Book excerpt: As biocompatible and osteo-inductive precursor to biological apatite formation, amorphous calcium phosphate (ACP) resorbs at the rate that closely coincides with the rate of new bone formation and is more osteo-conductive than its crystalline counterpart. In addition, in the oral environment, ACP intrinsically provides a protracted supply of the remineralizing calcium and phosphate ions needed for regeneration of mineral lost to tooth decay. These features make ACP composites a strong remineralizing tool at the site of caries attack. Our group has been on the forefront of the research on bioactive, remineralizing, polymeric ACP-based dental materials for over two decades. This entry describes methods for filler, polymer, and composite fabrication and a battery of physicochemical and biological tests involved in evaluation of ACP-based restoratives. Also presented is our most recent design of ACP remineralizing composites with added antimicrobial capability that shows promise for extended dental and, potentially, wider biomedical applications.

Book excerpt: Our objective was to investigate the complex structural and dynamical conversion process of the amorphous-calcium-phosphate (ACP)-to-apatite transition in ACP based dental composite materials. Composite disks were prepared using zirconia hybridized ACP fillers (0.4 mass fraction) and photo-activated Bis-GMA/TEGDMA resin (0.6 mass fraction). We performed an investigation of the solution-mediated ACP-to-apatite conversion mechanism in controlled acidic aqueous environment with in situ ultra-small angle X-ray scattering based coherent X-ray photon correlation spectroscopy and ex situ X-ray diffraction, as well as other complementary techniques. We established that the ACP-to-apatite conversion in ACP composites is a two-step process, owing to the sensitivity to local structural changes provided by coherent X-rays. Initially, ACP undergoes a local microstructural rearrangement without losing its amorphous character. We established the catalytic role of the acid and found the time scale of this rearrangement strongly depends on the pH of the solution, which agrees with previous findings about ACP without the polymer matrix being present. In the second step, ACP is converted to an apatitic form with the crystallinity of the formed crystallites being poor. Separately, we also confirmed that in the regular Zr-modified ACP the rate of ACP conversion to hydroxyapatite is slowed significantly compared to unmodified ACP, which is beneficial for targeted slow release of functional calcium and phosphate ions from dental composite materials. Significantly, for the first time, we were able to follow the complete solution-mediated transition process from ACP to apatite in this class of dental composites in a controlled aqueous environment. A two-step process, suggested previously, was conclusively identified.

Book excerpt: Several studies have found that resin-based amorphous calcium phosphate (ACP) composites can function well for applications that do not require high mechanical demand. Milled tricalcium phosphate (TCP), a new calcium-phosphate-releasing material, is crystalline in nature, suggesting it to be strong. In the present study, we investigated the use of a TCP-filled composite resin as a possible tooth restorative-material. An experimental TCP-based composite was prepared using monomer with a mixture of 34.3 percent by mass of EBPADMA, 34.2 percent by mass of HmDMA, and 30.5 percent by mass of HEMA. TCP fillers were added to the monomer mixture at different levels (30 percent, 40 percent, 50 percent, and 60 percent by weight). A universal testing machine (Sintech Renew 1121; Instron Engineering Corp., Canton, MA) was used to measure the compressive strength and modulus. FTIR was used to measure the degree of conversion. The depth of cure was determined according to the ISO standards for dental resin 4049 using the scrapping technique. Knoop hardness numbers were obtained by a microhardness tester (M-400; Leco Co., St. Joseph, MI). The viscosities of the experimental resin were determined in a viscometer (DV-II+ Viscometer; Brookfield, Middleboro, MA). The data were analyzed using a one-way analysis of variance (ANOVA). A 5-percent significance level was used for all the tests. Resin composites with 30-percent TCP filler showed the highest compressive strength and hardness values. Also, this group showed the lowest degree of conversion. Resin composites with 60-percent TCP filler showed the highest degree of conversion. However, this group showed the lowest compressive strength, depth of cure, and hardness. Resin composites with 50-percent filler showed the highest compressive modulus. Resin composites with 40-percent filler showed higher viscosity values than resin composites with 30-percent filler. In conclusion, increasing the filler level significantly reduced the compressive strength, hardness, and depth of cure, but increased the degree of conversion. Also, resin composites with the lowest filler level (30 percent) had the highest compressive strength, depth of cure, and hardness. From these results, it can be concluded that the experimental TCP-filled resin used in this study cannot be used as restorative material.

Book excerpt: Critical size bone defects are large defects that do not spontaneously heal over time and result in permanent non-union of the bone. These abnormalities can result from traumas, surgeries, and diseases. Surgical methods used to treat these defects include natural (autografts and allografts) and synthetic bone grafts. Autografts draw advantages from diminished immune response but suffer from complications during implantation as well as pain and cosmetic defects during removal. Allograft complications include rejection and infection. Infection can be minimized by sterile processing techniques, but this frequently leads to diminished mechanical properties post-processing. As a consequence of these limitations, synthetic bone grafts have drawn significant interest. Synthetic bone grafts include biodegradable polymers, metals, and ceramics. For example, poly(caprolactone) (PCL) and poly(L-lactic acid) (PLLA) are biodegradable polymers used widely for medical applications. These materials can be molded into any desired shape; however, they are unsuitable for load-bearing applications as a consequence of their poor mechanical properties post-processing. Bioceramics, such as amorphous calcium phosphate (ACP), benefit from their osteoconductive behavior but are too brittle for load-bearing applications. Combining the biodegradable polymers with bioceramics have shown increased mechanical properties in the composites. The goal of this study was to examine the resulting mechanical properties of combining an L-phenylalanine-based poly(ester urea) (PEU) with various amount of ACP. We hypothesize that adding ACP to the polymer will lead to enhanced mechanical properties compared to the pure polymer.
