Sexual activity is an essential part of intimate relationships, though it tends to decline in frequency as people grow older. Nevertheless, for many older people, sex still remains an important part of their life. There’s a common misconception that as people age, they lose their interest in sex and capacity for sexual behaviour. This is not the case, as a number of national surveys show. The new research presented here indicates that the less sex older people have, the more likely they are to experience mental and physical health problems. Men and women who reported a decrease in the frequency of sexual activities were also more likely to report a deterioration in self-perceived health. Additionally, men with erectile dysfunction were also more likely to be diagnosed with cancer or coronary heart disease. This research has also determined that older adults who experience a decline in sexual activity report poorer well-being than those who maintain their levels of sexual desire, activity and function in later life, and that men who remain sexually active in later life continue to have better cognitive performance compared to those who don’t. Despite these health benefits, medical professionals do little to promote sexual activity in older adults, and the literature suggests that there is a lack of knowledge on how to approach the subject and how to promote such activities. This is the first comprehensive book specifically exploring all areas of sexual behaviour and health in older adults, and drawing on the latest research in this area. It offers a valuable resource for researchers and clinicians in various health fields (medicine, nursing, occupational therapy, psychology, public health, etc.), as well as students in undergraduate and graduate programs.

Book excerpt: Despite continuing ageist beliefs that sexuality is a privilege designed only for the young and physically healthy, research continues to indicate that the majority of older adults maintain interest in sexuality and may engage in fulfilling sexual behavior well into their last decade of life. Unfortunately, many professionals remain unaware of general knowledge of elderly sexuality, including the expected and normal physiological changes that can occur within the context of both male and female aging. The presence of chronic illness and other medical problems certainly can influence the expression of an aging adult’s sexuality, and emergent research suggests that there are effective ways to cope with menopause, heart disease, arthritis, incontinence, diabetes, sleep disorders, breast cancer, prostate cancer, and erectile dysfunction (ED), among others. Dramatic changes have taken place within the last decade alone in terms of non-surgical treatment for incontinence and ED, with forms of sex therapy, biofeedback, and PDE-5 inhibitors. Regrettably, many aging adults and their care providers remain unaware of their increased risk factors for STDs, including HIV infection via lack of knowledge, changes in the vaginal lining, and typical declines in immune function. Estimates suggest that by the year 2020, more than half of all individuals living with HIV will be over the age of 50. Although some high quality professional books are available for clinicians, they tend to be disjointed research bibliographies, edited volumes on a narrowly focused aspect of elderly sexuality, or texts that are more than 10 years old. With the extent of new information available regarding sexuality and aging, an up to date, empirically based text is necessary.

Book excerpt: Throughout my clinical training and practice, I have been surprised by the number of times that sexual issues have emerged as an unexpectedly central feature in my work with older adults. I can vividly remember my own internal reaction on hearing one of my elderly female patients tell me that she was date raped a few years after the death of her elderly husband-when she was 68 years old. I can see in my mind's eye the blood splattered on the floor of an inpatient unit from an elderly man who smashed his arm through a window, furious that his antidepressant medication made it impossible to climax through masturbation. On a much less dramatic but equally important note, I think about the elderly amputee who told me softly about his fears of resuming sexual activity with his wife of 25 years. I also think about the elderly woman whose inability to take herself shopping to find fashionable, comfortable clothes to fit over her hunched shoulders and large breasts helped precipitate a serious depression. In sum, I learned early on that elderly sexuality is not just about how many times a week someone makes love. It is my hope that sharing these experiences and introducing the related theories, research, and interventions will assist other clinicians in dealing with these often challenging and clinically demanding situations. Without my patients, this text would not have been possible.

Intimacy is a sense of togetherness and connection in a relationship, which may or may not include physical contact. Aging creates possibilities for older individuals to reinterpret what sexuality and intimacy entail for them as a result of life changes. Certain older individuals want both a sexual and an intimate partnership, while others are fine with either, and some may opt to reject both sorts of partnership. Normal aging also causes physiological changes that might interfere with the capacity for and enjoyment of sexual activity. Our bodies, such as our size, skin, and muscular condition, shift as we age. Certain older individuals are unhappy with their deteriorating bodies. They may be concerned that their lover no longer finds them appealing. Intimacy and a satisfying sex life may be hindered by physical issues brought on by health disorders as well as by stress and anxiety. Men and women have different sexual problems. Erectile Dysfunction, or ED, occurs when a man has difficulty achieving or maintaining an erection. In certain cases, ED is caused by prostate cancer therapies, such as prostatectomy (a procedure in which the prostate is surgically removed in its entirety or in part). The condition is also caused by other health issues, such as excessive blood sugar, heart disease, or pelvic trauma. Notwithstanding the transformations brought on by natural aging, sexuality remains a vital aspect of our lifestyles as we age. Sexual health and function are essential elements of healthy aging. Sexual activity is connected with superior psychological health, self-esteem, marriage quality, as well as personal well-being, whereas its absence is linked to a worse standard of living. Considering the significance of sexual function and sexual well-being in evaluating healthy aging, as well as the inadequate knowledge of urological health among elderly people, this research topic highlights some of the key urological health aspects relevant to the journeys and mental or medical consequences of healthy aging, psychological wellbeing, and sexuality. Among the broad questions of interest are the following: - The experiences of older adults who identify as having a urological disorder or a mental disorder - Sexual health and sexual function/dysfunction in older adults - Correlation between psychological disorder and urological health among older adults - Culture/sociological discussion between psychological distress and healthy aging among older adults - Addressing sexual function and sexual well-being and healthy aging - Case studies of effective practice - Specific sexual orientation issues among older adults with urological disorders - Incorporating a patient care address for older adults with urological illnesses into the community We are thus seeking innovative research papers, review articles, hypothesis and theory pieces, viewpoint articles, short research report articles, commentary articles, opinion articles, and book reviews. Book excerpt: The current study surveyed clinical psychology doctoral students' (N=291) knowledge and attitudes about older adult sexuality. Knowledge and attitudes were measured using the Facts of Aging Quiz, the Aging Sexuality Knowledge and Attitudes Scale, Attitudes Towards Sexuality Scale, and measures to explore students' exposure to and interests in the older adult population, as well as academic and clinical exposure (coursework, practicum opportunities and clinical contacts with older adults) to the older adult population and older adult sexuality. Generally, students' knowledge about aging was associated with their attitudes about older adult sexuality; the more knowledge a student has about older adult sexuality, the more positive attitudes they have about older adult sexuality. However, the current study demonstrated that clinical psychology doctoral students' lack of exposure to the older adult population did not impact their knowledge or attitudes about older adult sexuality. This study suggests that the relationship between students' knowledge and attitudes about older adult sexuality may be more complex due to different types of exposure. Students' knowledge and attitudes about older adult sexuality may be shaped by personal experiences with older adult relatives and being a caretaker of an older adult. Future research is needed to further investigate different types of exposure and how those impact students' knowledge and attitudes about older adult sexuality.
