Book excerpt: A number of chronic respiratory diseases including chronic bronchitis, asthma, cystic fibrosis and bronchiectasis are characterized by mucus hypersecretion. Following damage to the airway epithelium, a repair process of dedifferentiation, regenerative proliferation and redifferentiation takes place that is invariably accompanied by mucus hypersecretion as a key element in the host defence mechanism. In chronic respiratory diseases, however, excessive mucus production leads to a pathological state with increased risk of infection, hospitalization and morbidity. An understanding of the mechanisms that underlie and maintain this hypersecretory phenotype is therefore crucial for the development of rational approaches to therapy. Despite a high and increasing prevalence and cost to healthcare services and society, mucus hypersecretion in chronic respiratory disease has received little attention until recently, probably because of the difficulties inherent in studying this pathology. Only in the last few years have some of the genes involved in mucus secretion been characterized. The recent availability of genomic sequence information and specific antibodies has led to an explosion of interest in this area making this publication particularly timely. This book draws together contributions from an international and interdisciplinary group of experts, whose work is focused on both basic and clinical aspects of the problem. Book excerpt: Asthma is a chronic airway disease affecting over 300 million people worldwide with an expected increase of an additional 100 million by 2025. Past decade has observed a notable increase in asthma prevalence on both national and global levels with highest rates observed in western countries (about 30%). Over the past 40 years, a drastic increase in global prevalence, morbidity, mortality, and economic burden have been observed due to asthma especially in children. The rising numbers of hospital admissions for asthma, especially young children, reflect an increase in severe asthma, poverty and lack of proper disease management. Worldwide, approximately 180,000 deaths annually are caused due to this condition. The financial burden on a single asthma patient per year in different western countries ranges from US$300–1,300. Asthma is an intricate respiratory disorder with differences in its severity, natural history and hence treatment response. These differences in intensities of various presentations such as bronchial hyper-responsiveness, airway inflammation, mucus production, airflow obstruction make asthma a heterogeneous disease. The mainstay of current therapies for asthma includes inhaled corticosteroids, phosphodiesterase inhibitors, leukotriene modifiers and β2-adrenoceptor agonists. Some of the currently available drugs are efficient in one or more aspects. However the associated side effects or heterogeneity of the disease limit their usefulness and efficacy, thereby putting a demand on development of new drugs and therapies. On the other hand, asthma has also been treated/managed via herbal medications. These approaches have been described in Unani, Ayurvedic or Chinese system of medicine since antiquity. In fact, several anti-asthmatic drugs were developed from herbs commonly utilized in the non-Western system of medicine. This book focuses on the pathophysiology of asthma, its medication (both herbal and modern), limitations and their future prospects.

Such defensive mechanism stems primarily from a constant clearance of mucus-entrapped inhaled pathogens and particulates by mucociliary transport. The success in removing mucus-entrapped harmful substances relies heavily on proper mucus rheology. It is well established that hydration is the single most important determinant of mucus rheological properties. In a healthy condition, the body precisely controls the degree of mucus hydration through modulating physiological parameters of various ionic concentrations in the nascent microenvironment which bathe the mucus. On the contrary, inadequate mucus hydration and abnormal mucus hypersecretion seem to be the major causal factors underlying pathological states involving the accumulation of highly viscous mucus plugs, obstructive airflow, chronic infection and inflammation. These symptoms are characteristic of pulmonary diseases including, but not limited to, chronic obstructive pulmonary disease (COPD), asthma, and cystic fibrosis (CF). My research focused on the mechanisms of how (i) environmental and (ii) physiological factors affect mucus secretion and rheological properties. My first project tested whether titanium dioxide nanoparticles (TiO2 NPs), a common air pollutant that constitutes airborne particulate matter, can induce mucin hypersecretion from airway epithelial cells via a Ca2+-mediated pathway. Aside from being an airborne pollutant, many engineered NPs are utilized in the nanoindustry and have acquired secondary surface modifications. The question of functionalized NPs with surface charges being able to alter mucus rheological properties is the focal point of the second project. The third study aimed at understanding how physiological parameters involving bicarbonate ion concentrations regulate mucus rheological characteristics.
