In the 1980s, implantable cardioverter defibrillators (ICDs) were introduced. These advances created new challenges for cardiac implantable electronic devices (CIEDs). Right ventricular pacing was the primary breakthrough; however, over the years, it has become apparent that it can induce cardiac contraction dyssynchrony. Biventricular pacing allowed for the alleviation of dyssynchrony and improved the survival of patients with heart failure and bundle branch block. In recent decades, His bundle pacing has become a new strategy for physiological ventricular activation. However, the use of CIEDs carries several risks, e.g., complications related to transvenous leads. This led to the development of percutaneous lead extraction techniques as well as the introduction of a subcutaneous ICD (S-ICD) and leadless pacing. Technological evolution promises an exciting future in the development of cardiac electrotherapy. Book excerpt: Atrial fibrillation is a common arrhythmia, difficult to treat pharmacologically and associated with significant morbidity and mortality. Although traditionally prescribed when pharmacologic therapy is not effective, not tolerated, or contraindicated, electrical therapy should no longer be considered as a treatment of last resort. Implantable devices such as cardiac pacemakers and internal defibrillators capable of monitoring atrial tachyarrhythmias have enhanced understanding of the mechanisms and natural history of atrial tachyarrhythmias. This knowledge allows electrical therapy to alter the natural course of atrial tachyarrhythmias early after onset or create an unfavorable electrophysiological environment in circumstances predisposing to their initiation. Thus, electrical therapies are playing an increasingly important role in the overall management of atrial fibrillation and tachyarrhythmias, often in combination with antiarrhythmic drugs and sometimes with other strategies. New dual-chamber ICDs can safely and effectively differentiate atrial from ventricular tachyarrhythmias and treat atrial tachyarrhythmias without compromising detection and treatment of ventricular tachyarrhythmias. Technological advances in standard electrical cardioversion including the use of prior pharmacotherapy have facilitated the success of the procedure and maintenance of sinus rhythm. This is the first book devoted solely to the rapidly expanding options to treat atrial tachyarrhythmias electrically. Electrical devices offer the promise of prevention and control of atrial tachyarrhythmias, with significant improvements in all health care professionals dealing with atrial fibrillation and other atrial tachyarrhythmias.

Book excerpt: Since the development of pharmacoconvulsive therapy in 1934 and of electroconvulsive therapy (ECT) in 1938, ECT has proven far more valuable than just the intervention of last resort. In comparison with psychotropic medications, we now know that ECT can act more effectively and more rapidly, with substantial clinical improvement that is often seen after only a few treatments. This is especially true for severely ill patients -- those with severe major depression with psychotic features, acute mania with psychotic features, or catatonia. For patients who are physically debilitated, elderly, or pregnant, ECT is also safer than psychotropic medications. The findings of the American Psychiatric Association (APA) Task Force on ECT were published by the APA in 1990 as the first edition of The Practice of Electroconvulsive Therapy, inaugurating the development of ECT guidelines by groups both within the United States and internationally. Since then, advances in the use of this technically demanding treatment prompted the APA to mandate a second edition. The updated format of this second edition presents background information followed by a summary of applicable recommendations for each chapter. This close integration of the recommendations with their justifications makes the material easy to read, understand, and use. To further enhance usability, recommendations critical to the safe, effective delivery of treatment are marked with the designation "should" to distinguish them from recommendations that are advisable but nonessential (with the designations "encouraged," "suggested," "considered"). The updated content of this second edition, which spans indication for use of ECT, patient evaluation, side effects, concurrent medications, consent procedures (with sample consent forms and patient information booklet), staffing, treatment administration, monitoring of outcome, management of patients following ECT, and documentation, as well as education, and clinical privileging. Book excerpt: The first invasive evaluation of cardiac arrhythmias in humans was performed in 1967 in Paris (Prof. P. Coumel) and Amsterdam (Prof. D. Durrer). This was the start of a rapid increase in our knowledge of the diagnosis, mechanism and treatment of cardiac arrhythmias. In that same year Prof. Hein J.J. Wellens became cardiologist in the Wilhelmina Gasthuis in Amsterdam. Initially in Amsterdam (1967-1977) and later on in Maastricht (from 1977), he was the driving force for many breakthroughs in clinical cardiac electrophysiology. With an active interplay between the knowledge derived from the 12-lead electrocardiogram and the recordings made with invasive electrophysiology, he composed new ideas leading to major contributions in clinical cardiac electrophysiology and, more generally, in arrhythmology. He published over 650 scientific papers and 14 books, and had numerous functions within scientific boards of prestigious journals. In addition he trained more than 120 cardiologists in clinical cardiac electrophysiology. On the occasion of the congress `2000, Future of Arrhythmology: Lessons From the Past, Promises For Tomorrow', we highlight the scientific work of Prof. Hein J.J. Wellens. A selection of more than 60 articles over the whole time span has been selected. These articles are accompanied by comments from an expert, co-worker and/or former fellow in order to place the paper in a scientific time frame, including the relationship of the author with Prof. Hein J.J. Wellens.
