educate their patient population. In conclusion, including the nursing staff and utilizing rapport built with parents will further influence parents and patients to make informed decisions can based on evidence-based information. Informed patients can accomplish several things: they can prevent parents them from potentially making misinformed judgements, can increase vaccination rates, and can overall increase cognizance of a virus that affects persons on a global level.

Book excerpt: Cervical cancer is a virally mediated disease with the majority of cases due to the human papillomavirus (HPV). HPV infections are the most prominent sexually transmitted diseases among college-aged women. Although HPV vaccines such as Gardasil afford women with a valuable method of cancer prevention, vaccination rates are often incomplete or inadequate. This study evaluated the effectiveness of an interactive, information-motivation-behavioral skills (IMB) based, multimodal program to promote Gardasil use among 62 undergraduate women enrolled at the University of Connecticut. Secondary aims targeted additional health behaviors, including risky sexual practices and Pap Screen utilization. Participants were randomly assigned to a single-session, active intervention group or an attention control group and were assessed pre-intervention, post-intervention, and at a 1-month follow-up. At baseline, a majority of participants demonstrated high levels of vaccine knowledge, low mastery of HPV/cervical cancer information, and ambivalence about pursuing vaccination. Following the intervention, the IMB group demonstrated increased levels of HPV/cervical cancer and Gardasil knowledge, higher levels of self-reported motivation and intention to get vaccinated, and more positive attitudes toward vaccination. In addition, women in the IMB group demonstrated greater intentions to engage in regular screening in accordance with screening guidelines. These results provide partial support for an IMB-based intervention in helping elucidate factors that impact young women’s decision to get vaccinated and engage in cancer prevention. Findings may help guide the development of a cost-effective, cancer preventive program that can be easily disseminated and implemented in university clinics and health centers.
