AFP can deliver toxins through specific receptors re-expressed in the majority of cancer cells, serving as a targeted chemotherapy. Depleting the regulatory top monocyte-derived suppressor cells (MDSCs) activates both the innate and adaptive immune responses. The thoroughly chosen toxins switch on apoptosis in MDSCs, restore the broken one in cancer cells, and destroy them naturally without any harmful by-products. Injections with AFP+toxin preparations have shown promising results in animals and the treatment of cancer patients. In addition to this breakthrough, the book also discusses the peroral administration of porcine AFP non-covalent complexes with selected toxins in patients with metastases. Book excerpt: Alpha-fetoprotein (AFP) has many applications across various disciplines. This oncofetal protein is routinely used by clinicians for antenatal testing as well for the screening and treatment monitoring of certain malignances. Innovative research using AFP has also been very promising. Recent developments include the use of AFP or its recombinant fragments for drug delivery to cancer cells, as well as for the engineering of cancer vaccines and T cell immunotherapies. The anti-estrogenic potential of AFP has also been utilised to develop drugs directed against estrogen receptor positive malignancies. This book is divided into five sections. In Section One, the history of AFP is recollected, and the role of AFP in normal and pathological conditions is explored. Section Two and Three focus on the utility of AFP during pregnancy. In Section Two, the use of routine maternal serum AFP screening for detection of chromosomal aneuploidies and neural tube defects in normal pregnancies and pregnancies with pre-existing disease is discussed; the potential use of AFP for the detection of preeclampsia is also reviewed. Section Three explores the pathogenesis and data linking unexplained elevations of AFP to adverse obstetrical outcomes. Section Four presents chapters that discuss AFP ligand binding, receptor targeting, and drug delivery strategies. Finally, Section Five serves to explain the regulatory roles of AFP, its use in immunotherapy, and in the development of cancer vaccines. The aim of this book is to bring together some of the great research and clinical insights that currently surround AFP. The target audiences for this book are basic science researchers as well as clinicians.

Book excerpt: Hepatocytes and bile ductular cells possess stem-like potential in restoring liver tissue. This was derived from rat and mouse injury models such as (a) partial hepatectomy by surgery; (b) acute carbon tetrachloride intoxication; © injuries by D-galactosamine (GalN) or by N-nitrosomorpholine (NNM); (d) experimental hepatocarcinogenesis with NNM using different doses. Liver parenchyma was restored by adult hepatocytes after partial hepatectomy as well as after carbon tetrachloride intoxication. Concurrently, hepatocytes resumed synthesis of alpha-1-fetoprotein (AFP) which depended to a high degree on the animal species and strains used in that studies. In contrast to this way of regeneration, liver repair after heavy injuries by agents such as GalN and NNM was initiated by oval cell proliferation deriving from ductular epithelial cells of the canaliculi of HERING (canals of Hering, intrahepatic bile ductules) as sources of endogenous progenitors. Progenitor populations reach levels of differentiation with fetal gene reactivation and significant AFP expression. Proliferating oval cells operated as transit and amplification compartment during liver regeneration. Oval cell proliferation and concurrent AFP synthesis occurred also in the early stages of hepatocarcinogenesis when the carcinogen NNM was applied in toxic doses. The histologic features compared to those seen in GalN intoxication. NNM carcinogenicity was of concern for both oval cells and adult hepatocytes with risks of transformation and malignant development. At the cancer stage, carcinomas either synthesizing AFP or not can be found side by side in the same liver. AFP expression by cancer cells seems to be a special attribute to differentiation stages in carcinogenesis. Clonality, maturation arrest and retro-differentiation feature cells with high autonomy.
