While worldwide investigation into the cause and nature of ME/CFS remains disproportionately small, and treatment remains symptomatic and controversial, modest research continues in all aspects of this disease: epidemiology, possible infectious origins and other triggers, possible involvement of genetics, metabolism, and microbiome, influence of co-morbid conditions, and more. Treatment of patients consists of providing symptomatic relief. Guidance in doing so is provided for the clinician. School-age children require not only treatment but, as revealed in a 25-year retrospective study, continued engagement with peers and social activity. This e-book explores the breadth and depth of current ME/CFS research and clinical care. Its impact for other chronic, complex illnesses should not be overlooked.

Book excerpt: After decades of intensive research and over 10,000 publications, preterm birth remains a major global obstetric healthcare problem. Each year, early birth is responsible for the deaths of more than one million infants worldwide and is a major cause of life-long disability. Preterm birth places an enormous financial burden on our healthcare systems, resulting in long-term adverse health outcomes and lost productivity for many people. Preterm birth is a syndrome, associated with several different aetiologies; hence, potential treatment strategies need to be matched to pathophysiology in order to be effective. There is now unequivocal evidence that inflammation is causally involved in a majority of spontaneous preterm deliveries. However, the triggers of inflammation, and the strategies by which it can be safely and effectively prevented and treated, remain the subject of ongoing investigation and debate. While intraamniotic infection is an important cause of inflammation-associated preterm birth, particularly in very preterm deliveries, ‘sterile’ inflammation is actually a more common finding associated with preterm birth. It is likely that the nature, localisation, timing and extent of the inflammatory insult all determine the obstetric outcome and degree of risk to the fetus. These factors will also influence the success of approaches that might be employed to achieve better pregnancy outcomes. Despite our increased understanding of the causes and significance of intrauterine inflammation, we have yet to translate this knowledge into effective therapeutic strategies for preventing prematurity and mitigating its consequences for the neonate. In this Research Topic we review recent progress in treating and preventing inflammation-associated preterm birth, approaching the topic from both the causal and therapeutic perspectives. With global attention increasingly focused on the need to translate knowledge discovery into clinical translation, we hope this EBook will provide a stimulating and timely discussion that will focus research and lead to improved healthcare outcomes for women and children.

Being the port of entry to the gastrointestinal and respiratory tracts, the oral cavity is also constantly challenged by antigens derived from air and food. Moreover, the oral cavity is the sole tissue of the body harboring a hard surface (i.e. the tooth) that is exposed to the hostile external environment, resulting in the formation of a complex biofilm that has local and systemic effects. To deal with such challenges, the oral immune system aims to prevent the invasion of pathogens/harmful antigens and to tolerate non-pathogenic counterparts in order to maintain homeostasis. In recent years, numerous studies have addressed these fundamental issues, revealing sophisticated mechanisms engaged by the immune system to maintain oral mucosal homeostasis and to combat various immunological insults. Some of these studies have identified novel immunological mechanisms, emphasizing the uniqueness of the oral immune system and the necessity to further investigate its functions.

Natural product has been used for thousands of years in the treatment of gastrointestinal diseases including cancer. To discover and develop the natural product with therapeutic selectivity and without toxicity are very important steps in the treatment of GI diseases. Because of their wide range of pharmacologic activities and low toxicity in animal models, many natural products have been used as alternative treatments for gastrointestinal diseases and cancers. Growing evidence has shown that natural products are beneficial for patients with gastrointestinal diseases or cancer, but it is still unconvincing and the mechanism behind the effect has not been fully revealed. In this Research Topic, we emphasize: (1) evaluation of clinical effectiveness of natural products in the treatment of gastrointestinal diseases including cancer; (2) mechanism of action of natural products; (3) translation of natural products into routine practice.
