Book excerpt: Squamous cell cancers of the head and neck (SCCHN), also known as head and neck cancers (HNC) encompass malignancies of the oral cavity, larynx, nasopharynx and pharynx, and are diagnosed in over 500,000 patients worldwide each year, accounting for 5% of all malignancies. In the past several years, there have been significant developments in understanding of HNC. It is now recognized that although alcohol and tobacco use has represented the likely predominant cause of SCCHN, the incidence of a second class of SCCHN related to oncogenic human papillomavirus (HPV) infection is increasing, with a four-fold increase in the past 2 decades, and now thought to represent up to 30% of cases. The first effective target for SCCHN, the EGFR-targeting antibody cetuximab, was approved as recently as in 2006; since then, a growing body of research has identified additional signaling pathways as important in disease pathogenesis, and in resistance to treatment. Proteins such as c-Met, Src, and HER2 are emerging as new therapeutic targets, with a considerable ferment in the clinical trial community. As a capstone of research progress, 2011 marked the first reports of high throughput sequencing of SCCHN tumors, with these efforts identifying unexpected players such as Notch as frequent subject of mutation, spawning new hypotheses for future research. This book will be of interest to researchers who are interested in better understanding the biology of head and neck cancers, with the goals of better designing therapies, identifying risk factors, or investigating the molecular basis of the disease.

Due to public health efforts encouraging smoking cessation, the overall incidence of HNSCC has decreased in recent years in many countries. In contrast, the incidence of oropharyngeal squamous cell carcinoma (OPSCC) has increased significantly, and this subtype of HNSCC is commonly associated with human papillomavirus (HPV) infection. Moreover, individuals with HPV-positive OPSCC are generally younger and are frequently non-smokers, suggesting that HPV-associated OPSCC represents a distinct biologic entity. They evaluated betel quid with tobacco as carcinogenic to humans (Group 1) on the basis of sufficient evidence of an increased risk of cancer of the oral cavity, pharynx and oesophagus. The working group reviewed epidemiological studies of human cancer, mainly studies from India, Pakistan and Taiwan (China). Studies on betel quid with tobacco and areca nut with tobacco in experimental animals now also provide sufficient evidence of carcinogenicity. The working group also evaluated betel quid without tobacco as carcinogenic to humans (Group 1), on the basis of sufficient evidence of an increased risk of oral cancer. Book excerpt: There is considerable geographic heterogeneity of human papillomavirus (HPV) involvement in head and neck squamous cell carcinoma (HNSCC). This study analyses for the first time the situation in the Philippines. One hundred seventy nine (179) formalin-fixed paraffin-embedded (FFPE) and 22 fresh frozen tissues from 163 rural Filipino cases with histologically-confirmed squamous cell carcinoma of the oropharynx (n=15, 9.2%), the oral cavity (n=88, 54.0%), and of the larynx/hypopharynx (n-60, 36.8%) diagnosed in 2003-2013 were analyzed for HPV DNA by BSGP5+/6+-PCR/MPG and realtime qPCR. HPV16 E6*I m RNA was further analyzed in the HPV DNA-positive [n=2, both non-oropharyngeal squamous cell carcinoma (OPSCC)], all FFPE OPSCC (n=9 DNA valid, n=4 DNA invalid) and additional randomly selected non-OPSCC (n=5 DNA valid, n=8 DNA invalid) samples by reverse transcription -PCR. The HPV DNA-positive, all FFPE OPSCC and randomly selected non-OPSCC (n=9 DNA valid, n=15 DNA valid) samples were evaluated for p16 INK4a and pRb expressions by immunohistochemistry. Serum samples from 22 cases and 20 controls were also analyzed for HPV antibodies by multiplex serology. Only 82 cases had DNA valid samples, of whom 2 (2.4%) were HPV DNA-positive (HPV11, HPV33). All RNA extracts, while Ubiquitin C RNA positive, were HPV16 E6*I transcripts negative. Of 39 tumor tissues two (5%) showed p16 INK4a up-regulation and 3 others (8%) showed pRb down-regulation. None of the serum samples showed HPV antibody patterns with strong anti-E6 or positivity for at least 2 early proteins characteristic for patients with HPV-induced OPSCC. These results indicate that involvement of HPV in HNSCC development among Filipinos if at all existent is very rare. Hence, HPV vaccination may not have any impact on HNSCC prevention among this population.
