Sixteen to eighteen million people are currently infected with this organism, and 45,000 deaths are attributed to the disease each year. Infection with T. cruzi is life-long, and 10-30% of persons who harbor the parasite chronically develop cardiac and gastrointestinal problems associated with the parasitosis. Although major progress has been made in recent years in reducing vector-borne and transfusion-associated transmission of T. cruzi, the burden of disability and death in persons chronically infected with the organism continues to be enormous. Eight to ten million persons born in countries in which Chagas disease is endemic currently reside in the United States, and epidemiologic and census data suggest that 50,000-100,000 are chronically infected with T. cruzi. The presence of these infected persons poses a risk of transmission of the parasite in the USA through blood transfusion and organ transplantation and several such cases have now been documented. American Trypanosomiasis, volume seven of World Class Parasites is written for students of tropical medicine, parasitology and public health, for researchers and practitioners alike who wish to bring themselves abreast of the status quo with respect to this disease. In order to donate blood, prospective donors should be in good health and free from any infections that can be transmitted through transfusion. Most blood donors perceive themselves to be healthy, but some are unsuitable to donate blood due to the potential risk of compromising or worsening their own health or the risk of transmission of infections to patients. Blood transfusion services (BTS) have a duty of care towards blood donors as well as to the recipients of transfusion. This duty of care extends to prospective donors who are deferred from donation--whether on a temporary or permanent basis--as well as those who donate blood and are subsequently found to have unusual or abnormal test results. BTS have a responsibility to confirm test results and provide information, counseling and support to enable these individuals to understand and respond to unexpected information about their health or risk status. Counseling is part of the spectrum of care that a BTS should be able to provide to blood donors--including referral to medical practitioners or specialist clinical services. Pre-donation counseling was recognized as one element of the strategy to reduce and, if possible, prevent the donation of blood by individuals who might be at risk for HIV and other TTI including hepatitis B and C viruses as well as to inform the donor of the donation process and testing of blood for HIV. Post-donation counseling was acknowledged to be a necessary element of donor management as an adjunct to informing donors of unusual or abnormal test results. Blood donor counseling by trained specialist staff is now considered to be a key component of the blood system in most countries with a well-developed blood transfusion service. It may be required at a number of stages in the blood donation process or following blood screening and should be available at any point at which the BTS has an interface with donors. In many countries, however, blood donor counseling is not yet available in a structured way. Blood Donor Counselling: Implementation Guidelines has therefore been developed to provide guidance to blood transfusion services that have not yet established donor counseling programs.

In addition to its medical importance, T. cruzi is an interesting biological model for studying processes such as: (1) cell differentiation, where a non-infective stage transforms into an infective one; (2) cell invasion, where the infective stages are able to penetrate into a mammalian host cell, where they multiply several times and thus amplify the infection; and (3) evasion from the immune system, using several mechanisms. Between 1909 and 1911, Carlos Chagas described an infection (pathogenic trypanosome), its intermediate host, and the illness that he believed it caused, parasitic thyroiditis. Chagas's work did not lack significance: the disease that came to share his name would be one of Latin America's most serious endemic diseases. However, the clinical identification of the disease through "Roma�a's sign" (a palpebral edema or swelling of the eyelid) some decades later marked a transformation in the general medical knowledge of the disease and its basis altogether. Not only was the disease entity that Chagas had described shown to be a nosological illusion, but twenty-five years of scientific controversy turned out to have been based on a misunderstanding. The continued use of the term "Chagas's Disease" even after Cecilio Roma�a's discovery thus refers to a fundamental ambiguity. Delaporte dispels this ambiguity by re-examining the various discoveries, dead ends, controversies, and major epistemological transformations that marked the history of the disease--a history that begins with the creation of the Oswaldo Cruz Institute in Rio de Janeiro and ends in the forests of Santa Fe in northern Argentina. Delaporte's study shows how an epistemological focus can add depth to the history of medicine and complexity to accounts of scientific discovery.
