Most of the countries in the region are lagging behind in achieving the health-related United Nations Millennium Development Goals. The African regions dismal health situation has largely been blamed on weakness pertaining to such factors as health leadership and governance; service delivery; health workforce; medicines, vaccines, and health technologies; health information; and health system financing that have undermined the capacity of health systems of countries in the region to improve population health without wastage of resources. Institutionalising health system efficiency monitoring, as a basis for the design and implementation of appropriate policy interventions, has been proposed as an effective way of curbing wastage of health system inputs. Efficiency of Health System Units in Africa: A Data Envelopment Analysis is the first book of its kind on application of the data envelopment analysis technique to examine the efficiency of health system decision-making units in Africa. Book excerpt: Without consistent and accurate information on the quality of services, it is difficult for citizens or politicians (the principal) to assess how service providers (the agent) are performing and to take corrective action. The service delivery indicators (SDI) provide a set of metrics to benchmark the performance of schools and health clinics in Africa. The indicators can be used to track progress within and across countries over time, and aim to enhance active monitoring of service delivery to increase public accountability and good governance. Ultimately, the goal of this effort is to help policymakers, citizens, service providers, donors, and other stakeholders enhance the quality of services and improve development outcomes. This report presents the findings from the implementation of the first SDI survey in Kenya. The production of health services requires three dimensions of service delivery: (i) the availability of key inputs such as drugs, equipment and infrastructure; (ii) providers who are skilled; and (iii) providers who exert the necessary effort in applying knowledge and skills. The SDI surveys allow for the assessment of how these elements come together to produce quality health services in the same place at the same time. This paper is structured as follows: section one gives introduction. Section two outlines the analytical underpinnings of the indicators and how they are categorized. Section three presents the methodology of the Kenya SDI education and health surveys. The results are presented and analyzed in section four and section five. The report concludes with a summary of the overall findings and some implications for Kenya.

The new Constitution was the institutional response to longstanding grievances over the centralization of state powers and public sector resources, and regional disparities in service delivery and development outcomes. This radical restructuring of the Kenyan state has three main objectives: decentralizing political power, public sector functions, and public finances; ensuring a more equitable spatial distribution of resources between regions; and promoting more accountable, participatory, and responsive government at all levels. The first elections under the new Constitution were held in 2013. Alongside the national government, 47 new county governments were established. Each county government is made up of a County Executive, headed by an elected Governor and works under the oversight of an elected County Assembly. Seven years after the "devolution train" left the station, this report takes stock of how devolution has affected the delivery of devolved basic services to Kenyan citizens. Whereas devolution was driven by political reform, the ensuing institutions and systems were expected to deliver greater socioeconomic equity through devolved service delivery. The Making Devolution Work for Service Delivery (MDWSD) study is the first major assessment of Kenya’s devolution reform. The study was a jointly coordinated effort by the Government of Kenya and the World Bank. The study provides key messages with respect to what is working, what is not working, and what could work better to enhance service delivery based on the currently available data. It provides an independent assessment of service delivery performance in five sectors, namely health, education, agriculture, urban, and water services and includes an in-depth review of the main pillars of devolved service delivery, namely public financial management, intergovernmental finance, human resource management, politics and accountability.
