Stomach cancer represents roughly 2 percent (25,500) cases of all new cancer cases yearly in the United States, but it is much more common in Japan, Great Britain, South America, and Iceland. It is associated with high salt in the diet, smoking, and low intake of fruits and vegetables.Infection with H. pylori is the main risk factor in about 80 percent or more of gastric cancers. It is more common in men. A very small percentage of diffuse-type gastric cancers are thought to be genetic. Hereditary Diffuse Gastric Cancer (HDGC) has only recently been identified and research is ongoing, but genetic testing and treatment options are available for families at risk. Metastasis occurs in 80-90 percent of individuals with stomach cancer, with a five year survival rate of 75 percent in those diagnosed in early stages and less than 30 percent of those diagnosed in late stages. However at the same time microbial activation of the immune system can help to generate a protective immunity that prevents allergen sensitization. The current book is collecting these evidences and connects epidemiologic and clinical mechanistic knowledge. Only the synthesis of this knowledge will help to find solutions to the ongoing allergy epidemic in terms of public health activities, prevention and therapy.

Book excerpt: Myelin oligodendrocyte glycoprotein (MOG) antibody disease (MOGAD) is a neurological, immune-mediated disorder targeting MOG protein, located on the outermost myelin sheaths in the central nervous system. The diagnosis is confirmed when serum MOG antibody is found in patients in the context of inflammatory attacks of the central nervous system, usually with the brain, brainstem, optic nerve, and spinal cord. Although MOGAD manifestations can fulfill diagnostic criteria for Neuromyelitis optica disorder (NMOSD), evidence suggests that they have distinct immunological mechanisms. Similar to NMOSD, patients with persistent MOG antibodies are at risk for recurrent events. Recovery of MOGAD is generally reasonable. However, some patients get disability after a severe acute attack. Treatment for an acute attack is typically intravenous high-dose steroids, plasma exchange (PLEX), or intravenous immunoglobulin (IVIG). Maintenance therapy with immunosuppressants, i.e., azathioprine, mycophenolate, rituximab, etc., usually prevents future attacks, although they have long-term side effects. Therefore, it is still controversial how long treatment should be continued.
