There were in your profession - even among those most unsparing and heroic in the use of Opium, 'calomel, and lancet not a few men of the highest order of nobility, mental and moral, uniting learning and skill, tenderness and intrepidity, the amenities that adorn and the virtues that glorify humanity. Indeed, no profession in New England has been more, or more worthily, honored in its members than yours. Yet half a century ago success in it did not demand any higher qualities than success in homoeopathy its caricature does now. Medical practice then consisted mainly in the observation of symptoms, and the exhibition of the supposed specifics in quantities. Not too small. It required good perceptive powers, cool judgment, and a care ful hand, - not much more. It was for the most part a mechanical business, - of the higher sort, indeed, and differing from other similar vocations in its being expected to repair machines while in motion, yet still a handicraft that by no means de manded the superior-endowments of mind and char acter with which it was often associated; and if not in our larger towns, there were in our rural districts, and even in the suburbs of Boston, physicians of very high reputation and extensive practice who possessed none of the attributes of a scholar, a gen tleman, or a Christian. Book excerpt: Excerpt from Success in the Medical Profession: An Introductory Lecture Delivered at the Massachusetts Medical College, November 6th, 1850 N 0 man who embarks his lot for life in our profession - no man who intends to practise it with a view to its highest usefulness to others, as well as to his own best advantage, should aim at the cultivation of the qualities that can only gain him a patronage so worthless and evanescent as this. I believe, gentlemen, I may say with truth, that everything re lating to your studies, your character, your habits, your manners, your social and professional intercourse, may have an influence more or less important on your prosperity in the profession you have chosen; and my present purpose is to offer a few suggestions to aid in preparing you to acquire that sort of confidence which I have described as the only sure foundation for permanent success. Of course I shall be expected to say that a thorough knowledge of medicine is of the first importance, and assuredly it is so. But this general statement requires some qualification. Medicine embraces a vast field of knowledge. To go over the whole of it is impossible, even in a long life. The pupil can only begin its cultivation. All this knowledge is of value; but all is not of equal value. All has some connection, but not an equal connection, with practice. Now the main purpose of the study of medicine with most of us, is to enable us to treat disease. This is the ultimate object, which is to be kept in view at every step. It is for this end that the science of medicine exists for this that the profession exists. This is never for a moment to be forgotten. No doubt there are some physicians whose aim is different - to whom practice is a secondary concern. They mean to acquire a great proficiency in some particular department. They mean to devote peculiar attention to anatomy, to pathology, to organic chemistry, to the microscopic study of organic forms. These are all useful pursuits, and they all have a useful bearing upon practice. But I am not speaking to such persons. The mass of medical students are to be practitioners, and practitioners only. Book excerpt: Although modern medicine enjoys unprecedented success in providing excellent technical care, many patients are dissatisfied with the poor quality of care or the unprofessional manner in which physicians sometimes deliver it. Recently, this patient dissatisfaction has led to quality-of-care and professionalism crises in medicine. In this book, the author proposes a notion of virtuous physician to address these crises. He discusses the nature of the two crises and efforts by the medical profession to resolve them and then he briefly introduces the notion of virtuous physician and outlines its basic features. Further, virtue theory is discussed, along with virtue ethics and virtue epistemology, and specific virtues, especially as they relate to medicine. The author also explores the ontological priority of caring as the metaphysical virtue for grounding the notion of virtuous physician, and two essential ontic virtues—care and competence. In addition to this, he examines the transformation of competence into prudent wisdom and care into personal radical love to forge the compound virtue of prudent love, which is sufficient for defining the virtuous physician. Lastly, two clinical case stories are reconstructed which illustrate the various virtues associated with medical practice, and it is discussed how the notion of virtuous physician addresses the quality-of-care and professionalism crises.
