Book excerpt: Persistent high fertility in sub-Saharan Africa has been of concern to demographers and global health experts for decades. Sub-Saharan African fertility levels and trends are singled out as markedly higher and different than the rest of the world and from what is expected based on historic fertility trends. Global population projections, which indicate continued population growth in sub-Saharan Africa while population stabilizes and declines elsewhere brings renewed focus on African fertility. This dissertation seeks to address some of the challenges to understanding modern fertility regimes and fertility dynamics posed by persistent high fertility in sub-Saharan Africa by examining it in a global, demographic context. Population measures of sterility are traditionally constructed for women, despite fertility and sterility being conditions of the couple. Estimates of male sterility provide insight into population-level sterility and complement estimates based solely on women. Chapter 2 seeks to estimate male sterility for the Gwembe Tonga of Zambia using male birth histories collected by the Gwembe Tonga Research Project from 1957 to 1995, while providing context by estimating female sterility for the Gwembe Tonga as well as female sterility in all of Zambia from Zambian DHS data (1992, 1997, 2001-02, and 2007). Sterility is measured using the Larson-Menken subsequently infertile indicator. Estimates are produced using discrete time event history analysis. The odds of sterility were higher for women than men, though women's odds of sterility were only 1.5 times that of men's in the middle reproductive years. The odds of sterility increased steadily with age for both men and women, and across all datasets. However, women's sterility increased much more sharply with age than men's and women's odds of sterility were higher than men's at all reproductive ages. Chapter 3 aims to understand trends in global fertility from 1950-2010 though the analysis of age-specific fertility rates. This approach incorporates both the overall level, as when the total fertility rate is modeled, and different patterns of age-specific fertility to examine the relationship between changes in age-specific fertility and fertility decline. Singular value decomposition is used to capture the variation in age-specific fertility curves while reducing the number of dimensions, allowing curves to be described nearly fully with three parameters. Regional patterns and trends over time are evident in parameter values, suggesting this method provides a useful tool for considering fertility decline globally. The second and third parameters were analyzed using model-based clustering to examine patterns of age-specific fertility over time and place; four clusters were obtained. A country's demographic transition can be traced through time by membership in the different clusters, with regional patterns in the trajectories through time and with fertility decline. Generally determinants of fertility decline are hypothesized to encourage declines at specific ages as a result of the mechanisms by which these determinants exert influence on fertility. Using the reduced parameterization of age-specific developed in this dissertation, Chapter 4 aims to examine the association of determinants of fertility with age-specific fertility curves to test hypotheses regarding how determinants of fertility decline are associated with age-specific fertility. Results of this analysis corroborate predictions made in the literature about how age-specific fertility is affected by mortality, development, HIV prevalence, women's empowerment, contraception use and urbanization. However, some hypotheses were not born out, notably the association between low mortality and stopping behavior. The evidence indicates that these relationships are not uniform across world regions and that these relationships are different across world regions.

The book examines the macro and micro theories of the value of children, and considers the multi-dimensional nature of value change. The chapters explore the nature of the mechanisms by which value change may serve to reinforce or promote the ideational essence of change and the impact of pressures for change. It is observed that gender, religion, and culture, all function as complementary lenses through which the necessity of value maintenance or modification is viewed. The book concludes that fertility behaviour is value-driven, but that fertility change is not necessarily driven by value change. The values of most significance to fertility are more fundamental and general values, rather than explicit 'fertility values'.

Available in PDF, EPUB and Kindle. Book excerpt: This paper analyzes the relationship between age-specific fertility, mortality and real wages in Sweden during the demographic transition. We take an overlapping generations model of life cycle fertility and fit it to actual Swedish time-series data over the past two and a half centuries. The model fits the data well, accurately portraying the total fertility decline from more than four children per female before the mid-19th century to about two children today. About 80% of this decline was in fertility that occurred at female ages over 30. The fitted model implies that reductions in child mortality over this period is the most important factor explaining the fertility decline, while increases in the real wage can explain only less than one-third of the decline in fertility. However, their combined effect was considerably larger than a simple summing up would predict. The fertility decline was magnified as well by the combination of increasing real wages and rising adult survival rates. In addition, we find that a model that is estimated based only on pre-transition data would actually overstate the subsequent fertility decline.

Book excerpt: The Republic of Indonesia, home to over 240 million people, is the world's fourth most populous nation. Ethnically, culturally, and economically diverse, the Indonesian people are broadly dispersed across an archipelago of more than 13,000 islands. Rapid urbanization has given rise to one megacity (Jakarta) and to 10 other major metropolitan areas. And yet about half of Indonesians make their homes in rural areas of the country. Indonesia, a signatory to the United Nations Millennium Declaration, has committed to achieving the Millennium Development Goals (MDGs). However, recent estimates suggest that Indonesia will not achieve by the target date of 2015 MDG 4 - reduction by two-thirds of the 1990 under - 5 infant mortality rate (number of children under age 5 who die per 1,000 live births) - and MDG 5 - reduction by three-quarters of the 1990 maternal mortality ratio (number of maternal deaths within 28 days of childbirth in a given year per 100,000 live births). Although much has been achieved, complex and indeed difficult challenges will have to be overcome before maternal and infant mortality are brought into the MDG-prescribed range. Reducing Maternal and Neonatal Mortality in Indonesia is a joint study by the U.S. National Academy of Sciences and the Indonesian Academy of Sciences that evaluates the quality and consistency of the existing data on maternal and neonatal mortality; devises a strategy to achieve the Millennium Development Goals related to maternal mortality, fetal mortality (stillbirths), and neonatal mortality; and identifies the highest priority interventions and proposes steps toward development of an effective implementation plan. According to the UN Human Development Index (HDI), in 2012 Indonesia ranked 121st out of 185 countries in human development. However, over the last 20 years the rate of improvement in Indonesia\'s HDI ranking has exceeded the world average. This progress may be attributable in part to the fact that Indonesia has put considerable effort into meeting the MDGs. This report is intended to be a contribution toward achieving the Millennium Development Goals.
