Book excerpt: Based on an analysis of data on people who graduated from U.S. medical schools in 1975, this study reports on the ways that specialty choice, practice location, patient populations served, and board certification rates differ between minority and nonminority graduates. It also considers the relationship of premedical school performance and socioeconomic status to these variables. Although they entered the primary care specialties to a greater extent than nonminorities, there is an impressive dispersion of minority graduates across all specialties. Minority graduates are practicing in physician-shortage areas at twice the rate of their nonminority counterparts, and they are caring for significantly greater proportions of minority and Medicaid patients. Only about half the minority physicians had obtained board certification in their specialty, compared with four-fifths of the nonminority graduates. The results for specialty choice, practice location, and patient characteristics support the continuing affirmative action in medical schools.

Book excerpt: Affirmative action programs have significantly changed American medicine for the better, not only in medical school admissions and access to postgraduate training but also in bringing a higher quality of health care to all people. James L. Curtis approaches this important transition from historical, statistical, and personal perspectives. He tells how over the course of his medical education and career as a psychiatrist and professor--often as the first or only African American in his cohort--the status of minorities in the medical professions grew from a tiny percentage to a far more equitable representation of the American population. Advancing arguments from his earlier book, Blacks, Medical Schools, and Society, Curtis evaluates the outcomes of affirmative action efforts over the past thirty years. He describes formidable barriers to minority access to medical-education opportunities and the resulting problems faced by minority patients in receiving medical treatment. His progress report includes a review of two thousand minority students admitted to U.S. medical schools in 1969, following them through graduation and their careers, comparing them with the careers of two thousand of their nonminority peers. These samples provide an important look at medical schools that, while heralding dramatic progress in physician education and training opportunity, indicates much room for further improvement. A basic hurdle continues to face African Americans and other minorities who are still confined to segregated neighborhoods and inferior school systems that stifle full scholastic development. Curtis urges us as a nation to develop all our human resources through an expansion of affirmative action programs, thus improving health care for everyone. James L. Curtis is Clinical Professor Emeritus of Psychiatry, Columbia University College of Physicians and Surgeons.

Book excerpt: Few organizational communication studies examine the organizational aspects influencing career satisfaction specifically among non-white cultures in the medical physician population. This study examines minority physicians' perceptions of extrinsically controlled work environment factors in comparison to their white counterparts. Three research questions were analyzed from a 17-question survey tool to measure: physician satisfaction levels with autonomy over medical decision-making; autonomy over non-medical workplace decisions; and hospital cost containment efforts. These organizational variables have served as major points of discourse within the healthcare arena and they relate to the enigmatic nature of career satisfaction. Determined by the volume of respondents representing each race and ethnicity, five categories were selected for comparison: Asian/Pacific Islander, Indian/Pakistani, White/Non-Hispanic, Hispanic, and Black/African American. Participants that were surveyed included all physicians listed on the medical staff roster of a Southeastern, not-for-profit hospital group, regardless of status and medical specialty. The primary findings indicate that substantial variance exists among racial and ethnic subgroups regarding satisfaction with the dependent measures. Due to low numbers of minority health care physicians, previous studies have commonly measured physician job satisfaction aggregately, failing to differentiate cultural groups. Interestingly, when minority and non-minority groups were aggregately juxtaposed, no significant differences were reported in the data. However, when satisfaction was measured contrasting minority subgroupings with that of non-minority physicians, significant variations emerged from the data set. This study contributes to understanding better the organizational experiences of minority physicians in healthcare and the body of knowledge concerning minority health research as a whole.

Book excerpt: In its 2004 spring report, the Institute of Medicine (IOM) posits that Behavioral Sciences provides a perspective that can assist physicians in understanding their patients as embedded in a larger social and environmental context (Patricia A. Cuff and Neal Vanselow, Editors, Improving Medical Education: Enhancing the Behavioral and Social Science Content of Medical School Curricula). The report further indicates that perspective and context is crucial to patient care in terms of understanding the interaction between biological, cognitive, and learning processes to produce behavior, illness and health. Perspective and context is equally important in understanding physician satisfaction. The authors present their findings from a qualitative research study at one northeastern medical college in which they explored factors influencing physician satisfaction. The authors describe responses from practicing physicians and how responses were categorized into twenty (20) themes. Categorized themes from the study include: academic side of medicine, balancing personal and professional life, business side of medicine, clinical issues within specialty, dissatisfaction with medicine, expectations of medicine, gender, geographical location, match, monetary rewards, personal rewards/ satisfaction, politics within medicine, practice of medicine, pressures and demands, switching specialty/quitting medicine, stress associated with medicine, time commitments, training issues, full time/part time work, and other circumstances (i.e., military, locum tenums). The authors share "rich" examples from various themes found in the study. What are the implications of this study, specifically as it relates to behavioral science departments in American medical colleges? Participants in the study offer advice to medical students and faculty. The authors share this advice and solicit further suggestions for behavioral sciences from conference participants. The IOM recent report recommends that medical students demonstrate competency in one of six domains, including physician role and behavior. Physician well being is one of the high priority topics for inclusion in medical school curricula. Understanding physician satisfaction and assisting medical students in making informed choices about specialty choice is an important area for medical schools to examine. Including physician satisfaction and medical specialty choice in the behavioral sciences curriculum can also provide perspective and context about the complexity of medicine--a positive step towards physician well being.
