Book excerpt: Excerpt from The American Medical Intelligencer, Vol. 1: A Concentrated Record of Medical Science and Literature; July, 1841, to July, 1842 Having seen in your work upon New Remedies, published some time since in the Library, an article upon the beneficial effects derived from inoculation with morphia in the treatment of local diseases, I determined to make a trial of the plan in local rheumatism, an affection which I have fre quently found great difficulty in managing. I was not long afterward called to treat an obstinate rheumatic affection of the knee-joint, which had resisted the usual general and local means. One quarter of a grain of the sulphate Of morphia was inserted, by means of punctures, in the skin over the affected part, twice a day for two days, with the most marked and satisfactory results. The patient, who for two weeks previous had suffered extreme pain upon the slightest motion of the limb, was, at the expiration of this period, able to walk with slight inconvenience, and upon the third day, threw aside the crutch with which he had before hobbled across his room. Friction with stramonium ointment, two or three times repeated during the fourth day, removed all remaining disease in the affected part. The next case was a rheumatism of the wrist, in which no previous treat ment had been practised. In this case, which was recent, two applications of the morphia, upon two succeeding days, so effectually removed the pain and tenderness, as to allow of the free use of the hand on the third day. If the individual be achild, he may be laid across his mother's lap; if older, he shouldbe placed flat on his face upon a bed or table; an assistant steadies the limb, while the surgeon grasps the foot with the right or left hand, as thefcase may be, and forbibl extends it, so as to relax/.the tendon and the integu ments covering it. 1he then passes from within outwards a narrow convex edgedf bistoury, about one or two inches above the os calcis, and between the integument and tendon, until its point, gets bey'on'd thefouter margin of the latter ithe foot is then suddenly flexed, which brings the tendon against the knife, previously turned upon its edge, and with very little pressure upon the'instrument the operation is completed, which is generally indicated by the snap, and by a sudden jerk; As soon as this is perceived, and not until then, the knife is Withdrawn in the same way in which it had been introduced. The little wound is then closed by adhesive plaster, the stretching apparatus applied, and the subsequent treatment conducted as I have already indicated. Whenever it seems necessary to divide other ten dons, the operation is to be performed upon a similar 'plan; make but one puncture, and divide them directly across, and then begin to extend the parts at once. Book excerpt: Excerpt from The American Medical Intelligencer, Vol. 4: A Concentrated Record of Medical Science and Literature; From April 1, 1840, to April 1, 1841 On the present occasioni wish to call your attention to the subject of uterine hemorrhage, in one of its most peculiar and imminently dangerous forms - I mean hemorrhage accompanied by a detachment of the placenta, together with an infiltration of blood in its substance, constituting what has been termed placentary apoplexy, and arising about the close of pregnancy. It may occur either independently of labour, or whilst labour is progressing. The one Object I have in view being to have the matter fully understood by you'all, I will enter upon the subject without further preface, studying only plainness and clearness of description. I have said I Wish to call your attention to hemorrhage, accompanied with detachment of the placenta. First Of all, then, I would have you Observe, that the detachment usually commences abmrt the centre of the mass, and extends to every part Of it, the edge excepted, which maintains its natural apposition; consequently, a large quantity Of blood soon becomes confined between the placental and uterine surfaces. The uterine tumour at these points becomes raised in proportion to the amount of effusion, its rapid aug mentation constituting the most striking feature in the case. But it is im portant to Observe, that the effusion may commence any where between the centre of the placenta and its edge, which almost necessarily becomes more or less detached, so that whilst a large coagulum is confined, partly under neath the placenta and partly exterior to the membranes, the liquid blood continues detaching the membranes until it reaches the vagina I have seen both forms Of hemorrhage. The first, or concealed form, from its greater liability to deceive the practitioner than the second, may be regarded as the more dangerous case, although the extent Of hemorrhage in thisform is less considerable than in the other.
